Quality of prescription of psychotropic drugs in nursing home patients with dementia: need for optimization

PROPER was een onderzoek over het voorschrijfbeleid van psychofarmaca bij bewoners op psychogeriatrische afdelingen van 12 zorginstellingen in Nederland.

Er is ten eerste onderzocht wat de kwaliteit van het voorschrijven was en wat daarbij belangrijke beïnvloedende factoren waren. Ten tweede is de effectiviteit van een gestructureerde medicatiebeoordeling (de PROPER-interventie) op deze kwaliteit van voorschrijven onderzocht. De PROPER-interventie werd uitgevoerd door de specialist ouderengeneeskunde, lokale apotheker en verzorgende. Resultaat
Het bleek dat factoren die in diepte-interviews door betrokken professionals werden genoemd als mogelijk relevant voor psychofarmacagebruik (zoals attitude, kennis en ervaring, communicatie van betrokken professionals) niet als zodanig uit statistische analyses naar voren kwamen.
Uit het effectonderzoek bleek dat de PROPER-interventie tot betere evaluatie en duur van psychofarmacagebruik leidde. De frequentie van het psychofarmacagebruik daalde niet. De betrokken professionals waren tevreden over deze manier van werken en de uitvoering verliep goed. De PROPER-interventie is vrij beschikbaar via de websites van het Universitair Kennisnetwerk Ouderenzorg Nijmegen en het Instituut voor Verantwoord Medicijngebruik.

De ‘PRescription Optimization of Psychotropic drugs in Elderly nuRsing home patients with dementia-study’ bestaat uit twee delen: PROPER-I en PROPER-II.
PROPER-I was een onderzoek over het voorschrijfbeleid van psychofarmaca bij bewoners op psychogeriatrische afdelingen en het verband met bewoners- en omgevingsfactoren. Voor het project is een index ontwikkeld die het mogelijk maakt om een score te geven aan de kwaliteit van voorschrijven van verschillende groepen psychofarmaca voor probleemgedrag bij dementie, de APID index (Appropriate Psychotropic drug use for neuropsychiatric symptoms In Dementia index). Deze index richt zich op 7 domeinen: indicatie, evaluatie, dosis, interacties met andere medicijnen, contra-indicaties, duplicaties (twee geneesmiddelen uit dezelfde groep) en therapieduur. Er zijn bij 12 zorgorganisaties data verzameld op één moment. Van 559 bewoners is het psychofarmacagebruik beoordeeld, het probleemgedrag gescoord en zijn demografische gegevens verzameld. Door 112 verzorgenden zijn vragenlijsten over de werkomgeving, organisatiecultuur, werkdruk en werktevredenheid ingevuld. Ook zijn kenmerken zoals leeftijd en werkervaring verzameld van deze verzorgenden evenals van de 25 artsen die bij de deelnemende afdelingen betrokken zijn. Van 44 psychogeriatrische afdelingen zijn kenmerken verzameld, zoals aantallen bewoners, medewerkers, grootte en inrichting. Daarnaast zijn uitgebreide interviews met vijftien artsen en veertien verzorgenden uit deze 12 zorgorganisaties gebruikt om een conceptueel raamwerk van belangrijke factoren bij psychofarmacagebruik te ontwikkelen.

Van de drie afdelingen per organisatie die deelnamen aan PROPER-I, participeerden twee afdelingen ook aan PROPER-II. De helft van de organisaties is willekeurig toegewezen aan de interventiegroep, de andere organisaties bleven hun gebruikelijke zorg leveren. In de organisaties uit de interventiegroep is een training gegeven over gestructureerde medicatie beoordeling door het Instituut voor Verantwoord Medicijngebruik (IVM) met het doel de kwaliteit van voorschrijven te verbeteren. De medicatiebeoordeling zelf werd uitgevoerd op de afdeling door de specialist ouderengeneeskunde, een verzorgende en de lokale apotheker. Daarnaast zijn er per afdeling twee bijeenkomsten geweest, geleid door het IVM, om de te implementeren medicatiebeoordelingen te evalueren. In alle 6 interventie-instellingen is de interventie succesvol uitgevoerd.

