Permissive weight bearing in trauma patients with fracture of the lower extremities: prospective comparative cohort study.

Herstel na een botbreuk is afhankelijk van een aantal factoren, zoals genezing van de breuk en de plaats van de botbreuk. Verder is het herstel afhankelijk van de behandeling die iemand krijgt. Standaard wordt na een operatie een been of voet 6-12 weken niet belast. In deze studie is onderzocht of sneller belasten leidt tot een sneller herstel met een betere kosteneffectiviteit en dat vroeg belasten veilig is. Vroeg belasten is niet gewoon staan en lopen, maar gebeurt volgens duidelijke regels. Dit concept kennen we als Permissive Weight Bearing (PWB).
Deze studie liet zien dat het vroegbelasten op geleide van klachten van de patiënt als nabehandeling van onderbeenfracturen leidde tot vroegere en betere belasting ten opzichte van standaard zorg. Tevens verbeterde het looppatroon, de kwaliteit van lopen en trad er een eerder hervatting op van (werk) activiteiten. Het permissive weight protocol (PWB) is kosteneffectief gebleken ten opzichte van de standaard zorg. Indien PWB een standaard nabehandeling zou zijn, bespaart dit Nederland op jaarbasis 7 miljoen euro en 2.5 miljoen euro vanuit het perspectief van de zorgverzekeraar.

Samenvatting van de aanvraag
DOEL/VRAAGSTELLING In welke mate verschilt het functionele herstel in traumapatiënten 6 maanden na een fractuur van de onderste extremiteit bij een nabehandeling met een nieuw ontwikkeld ‘permissive weight bearing (PWB) protocol’ ten opzichte van de behandeling protocol zoals beschreven in de richtlijnen? HYPOTHESE Patiënten die het PWB protocol volgen hebben A; een betere ADL (gemeten met de Lower Extremity Functional Scale (LEFS)), B; een beter functioneel herstel (gemeten met de Brunnstrom Fugl-Meyer (BFM) test), C; een betere participatie (gemeten met de SF36) en D; een betere kwaliteit van leven (gemeten met de EQ-5D-5L) in de eerste 6 maanden postoperatief dan patiënten die het huidige protocol volgen. We hebben gekozen voor deze niveau’s om alle grote uitkomstmaten volgens het ICF model te coveren.(1) STUDIE OPZET Prospectieve comparatieve cohort studie STUDIEPOPULATIE/DATABRONNEN Traumapatiënten ouder dan 18 jaar met een bekken/acetabulum of (peri)- of intra-articulaire fractuur van de onderste extremiteiten, waar PWB niet toegestaan is in de eerste 6 weken postoperatief volgens de huidige richtlijnen. INTERVENTIE PWB protocol GEBRUIKELIJKE ZORG/VERGELIJKING Nabehandeling volgens het huidige gangbare protocol, waarbij fracturen langdurig niet belast of hoogstens partieel belast worden. UITKOMSTMATEN Primaire uitkomstmaat: Niveau van ADL, gemeten met LEFS Secundaire uitkomstmaten: Functie (BFM test), participatie (SF-36), kwaliteit van leven (EQ5D5L), maatschappelijke kosten, complicaties. SAMPLE SIZE BEREKENING/DATA-ANALYSE In de PWB groep verwachten we een toename van 10% of meer in LEFS. Alfa van 5% en bèta van 20% (80% power), moet de sample size zijn 51 voor beide groepen. Uitgaande van een drop out van 20% en 10% meer patiënten om gelijke groepen te krijgen moeten er in totaal 134 patiënten worden geïncludeerd. KOSTENEFFECTIVITEITSANALYSE/BUDGET IMPACT ANALYSE Een economische evaluatie zal worden uitgevoerd vanuit het ziekenhuis en maatschappelijk perspectief, waarbij de PWB wordt vergeleken met de gebruikelijke zorg. Een budget impact analyse zal worden uitgevoerd om de financiële consequenties van beide protocollen in kaart te brengen. TIJDPAD 0-3 maanden: voorbereiding, inclusief trainingen voor deelnemende fysiotherapeuten om gestandaardiseerde instructies voor de patiënten in beide groepen (PWB vs usual care) te waarborgen. 3-15 maanden: Inclusie van de patiënten. 16-24 maanden: Follow-up, analyse en disseminatie. OBJECTIVE(S)/RESEARCH QUESTION(S) How do the current guidelines and a newly developed permissive weight bearing (PWB) protocol compare as to early recovery of functional outcome in trauma patients with fractures of the lower extremities after 6 months? HYPOTHESIS Hypothesis 1: Patients have A; better outcome at activity level (as measured with the Lower Extremity Functional Scale (LEFS)), B; better early recovery at function level(as measured with the Brunnstrom Fugl-Meyer (BFM) test), C; better participation (as measured with the SF-36) and D; a better quality of life (as measured with the EQ-5D-5L) in the first 6 months post-surgery when they are treated according to the PWB protocol compared to patients treated according to standard current guidelines. We have chosen these scales to cover the major outcome levels in the ICF model.(1) STUDY DESIGN Prospective comparative cohort study STUDY POPULATION(S)/DATASETS Patients older than 18 years with a surgically treated fracture of the pelvis/acetabulum or a (peri)- or intra-articular fracture of the lower extremities, where PWB would not be allowed in the first weeks post-operatively according to the current guidelines. INTERVENTION PWB protocol USUAL CARE/COMPARISON Treatment according the current guidelines, patients will have a non or restricted partial weight bearing treatment. OUTCOME MEASURES Main outcome variable: ADL with LEFS Secondary outcome variables: Function (BFM test), Participation (SF-36), Quality of life (EQ5D5L), Health and societal costs, complications. SAMPLE SIZE CALCULATION/DATA ANALYSIS In this study we consider an increase in LEFS of 10% or more in the PWB treatment group. Alpha of 5% and beta of 20% (power 80%), the sample size should be at least 51 for both groups. Anticipating a 20% drop out and 10% more patients to obtain equal groups a total of 134 patients have to be recruited. COST-EFFECTIVENESS ANALYSIS/BUDGET IMPACT ANALYSIS An economic evaluation from the hospital and societal perspective will be performed to compare the PWB to the usual care. In line with Dutch guideline, a BIA will be performed. This analysis will be performed from the health care budgetary and societal perspective. TIME SCHEDULE 0-3 months: all necessary preparations will be done. Joined training sessions for physical therapists will be organized to ensure standardized instructions for patients in both groups (restricted or permissive). 3-15 months: Inclusion of patients. 16-24 months: Follow-up, data analysis, report and implementation.
