Efficient designs in cluster randomized trials: fewer clusters by including baseline outcome as a covariate?

Als niet duidelijk is welke van twee behandelingen het beste is, dan vergelijk je een groep patiënten op de ene behandeling met een groep patiënten op de andere behandeling. Hoe meer patiënten in zo’n studie, hoe duidelijker het verschil tussen de behandelingen te zien is. Maar als het aantal clusters patiënten beperkt is, is de vraag of je voldoende informatie uit je studie gaat halen.

Dit kan worden ondervangen door een voormeting te doen. Analyse methoden die meer statistische power hebben, zoals het corrigeren voor een voormeting, vereisen vaak minder clusters. Hiervoor zijn empirische schattingen nodig van de correlatie tussen voor- en nameting en een formule of computer programma, om uit deze twee gegevens de reductie in het aantal clusters te berekenen. We zullen aangeven wanneer een voormeting zinvol is; specifiek voor behandelingen die ingevoerd worden per ‘cluster’ patiënten. Bijvoorbeeld het wel of niet invoeren van een nieuw hygiëne protocol op verpleegafdelingen.

Problem statement:
Ways to improve the quality and organization of care often concern training complete health care professionals or units. For example, recently a patient-tailored state of the art program for occupational therapy working with clients at home has been developed. The question arises which implementation strategy is best to train occupational therapists to work with this program: mere education or a combined strategy of education, web-based monitoring, feedback and reminders. In such case, the implementation strategy aims at the institutes in which the occupational therapists work, but the effects are measured on their clients. The clients within one institute form a ‘cluster’ and comparing two implementation strategies comes down to comparing two groups of clusters: clusters of clients from institutes that had the educational-only strategy or the combined strategy. This is an example of a cluster randomized trial with as clusters the clients within one institute.

The cluster randomized trial with a concurrent economic evaluation is nowadays considered the gold
standard evaluative design for research into the organization of care. For implementation research situated in hospitals and other secondary care settings, the number of health care units (‘clusters’) available is often limited (e.g. the nursing homes, hospitals or clinics within a region is limited). A consequence of a small number of clusters is that the (statistical) power to detect differences in implementation strategies is low and trials become unfeasible.

Often a baseline measurement is available and it can be informative of the outcome at the end of the trial. We want to investigate whether and to what extent including this baseline measurement in the analysis reduces the number of clusters needed to detect relevant effects.

Results:
We found that the number of clusters can be reduced by a factor r2 where r is the information fraction that the baseline measurement contains about the end measurement (i.e. the correlation of the cluster means between baseline and end measurement).
If r is at least 0.3 (small), 0.5 (moderate), or 0.7 (large), the number of clusters can be reduced by 10%, 25% or even 50%.

Conclusion:
The number of clusters needed can be (substantially) reduced, depending on how much information the baseline measurement carries about the measurement at the end of the trial.

Vaak zijn voor onderzoek in de gezondheidszorg een beperkt aantal afdelingen, ziekenhuizen, of instellingen (algemeen gezegd: clusters) beschikbaar. Analyse methoden die meer precisie hebben, zoals het corrigeren voor een voormeting, vereisen vaak minder clusters en zijn daarom belangrijk.

Onze vraagstelling is “hoeveel clusters kunnen uitgespaard worden, wanneer een voormeting als covariaat in de geplande statistische analyse wordt meegenomen?”
Hiertoe hebben we empirische schattingen nodig van de mate waarin de voor- en nameting samenhangen (correlaties) en een formule of computer programma, om uit deze correlaties de reductie in het aantal clusters te berekenen.

Om een indruk te krijgen van de grootte van zulke correlaties zullen we deze berekenen in de studie “Implementation and evaluation of an occupational therapy intervention at home for dementia patients and their primary caregivers” (projectnummer 17099.3005). Verder zullen we een formule of computer programma maken om uit de correlatie tussen de voor- en nameting, de reductie in het benodigde aantal clusters te berekenen. Uit deze twee gegevens, zullen we voor verschillende scenario’s vuistregels maken voor hoe groot de correlatie moet zijn om het benodigde aantal clusters substantieel te kunnen terugbrengen.