PROPER-II duurde 1,5 jaar, waarbij ieder half jaar een cyclus van meting – bijeenkomst – medicatiebeoordeling werd doorlopen. Daarbij is opnieuw de kwaliteit van het psychofarmacagebruik beoordeeld. Daarnaast zijn vragenlijsten ingevuld over gedragsproblemen, depressieve symptomen, kwaliteit van leven, sociale aspecten en functioneren van de bewoners. Ook zijn vragenlijsten over cognitief functioneren afgenomen, zijn ziekten en ziekenhuisopnamen gedocumenteerd en bijwerkingen van psychofarmaca beoordeeld. In een procesevaluatie zijn de mate en de kwaliteit van de uitvoering van de medicatiebeoordeling onderzocht, de relevantie en haalbaarheid volgens deelnemers evenals belemmerende en bevorderende factoren. Dit vond plaats aan de hand van analyse van notulen van evaluatiebijeenkomsten die onderdeel waren van de interventie, en via een vragenlijst voor betrokken artsen, apothekers en verzorgenden.

In Nederland gebruiken verpleeghuisbewoners met dementie vaker en langer psychofarmaca dan aanbevolen volgens de richtlijnen. Uit eigen onderzoek weten we dat bijna tweederde van de bewoners minstens één psychofarmacon krijgt voorgeschreven. Ook weten we dat de verschillen tussen (afdelingen van) instellingen groot zijn. Zo varieert het gebruik van antipsychotica tussen de 0 en 70% van de bewoners van afdelingen. Mede omdat psychofarmaca tal van ongewenste bijwerkingen hebben, is het wenselijk om het gebruik ervan te beperken. Het gebruik van psychofarmaca is dan ook een belangrijke kwaliteitsindicator binnen de verpleeghuiszorg. In samenwerking met de Inspectie voor de Gezondheidszorg (IGZ), het Instituut voor Verantwoord Medicijngebruik (IVM), ActiZ en het VUmc heeft het UMC St Radboud de door ZonMw gefinancierde Proper-studie opgezet om inzicht te krijgen in de kwaliteit van het voorschrijfbeleid van psychofarmaca en om dit beleid te optimaliseren. Het Proper-project bestaat uit drie delen: deel A, B en C.

Deel A betreft een observationele studie over het voorschrijfbeleid van psychofarmaca bij bewoners van 36 psychogeriatrische afdelingen van 12 verpleeghuisorganisaties en het verband met bewoners- en omgevingsfactoren. De kwaliteit van voorschrijfbeleid van psychofarmaca wordt in een eenmalige meting aan de hand van de medische dossiers van de bewoners beoordeeld door middel van een kwaliteitsindex, speciaal ontwikkeld voor de beoordeling van psychofarmacagebruik. Verder beantwoorden de specialisten ouderengeneeskunde en het verzorgend personeel internetvragenlijsten en worden geïnterviewd door één van de onderzoekers.
Van de drie afdelingen per organisatie die deelnemen aan A, participeren twee afdelingen ook aan deel B, de interventiestudie. De helft van de organisaties is gerandomiseerd tot de interventiegroep. Op de twee deelnemende afdelingen van die organisaties wordt een training over gestructureerde medicatie beoordeling gegeven door het IVM met het doel de kwaliteit van voorschrijven te verbeteren. De medicatiebeoordeling zelf wordt uitgevoerd op de afdeling door de specialist ouderengeneeskunde, verzorgende en lokale apotheker. Deel B duurt 1,5 jaar waarbij ieder half jaar gegevens verzameld worden.
Deel C betreft verspreiding en implementatie van de resultaten. Ook ontvangen de organisaties die in de controlegroep zijn ingedeeld de training van het IVM, om ook de methodiek van medicatiebeoordeling te kunnen implementeren.

