Tacrolimus voor Auto-Immuun Hepatitis met incomplete response op standaardbehandeling: een gerandomiseerde studie

Bij auto-immuun hepatitis (AIH) kan aanhoudende ontsteking van de lever leiden tot levercirrose. De noodzaak tot levertransplantatie is kleiner naarmate de leverontsteking beter onderdrukt is. Met de standaardbehandeling prednisolon en azathioprine lukt dit bij 20-50% onvoldoende.

Onderzoek

Doel van dit onderzoek is om te bepalen of mycofenolaat mofetil (MMF) of tacrolimus (TAC) (geregistreerd voor behandeling na transplantatie) de beste behandeling is voor patiënten met AIH. In de Europese AIH richtlijn zijn deze geneesmiddelen gesuggereerd indien de standaardbehandeling onvoldoende effect heeft. Dit is echter nooit onderzocht in gecontroleerde studies.
In de gerandomiseerde TAILOR studie wordt onderzocht of TAC dan wel MMF effectiever en veiliger is als tweedelijns behandeling van patiënten met AIH die na een jaar behandeling geen complete remissie hebben bereikt. De ene helft van de patiënten krijgt TAC, de andere helft MMF.

Verwachte uitkomsten

Als primaire uitkomstmaat wordt gekeken naar het deel van de patiënten met complete remissie 12 maanden na de start van de behandeling. Daarnaast wordt onder andere gekeken naar kwaliteit van leven, bijwerkingen en kosteneffectiviteit.

Het onderzoek wordt uitgevoerd in samenwerking met alle academische en een aantal algemene Nederlandse ziekenhuizen, verenigd in de al meer dan 10 jaar bestaande Nederlandse Auto-Immuun Hepatitis Werkgroep.

Samenvatting van de aanvraag

Autoimmune Hepatis (AIH) is a chronic inflammation of the liver caused by autoimmunity. Without treatment the prognosis was dismal. However since the 1970s treatment with prednisolone and azathioprine became standard therapy, with major improvement of survival. Although serum aminotransferases (AST and ALT) as a reflection of hepatitis, and serum immunoglobulin G (IgG) as reflection of immunological activity, improve in 90% of patients, these parameters are still elevated in 50% after one year of treatment, and in 20% after three years. This can lead to development of cirrhosis despite treatment. First goal of treatment is complete biochemical remission (CR, defined as normal AST, ALT and IgG), since this is associated with better liver-transplant-free survival compared to patients with no CR. Patients with CR have less progression to cirrhosis, and the one-third of patients who already have cirrhosis on diagnosis develop less decompensation compared to those without CR. In case of intolerance for azathioprine or for insufficient response to azathioprine other thioguanins -like mercaptopurine or thioguanine-, but also mycophenolate mofetil (MMF) or tacrolimus (TAC) have been used with success in a proportion of patients. Especially MMF and TAC are recommended in guidelines as second-line therapy. In previous reports no difference was made in indication for use of MMF or TAC, leading to a wide range of success rates. If we make this difference it seems that for intolerance to azathioprine MMF and TAC are equally effective, while for insufficient reponse to azathioprine TAC seems more effective than MMF. The aim of the current study is to compare TAC and MMF for one year in AIH patients with incomplete response (no CR) with at least 6 months of first-line treatment. Primary outcome is the proportion of patients with complete biochemical remission (CR). Improvement of AST and ALT, fibrosis parameters, quality of life, and cost-effectiveness will also be evaluated.
