Implementing a routine outcome monitoring system to study the effectiveness of long-term psychoanalytic treatment and therapeutic decision making in four cohorts of personality disordered and/ or recurrent or chronic depressive patients

Psychoanalyse en psychoanalytische psychotherapie zijn behandelvormen die doorgaans lang duren (tot wel 5 jaren). In Amsterdam is onderzocht hoe effectief beide behandelvormen zijn op een afname van de psychische klachten, op het kunnen aangaan van sociale contacten en op veranderingen in de persoonlijkheid van de patiënt. Hiertoe werden deze parameters gemeten bij patiënten die zich in verschillende stadia van de psychoanalyse of psychoanalytische psychotherapie bevonden (begin, 1 jaar na aanvang, einde van behandeling en 1-2 jaar na einde behandeling). De gemeten gegevens werden vergeleken met veranderingen in de tijd van deze gegevens bij onbehandelde patiënten. Psychoanalyse en psychoanalytische psychotherapie blijken de klachten en de sociale onaangepastheid te verminderen en het aangaan en onderhouden van contacten te verbeteren. De behandelingen zijn beide kosteneffectief, waarbij psychoanalyse duurder is dan psychoanalytische psychotherapie.

The proposed project is a collaboration of four mental health institutes who aim to study the effectiveness of ambulant psychoanalytic psychotherapy and psychoanalysis in regular clinical practice in patients suffering from personality disorders and/ or recurrent or chronic depression. This will be accomplished using a routine outcome monitoring system especially developed for long-term psychotherapeutic treatments. In addition, the project evaluates the usefulness of the monitoring system in producing quality indicators and transparency in therapeutic decision making. The effectiveness will be studied in four cohorts that differ in the phase in which the patients are in treatment: before, at one year, at the end, and one to two years after treatment (follow-up). The patients in each cohort will be measured at three points in time with a six-month interval. These cohorts will be compared on clinically, theoretically and empirically important outcome and process measures. In this way, results can be obtained in a comparatively short period over a long-term treatment. We expect psychoanalytic treatment to be effective in reducing the patient's symptomatology and to improve the patient's interpersonal and intrapsychic functioning and overall well-being to a non-clinical level after 3 to 5 years of psychoanalytic treatment with dose and frequency of sessions per week correlating positively with improvement rates. Furthermore, we expect psychoanalytic treatment to be cost-effective in comparison to health costs for patients with similar pathology who do not receive psychotherapeutic treatment (natural remission). Finally, we expect to construct meaningful quality indicators for long-term psychotherapeutic treatment and to find that clinicians and administrators will start to use these indicators to guide their decision making. Het voorgestelde project is een samenwerking tussen vier GGZ-instellingen dat tot doel heeft de effectiviteit van ambulante psychoanalytische psychotherapie en psychoanalyse in de reguliere praktijk te meten bij patiënten met persoonlijkheidsstoornissen en/ of chronische of recidiverende depressiviteit. Effectiviteit wordt nagegaan met behulp van een cliënten volgsysteem speciaal ontwikkeld voor langdurige psychotherapeutische behandelingen dat routinematig zal worden ingezet. Verder zal het project de bruikbaarheid van het cliënten volgsysteem evalueren in het maken van kwaliteitsindicatoren en het verbeteren van de transparantie in klinische besluitvorming. Effectiviteit zal worden onderzocht in vier cohorten die verschillen in de fase waarin de patiënten zich in de behandeling bevinden: voorafgaande, 1 jaar na aanvang, aan het eind en 1-2 jaar na afloop van de behandeling. De patiënten in elk cohort zullen worden gevolgd op drie momenten in de tijd met zes maanden ertussen. De cohorten zullen worden vergeleken op belangrijke klinische, theoretische en empirische uitkomst- en procesmaten. Op deze manier kunnen resultaten worden verzameld in een relatief korte periode over de effectiviteit van een langdurige behandeling. We verwachten dat psychoanalytische behandeling effectief zal zijn in het verminderen van de symptomatologie van de patiënt en het verbeteren van het interpersoonlijk en intrapsychisch functioneren van de patiënt in die mate dat diens algemene welbevinden op een niet-klinisch niveau komt na 3 tot 5 jaar behandeling waarbij frequentie van de sessies per week waarschijnlijk positief correleert met de mate van verbeteringen. Verder wordt verwacht dat de psychoanalytische behandeling kosten effectief (doelmatig) is in vergelijking met gezondheidskosten van patiënten met gelijksoortige problematiek die geen psychotherapeutische behandeling krijgen. Tot slot, verwachten we betekenisvolle kwaliteitsindicatoren te construeren voor langdurige psychotherapeutische behandelingen en te vinden dat therapeuten en andere gebruikers de indicatoren steeds vaker bij hun beslissingen zullen gaan betrekken.