Samenvatting van de aanvraag

INTRODUCTION Dutch nursing home patients with dementia use psychotropic medication more frequent and longer than recommended by guidelines. Since psychotropic drug use is one of the quality indicators in nursing home care, the awareness of the need for optimizing psychotropic drug use is increasing. However, knowledge about factors related to the quality of prescription is scarce. Moreover, it is hypothesized that psychotropic drug prescription can be optimized to increase its appropriateness, by a structured repeated medication review using a multidisciplinary approach. OBJECTIVES/ RESEARCH QUESTIONS The aim is to (A) study the quality of prescription of psychotropic drugs in nursing home patients with dementia and the relationship with patient’s and environmental correlates and (B) to study the efficacy of a structured and repeated medication review on the quality of psychotropic drug prescription and (C) to transfer the knowledge to other nursing homes, relevant participants and stakeholders. STUDY DESIGN The study consists of three parts: (Part A:) A cross-sectional observational study to search for patient’s and environmental (including physician-related) correlates regarding the variation in prescription-patterns of psychotropic drugs. Patients with dementia (n=540), nursing staff and nursing home physicians (n=70-100) of 36 dementia special care units (SCU) of 12 nursing homes will be included, 6 with high rates and 6 with low rates of psychotropic drug use. The dependent variables are the frequency and appropriateness of psychotropic drug prescription (antipsychotics, anxiolytics, hypnotics, antidepressants, anticonvulsants, anti-dementia drugs). Possible correlates are neuropsychiatric symptoms (NPS), number of patients with physical restraints, psychosocial interventions, physical/ psychosocial environment, culture, and workload. Data of psychotropic drug prescription are derived from medical charts using a checklist, largely based on current guidelines on neuropsychiatric symptoms. Since the drug prescription to patients may be related to environmental or physician-related factors, we use multilevel regression analysis to study the influence of the possible correlates on the frequency/ appropriateness of psychotropic drug prescription (the levels being patients and SCU). (Part B:) An intervention study investigating the efficacy of a structured and repeated medication review on the quality of psychotropic drug prescription (frequency of appropriate prescription) in nursing home patients with dementia. Two out of three SCUs from the nursing homes engaged in part A are addressed to participate. Patients with dementia of 24 SCUs from 12 nursing homes (n=360) are included. The study is a 18-month clustered randomized controlled pragmatic trial with 6 monthly measurements. Randomization is on the level of the nursing home. The control group receives care as usual. The intervention group receives a structured repeated medication review at 3 months interval accompanied by 4 education and training sessions, carried out by the Dutch Institute for Rational Use of Medication (IVM). The primary outcome is the frequency of appropriate psychotropic drug prescription. Secondary outcomes are: the frequency of psychotropic drug prescription, adverse events (cognition, motor symptoms, activities of daily living, falls, mortality, drug related hospital admissions), neuropsychiatric symptoms and quality of life. A process analysis is carried out for facilitators and barriers regarding the introduction of the medication review. A number of 360 patients is large enough to detect a 20% increase in appropriate psychotropic drug use. Multilevel regression analysis will be used to compare changes in frequency of appropriate psychotropic drug use between baseline and after 6, 12, and 18 months between the intervention SCUs and control SCUs, after correction for relevant covariates/ confounders, such as baseline psychotropic drug use, NPS. (Part C:) Knowledge transfer and implementation in Dutch nursing homes. Based on the results of the process analysis from part B, the training and educational materials will be modified and implemented in the 6 control homes. The results of part A (the analysis about the quality of prescription and its correlates) will be used in the National steering committee on professional care - with members of nursing home organizations and the Dutch Health Care Inspection - to further improve the validity of the quality indicators . The results and experience regarding the structured and repeated medication review will be used to make a comprehensive protocol for elderly care physicians and pharmacists in other nursing homes. This material will be further conveyed to the specialist postgraduate training of elderly care physicians and training of nurses. TIME SCHEDULE: Part A: 1-12 months, Part B: 13-48 months, Part C: 44-48 months. BUDGET: 591.719 euro.
