Resultaten
De Persoonlijke Antipsychotica Keuzewijzer – PAKwijzer – is een online keuzehulp voor patiënten die gebruikmaken of gaan maken van antipsychotische medicatie. Het doel van de PAKwijzer is patiënten te betrekken en te ondersteunen bij het maken van een keuze voor een antipsychoticum. Dit kan helpen om betere beslissingen omtrent antipsychotica te maken en de medicatietrouw bij behandeling met antipsychotica te verbeteren.

De PAKwijzer is ontwikkeld door de 15 meest voorgeschreven antipsychotica in Nederland met elkaar te vergelijken op de belangrijkste effecten en bijwerkingen. In de keuzehulp op www.PAKwijzer.nl geeft de patiënt aan hoe belangrijk hij 3 verschillende effecten en hoe acceptabel hij 15 veelvoorkomende bijwerkingen van antipsychotica vindt. Dit resulteert in een persoonlijke rangorde van antipsychotica, die de patiënt vervolgens kan opslaan/afdrukken om met zijn behandelaar te bespreken en gezamenlijk voor een bepaald antipsychoticum te kiezen.

Eindverslag

De Persoonlijke Antipsychotica Keuzewijzer (PAKwijzer) is een online tool bedoelt om patienten te helpen bij de keuzes rondom antipsychotica. Door de online tool in te vullen krijgt de patient een persoonlijke rangorde van meer of minder geschikte antipsychotica. Tijdens het ontwikkelingsproces van de PAKwijzer hebben patiënten een centrale rol gespeeld. Allereerst is er in kaart gebracht welke bijwerkingen voor patiënten het meest relevant zijn. Vervolgens zijn van de 12 meest voorgeschreven antipsychotica in Nederland rangordes gemaakt van effect en bijwerkingen. Voor gewenste effecten krijgen antipsychotica die hoog in de rangorde staan op dat effect, meer punten dan antipsychotica die lager in de rangorde staan. Voor bijwerkingen krijgen antipsychotica die hoog in de rangorde staan meer strafpunten dan antipsychotica die lager in de rangorde staan. De patiënt bepaalt door aan te geven hoe hij belangrijk hij elk effect en hoe acceptabel hij elke bijwerking vindt, hoe zwaar een effect of bijwerking wordt meegenomen in de berekening van zijn persoonlijke rangorde. De PAKwijzer bleek gebruiksvriendelijk en accuraat in de praktijk. Patiënten waren tevreden over de PAKwijzer en het gebruik had een gunstig effect op de communicatie tussen patiënt en behandelaar.

Patiënten met een psychose kunnen nu moeilijk informatie vinden over antipsychotica die rekening houdt met hun individuele situatie en voorkeur.

Doel
In samenwerking met clinici en patiënten ontwikkelen en invoeren van een web-based PAKwijzer, om patiënten te voorzien van informatie over de antipsychotische medicatie, zodat ze kunnen deelnemen aan de beslissing over hun behandeling.

De volgende stappen zijn gevolgd:

1.In samenwerking met patiënten zal een lijst worden opgesteld met selectiecriteria voor een effectieve en aanvaardbare antipsychotische medicatie.
2.In samenwerking met de Multidisciplinaire Richtlijn Commissie Schizofrenie zal per selectiecriterium een ranglijst van meer tot minder geschikte antipsychotica worden opgesteld.
3.Een online 'tool' zal worden ontwikkeld die op basis van de individuele voorkeur van patiënten een rapport genereert waarin de beste opties voor antipsychotica worden beschreven.

Samenvatting van de aanvraag

Antipsychotic medication is effective in reducing symptoms and preventing relapses in patients with psychotic disorders. These patients are however often reluctant to start pharmacotherapy, and once treatment has started, up to 70% of them become non adherent. When patients are more involved in decisions concerning their treatment and when they feel taken serious by their practitioners, treatment recommendations are followed better. Good collaboration between patients and clinicians in the choice of an antipsychotic agent may improve adherence and thereby the long term outcome of treatment. To become an influential partner in decisions concerning the choice of an antipsychotic agent, patients need valid and focussed information about medication. Currently the majority of patients lack such information. We aim to improve the involvement of patients in choosing their antipsychotic therapy. To this end we will provide patients with information concerning their medication in a way that will enable them to weigh different pharmacotherapeutic options by means of a ranking list of medication based on their individual preferences. We plan to develop an online tool called the Personal Antipsychotic Choice Index [PAC Index, in Dutch: Persoonlijke Antipsychotica Keuzewijzer: PAKwijzer]. To develop this tool we will first compose a list with the 10 criteria patients consider to be most important for the choice of an antipsychotic agent. Second, based on all evidence available, we will put together a ranking list of antipsychotic medication for each selection criterion. This ranking list will be updated when new research findings or new antipsychotic drugs are available in the Netherlands. Third, we will develop an online instrument that composes an individualized ranking list of antipsychotic medications in order of suitability based on the individual importance scores for their respective selection criteria taking into account potential drug interactions with medication already used by the patient. Thereafter we will evaluate the PAC Index in several patient populations. Usabililty of the tool will be assessed in a small sample of service-users. Next the clinical accuracy (or clinical appropriateness) of the advise given by the PAC Index will be evaluated by an independent expert group [including patients]. Then, a broad group of patients will be asked to value the contribution of PAC Index for the consultation. The impact on the communication between patient and profesonial will be assessed using a standardized method. Implementation of the PAC Index will start after a favorable evaluation of the tool. Implementation is feasible since we have broad support of several mental health care institutions, patients associations, national knowledge centers, and an association of professionals. We expect that using the PAC Index will improve involvement of patients in treatment decisions and thereby increase adherence and diminish psychotic relapse and associated suffering and costs [even a relatively minor increase in adherence [10%] is associated with about 25% less psychotic relapse].
