COURAGE-PD: Comprehensive Unbiased Risk Factor Assessment for Genetics and Environment in Parkinson's Disease

Ondanks de vooruitgang in de identificatie van genen die betrokken zijn bij de ziekte van Parkinson (PD), zijn er nog steeds aanzienlijke lacunes in ons begrip van de mechanismen die ten grondslag liggen aan het neurodegeneratieve proces en de relatie ervan tot omgevingsfactoren in PD. 

Aanpak

Daarom stellen we een aanpak voor gebaseerd op;

  1. een unieke verzameling van families met autosomaal dominant en autosomaal recessieve PD;
  2. grote cohorten van klinisch goed gedefinieerde sporadische PD-patiënten van verschillende populaties wereldwijd; 
  3. genetische studies en;
  4. studies naar omgevingsmodificatoren gevolgd door
  5. functionele validatiestudies in cellulaire modellen van patiënten. 

We zullen Next Generation Sequencing strategieën inclusief exome sequencing in multiplex families en targeted re-sequencing in sporadische PD-patiënten gebruiken om de complexe genetische architectuur van PD te ontwarren in verschillende populaties en zullen proberen de onderliggende functionele varianten in ziekte-geassocieerde GWAS loci te identificeren. Nieuw geïdentificeerde genetische varianten worden gefilterd op pathogene relevantie gebaseerd op nieuwe voorspellingsalgoritmen gecombineerd met unieke expressiedatabases en gerepliceerd in grote cohorten PD-patiënten. Hier biedt de genetische epidemiologie van het GEO-PD consortium een unieke hulpbron met een groot aantal DNA-monsters en gegevens over omgevingsfactoren van PD-patiënten en controles uit verschillende populaties wereldwijd.

Resultaten 

We gebruikten Next Generation Sequencing strategieen, waaronder Whole Genome Sequencing bij 180 personen uit 117 multiplexfamilies, en hebben onze krachten gebundeld met collega's in de VS en daarbuiten om het grootste internationale consortium te produceren in het onderzoek naar de ziekte van Parkinson, dat in de afgelopen 18 maanden 8178 exomen heeft gesequenced (2835 gevallen) en 5343 controles) en alle significante loci uit de nieuwste meta-analyse in PD werden opnieuw geëvalueerd in 2310 PD-gevallen en 2290 controles van verschillende Europese en Aziatische populaties.
Op basis van de resultaten van de sequencing hebben we vervolgens een nieuwe genotyperingsarray ontwikkeld, de NeuroChip, in samenwerking met Illumina Inc. In 2015 hebben we in totaal 29.390 varianten aan Illumina aangeboden. Gezien het beschikbare budget kon het consortium 21.400 chips beveiligen, inclusief genotyperingkosten. Verdere 5.000 chips werden gedaan in samenwerking met en met financiering van Amerikaanse collega's (Dr. Singleton, NIA).

Samenvatting van de aanvraag

Despite the advances in the identification of genes involved in Parkinson’s disease (PD), there are still appreciable gaps in our understanding of the mechanisms underlying the neurodegenerative process and its relation to environmental factors in PD. Therefore we are proposing a comprehensive approach based on (i) a unique collection of families with autosomal dominant and autosomal recessive PD and (ii) large cohorts of clinically well-defined sporadic PD patients from different populations worldwide for (iii) genetic studies and (iv) assessment of environmental modifiers that will translate into (v) functional validation studies in patient-derived cellular models. Using next generation sequencing strategies including exome sequencing in multiplex families and targeted resequencing in sporadic PD patients, we will disentangle the complex genetic architecture of PD in different populations and attempt to better define the underlying functional variants in disease-associated GWAS loci. Newly identified genetic variants are filtered for pathogenic relevance based on novel prediction algorithms combined with unique expression databases and replicated in large cohorts of PD patients . Here the Genetic Epidemiology of Parkinson’s disease Consortium (GEO-PD) provides a unique resource with a large number of DNA samples and environmental exposure data of PD patients and controls from different populations worldwide. Subsequent assessment of disease modifiers includes two complementary approaches: Mendelian randomization , and gene-environment interaction studies . In order to validate genetic risk variants, functional studies on patient-based material will be performed. Here the applicants provide unique expertise for fibroblasts- and induced-pluripotent-stem-cells-(iPSC)-derived cellular models of PD and a large repository of biomaterials from carriers of PD-associated mutations. Established readouts allow to study functional effects of identified genetic risk factors and will be used to assign novel disease genes and risk variants to defined pathogenic pathways. Moreover patient-based cellular models will be challenged with environmental risk factors identified as modulators of disease . We expect that the combination of comprehensive state-of-the-art genetic technologies with a detailed ascertainment of environmental modifiers will provide important clues to decipher the complexity of neurodegeneration in PD. Subsequent modelling of PD in patient-based material allows to discover molecular mechanisms and pathways involved and leading to therapies for this still incurable disease.