Naast de uitkomstmaten in de hoofdstudie zullen we een extra maat meten, de zogeheten Goal Attainment Scores (GAS). Deze maat en eigenlijk subjectieve patiënt-relateerde uitkomstmaten in het algemeen worden steeds belangrijker in klinisch onderzoek vanwege de trend naar patient empowerment. We zullen de GAS voor dementie patiënten in Nederland valideren. Tevens zullen we de correlaties tussen voor- en nametingen van GAS scores schatten, zodat we ook voor onderzoek waarbij de GAS de hoofduitkomst is, kunnen aangeven hoeveel clusters kunnen worden uitgespaard, als de GAS voormeting in de analyse wordt meegenomen.

Samenvatting van de aanvraag

*Background: The feasibility of cluster randomized trials is often hampered by the limited number of clusters available. More powerful methods of analysis, such as including the baseline outcome as a covariate in the analysis, may help to decrease the number of clusters needed to achieve a prespecified power level. However, a realistic impression of the possible gain depends on empirical estimates of correlation between the pre- and post-intervention outcome and a formula/computer program to calculate from this correlation, the reduction in number of clusters needed. *Objective / Research question: How much clusters can be saved by including the baseline outcome as a covariate in the planned analysis? *Research plan: For outcome measures data in the main study, we will estimate the correlations between pre- and post-intervention outcome. Also, we will derive a formula or computer program to calculate, from these correlations, the reduction in number of clusters needed. Given these, we can estimate realistic reductions in the number of clusters by including the baseline outcome as covariate. As special focus, we will measure an additional outcome measure alongside the main trial: goal attainment scores (GAS). The rationale for this is that subjective patient related outcomes become more important in future trials, due to the trend towards patient empowerment, also in trials in the elderly. Moreover, GAS scores are very close to individual treatment goals, which makes them outcome measures that are very efficient with regard to the numbers needed to include. Therefore, we expect that sample size calculations for future trials will more often be based on goal attainment scores. To take advantage of including baseline GAS outcome as covariate in GAS based sample size calculations, we will estimate alongside the main trial, the correlation between pre- and post-intervention GAS scores. As a by-product, GAS scores will be validated in dementia patients in the Netherlands. *Datasets: 1) Outcome data of the main study (dossier number: 80-82315-98-09010). 2) Additional dataset of goal attainment scores acquired alongside the main study. *Time schedule: The goal attainment scores will be additionally measured alongside the 24-month period of data-acquisition of the main study (research assistant & PhD-student, trained and coached by Graff). During the 6 months report period (months 31-36 in the main study), we will 1) derive a formula or program to calculate the reduction in number of clusters, 2) estimate the relevant correlations for outcome measures in the main study and for GAS (Teerenstra), and 3) validate the GAS score for the Netherland setting (PhD-student & Graff). Speciale aandacht geven we aan zogeheten Goal Attainment Scores (GAS). We zullen deze extra uitkomstmaat meten in de hoofdstudie om twee redenen. Ten eerste worden subjectieve patiënt-relateerde uitkomstmaten steeds belangrijker in klinisch onderzoek vanwege de trend naar patient empowerment, ook in onderzoek bij ouderen. Ten tweede liggen GAS scores dicht tegen individuele behandeldoelstellingen aan, zodat ze als uitkomstmaten efficient zijn i.e. kleinere steekproefgroottes vragen. We verwachten dus dat steekproefgrootte berekeningen in toekomstig onderzoek vaker op GAS scores gebaseerd zullen worden. Om in de steekproefberekening van zulk onderzoek ook profijt te kunnen trekken van een GAS voormeting, zullen de correlaties tussen voor- en nametingen van GAS scores worden geschat. Als bijprodukt zal de GAS ook gevalideerd worden voor dementie patiënten in Nederland. *databronnen: 1) de uitkomstmaten in de hoofdstudie (dossier nummer: 80-82315-98-09010). 2) de extra gemeten GAS scores in de hoofdstudie. *tijdplanning: meting van de GAS scores: 24 maanden.
