Corticosteroids for treatment of chronic subdural hematoma (ChERISH)

Projectomschrijving

Per jaar worden ongeveer 1800 patiënten in Nederland gediagnosticeerd met een chronisch subduraal hematoom (CSDH), een niet-acute bloeding in de hersenen. Dit aantal zal nog toenemen, onder andere door vergrijzing en meer gebruik van anti-stollingsmedicatie. Een CSDH kan worden behandeld met een operatie (boorgatdrainage) of medicijnen (corticosteroïden). Tot op heden is er geen wetenschappelijk bewijs over welke behandeling beter is. Als gevolg daarvan wordt in sommige ziekenhuizen voor een operatie gekozen en in andere voor medicijnen. In deze studie wordt onderzocht of corticosteroïden even effectief zijn als boorgatdrainage. Als dat zo is kan standaardbehandeling met corticosteroïden in plaats van boorgatdrainage een kostenbesparing opleveren van 6.5 miljoen euro per jaar. Ook wordt verwacht dat corticosteroïden minder complicaties geven dan boorgatdrainage. Hopelijk wordt er middels deze studie een nationale richtlijn te kunnen ontwikkelen voor een optimale behandeling van het CSDH.

Samenvatting van de aanvraag

­SAMENVATTING DOEL/VRAAGSTELLING Wat is de kosteneffectiviteit van behandeling met medicatie (Dexamethason) in vergelijking met chirurgische behandeling bij patiënten met een chronisch subduraal hematoom? ­HYPOTHESE Dexametason is net zo effectief als chirurgie, maar goedkoper en gaat gepaard met minder complicaties bij patienten met een chronisch subduraal hematoom STUDIE OPZET Een gerandomiseerd onderzoek met geblindeerde evaluatie van uitkomsten in meerdere centra. ­STUDIEPOPULATIE/DATABRONNEN Alle nieuw-gediagnostiseerde patiënten met een chronisch subduraal hematoom. Patiënten waarbij behandeling geïndiceerd is, maar waarvan onduidelijk is welke de voorkeur geniet, worden gerandomiseerd tussen behandeling met Dexamethason of chirurgie. ­INTERVENTIE Dexamethason gedurende 2 weken versus chirurgie (boorgaten) ­GEBRUIKELIJKE ZORG/VERGELIJKING Zowel behandeling met een 2-weken durend schema met Dexamethason als operatieve behandeling met boorgaten en drainage zijn gebruikelijk in Nederland. Er is nog geen behandeling die op basis van wetenschappelijk bewijs de voorkeur geniet. We vergelijken derhalve beide behandelopties. ­UITKOMSTMATEN Primair: Functionele uitkomst, bepaald met de "modified Rankin Scale" na 12 maanden. Secundair: Kwaliteit van leven gemeten met de EuroQoL-5D en Qolibri na 12 maanden, 30-dagen mortaliteit, complicaties, het falen van de therapie en de kosten na 12 maanden. ­SAMPLE SIZE BEREKENING/DATA-ANALYSE De primaire uitkomst zal geanalyseerd worden met ordinale regressie, met correctie voor covariaten. De verwachte sample size van 420 patiënten betekent dat we 90% power hebben om aan te tonen dat Dexemathson even effectief is als chirurgie. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE De kosteneffectiviteitsanalyse zal worden gedaan volgens de Nederlandse richtlijnen, met maatschappelijk perspectief. We zullen alle relevante kosten en effecten van de verschillende behandelopties over 12 maanden meenemen. De primaire uitkomst is kosten per punt daling op de modified Rankin Scale, de secundaire uitkomst is kosten per gewonnen ‘quality adjusted life year’. TIJDPAD De studie zal 4 jaar duren met een inclusieperiode van ruim 2 jaar SUMMARY RESEARCH QUESTION What is the cost-effectiveness of dexamethasone compared to surgery in the treatment of chronic subdural hematomas (CSDH)? HYPOTHESIS Dexamethasone is as effective as surgery at a lower cost and with less complications in patients with CSDH. STUDY DESIGN A multicenter randomized controlled trial (RCT) with blinded outcome assessment STUDY POPULATION All patients with newly diagnosed CSDH. INTERVENTION A 2 week course of oral dexamethasone versus surgery (burr holes) USUAL CARE/COMPARISON Currently, there is no agreement on the best treatment of CSDH. We will compare a 2-week course of oral dexamethasone with surgical treatment. OUTCOME MEASURES Primary outcome: Functional outcome, assessed with the"modified Rankin Scale" at 12 months. Secondary: Quality of life measured with the generic EuroQoL-5D and disease- specific, Qolibri, at 12 months, 30-day mortality, complications, failure of therapy, costs at 12 months. SAMPLE SIZE CALCULATION / DATA ANALYSIS The primary outcome will be analysed with multivariable ordinal logistic regression adjusted for potential confounders. The expected sample size of 420 patients yields a power of over 90% to show equivalence. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS The economic evaluation will be performed according the Dutch guidelines, using a social perspective. The time horizon will be from inclusion to 12 months follow-up, in order to take all relevant costs and effects regarding the surgery and dexamethasone into account. The primary outcome for the cost-effectiveness will be the costs for one point decrease on the modified Rankin Scale (Cost Effectiveness) and secondary outcome will consist of the costs per quality-adjusted-life-year (Cost Utility). TIME SCHEDULE The trial will run for a period of 4 years, with an inclusion period of 24 months.
