Operatie of gips voor een gebroken pols? Wat is de beste behandeling boven een leeftijd van 65 jaar?
Een gebroken pols komt vaak voor bij ouderen. Op een gebroken heup na is het de meest voorkomende breuk in deze leeftijdscategorie. De breuklijn loopt regelmatig door het gewrichtsvlak van de pols. Dan is niet bekend wat de beste behandeling is. Een behandeling met gips en een operatie hebben allebei voor- en nadelen. Bij een operatie kan de breuk goed op zijn plek worden vastgezet. Een operatie heeft wel risico’s. Ook is onduidelijk hoe belangrijk het is dat de breuk perfect wordt hersteld. Het is mogelijk dat een afwijkende stand op de röntgenfoto voor een patiënt geen belemmering vormt in het dagelijks leven.
De beste behandeling is dus niet bekend. De beste manier om hierachter te komen is doormiddel van een studie waarbij patiënten worden geloot tussen operatie en gips. Door het nauwkeurig volgen van patiënten met vragenlijsten en röntgenfoto’s worden de behandelingen vergeleken. Resultaten worden verwacht in voorjaar 2024.

Samenvatting " DOEL/VRAAGSTELLING Het beoordelen van de kosteneffectiviteit van operatieve behandeling in vergelijking met niet-operatieve behandeling voor ouderen met een intra-articulaire distale radius fractuur. " HYPOTHESE Niet-operatieve behandeling (gips) is niet inferieur aan operatieve behandeling (open reductie, interne fixatie (ORIF)) gemeten in patiënt gerapporteerde uitkomstmaten (PROMs). " STUDIE OPZET Non-inferioriteit design. Een gerandomiseerde multicenter trial met een kosteneffectiviteit analyse. " STUDIEPOPULATIE/DATABRONNEN Patiënten tussen 65-85 jaar met een intra-articulaire (AO Type C) distale radius fractuur. " INTERVENTIE (experimenteel) Niet-operatieve behandeling (gips). " STANDAARD WAARMEE DE INTERVENTIE WORDT VERGELEKEN Operatieve behandeling (ORIF) " UITKOMSTMATEN De primaire uitkomst is de na 1 jaar door de patiënt gerapporteerde functie van de pols: de Patient-Rated Wrist Evaluation (PRWE). Ook de secundaire uitkomsten zijn door de patiënt gerapporteerde uitkomstmaten: de Disability van de Arm, Schouder en Hand (DASH), kwaliteit van leven (EQ-5D), het catastroferen van pijn (PCS). Verder worden radiologische parameters en complicaties geëvalueerd. " SAMPLE SIZE BEREKENING/DATA-ANALYSE Gebaseerd op een non-inferioriteit design, een power van 90%, een alfa van 0.025, een standaard deviatie van 23 punten, een verlies van 20% tijdens follow-up en een minimaal klinisch belangrijk verschil (MCID) van 14 points gescoord met de PRWE(1, 2) als de primaire uitkomstmaat, moeten er in totaal 154 patiënten worden geïncludeerd. " KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Als gipsbehandeling niet inferieur is aan een operatie, verwachten we een jaarlijkse kostenbesparing van € 2.7 miljoen. " TIJDPAD 42 maanden: voorbereiding 6 maanden, inclusie 18 maanden follow-up 30 maanden (loopt eerste 1,5 jaar parallel met inclusie), analyse 6 maanden, implementatie 12 maanden (loopt parallel met analyse en laatste 6 maanden van de follow-up). " HYPOTHESIS Non-operative (closed reduction and casting) is not inferior to operative (open reduction and internal fixation (ORIF)) in terms of patient-reported outcome measurements (PROMs). " STUDY DESIGN Non-inferiority design. Economic evaluation alongside a randomized controlled multi-center trial. " STUDY POPULATION (S)/DATASETS Patients aged between 65 and 85 years with intra-articular (AO Type C) distal radius fractures. " INTERVENTION (experimental) Non-operative treatment (casting). " STANDARD INTERVENTION TO BE COMPARED TO Operative treatment (open reduction, internal fixation (ORIF)). " OUTCOME MEASURES The primary outcome will be evaluated after 1 year with the Patient-Rated Wrist Evaluation score (PRWE). Secondary outcomes comprise other PROMs including the Disability of the Arm, Hand and Shoulder (DASH), Quality of life (EQ-5D), Patient health (SF-12), Pain Catastrophizing Scale (PCS). Radiographic parameters and Complications will be evaluated. " SAMPLE SIZE CALCULATION/DATA ANALYSIS Based on a non-inferiority model, a power of 90%, an alpha of 0.025, a standard deviation of 23 points, a 20% loss to follow-up and a minimal clinically important difference (MCID) of 14 points based on the PRWE(1, 2) as primary outcome measure, 154 patients need to be included. " COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS If non-operative treatment is non-inferior to operative treatment in improving patient-reported wrist function, we expect to generate an annual healthcare cost reduction of € 2.7 million.
