Technology Assessment of Next Generation Sequencing in Personalized Oncology (TANGO)

Er komen steeds meer veelbelovende, maar vaak ook dure kankermedicijnen op de markt. Veel behandelingen die nu worden gegeven werken voor slechts een deel van de patiënten. Therapie-op-maat kan uitkomst bieden door patiënten de voor hen meest optimale behandeling te geven en daarmee het medicijngebruik doelmatiger in te zetten.

Doel

De belangrijkste doelstelling van dit project is het bepalen van het optimale gebruik van de nieuwste DNA-analyse technieken op basis van Whole Genome Sequencing (WGS), ten behoeve van therapie-op-maat voor patiënten met vergevorderd niet-kleincellig longkanker. Onderdelen van het project zijn: het bepalen van de diagnostische waarde en de doelmatigheid van WGS versus huidige diagnostiek, het berekenen van uitkomsten van behandelbeslissingen op basis van WGS-diagnostiek, het onderzoeken wat de meest efficiënte vorm van nationale organisatie en implementatie van WGS is en wat de ethische, juridische en maatschappelijke implicaties zijn van het invoeren van WGS.

Eindverslag

De rationale achter het TANGO-project was dat er een grote variatie tussen ziekenhuizen bestaat in het type- en gebruik van moleculaire diagnostiek testen, waardoor diagnostiek resultaten voor patiënten niet altijd even volledig zijn en behandel mogelijkheden niet optimaal kunnen worden ingezet. Whole Genome Sequencing (WGS) is een test waarbij gelijktijdig alle relevante genetische veranderingen in het tumorweefsel van individuele patiënten kan worden geanalyseerd. Hierdoor heeft WGS het voordeel dat naast detectie van alle relevante bestaande genetische veranderingen en biomarkers (voorspellers van respons op behandeling) voor de directe patiënt, de data óók gebruikt kan worden voor de ontdekking van nieuwe biomarkers voor behandeling van toekomstige patiënten. De belangrijkste doelstelling van het TANGO-project was het bepalen van het optimale gebruik van WGS, het selecteren van therapie-op-maat, met focus op patiënten met vergevorderd melanoom of niet-kleincellig longkanker. Het project bestond uit 6 deelprojecten: 1) Diagnostische waarde van WGS versus huidige diagnostiek vergelijken, 2) Uitkomsten bepalen van behandelbeslissingen op basis van WGS, 3) Voorspelling van de lange termijn gezondheidswinst van het gebruik van WGS voor de Nederlands populatie, 4) Doelmatigheid inzetten van WGS in vergelijking met huidige diagnostiek, 5) Nationale organisatie en implementatie van WGS voor therapie selectie, en 6) Ethische, juridische en maatschappelijke implicaties van implementatie van WGS in Nederland. Het projectteam bestond uit alle relevante disciplines (pathologen, genetici, oncologen, ethici, zorgeconomen) vanuit expertise centra in heel Nederland en de klinische onderzoeksactiviteiten waren ingebed in het Center for Personalized Cancer Treatment (CPCT). Wat we hebben gevonden is dat WGS dezelfde targets kan vinden als standaard diagnostiek. De toegevoegde waarde van WGS op dit moment leidt tot extra actionable (studie/off-label) targets voor met name longkanker, en het kan potentieel bijdragen tot identificatie van een biomarker voor de selectie van longkanker en melanoom patiënten die géén baat hebben bij immunotherapie. Daarnaast is WGS potentieel kosteneffectief in vergelijking met standaard diagnostiek voor longkanker patiënten, als voldaan wordt aan 2 criteria; de kosten van WGS moeten lager zijn dan 2,000 euro, en WGS moet 2,7% extra targets vinden.

Samenvatting van de aanvraag

Personalized medicine driven treatments in major diseases like advanced melanoma and non-small cell lung cancer (NSCLC) offer important health benefits to genetic subgroups, but can be expensive and may induce severe side effects. Whole Genome Sequencing (WGS) simultaneously tests for all relevant genetic aberrations in tumor tissue from individual cancer patients thereby allowing immediate selection of optimal therapy. This approach is likely to improve patient survival, avoid adverse effects, and to assist in controlling health care costs by offering treatment to only those identified to benefit. Objectives of the current proposal are therefore: A) to expand molecular profiling of tumors to improve immune- and targeted treatment selection in patients with advanced melanoma or NSCLC, and B) to determine the cost-effectiveness and budget impact of WGS to facilitate responsible introduction. The project consists of 6 Work Packages: 1)Diagnostic value of WGS, 2)Treatment decisions based on WGS, 3)Prediction of long-term health benefits and harms by micro-simulation, 4)Tumor-overarching early cost-effectiveness modelling, 5)Nation-wide organization of WGS, 6)Ethical, Legal and Societal Implications (ELSI) of WGS. The project team consists of a large nation-wide consortium embedded in the Center for Personalized Cancer Treatment (CPCT)/Hartwig Medical Foundation (HMF), covering all relevant disciplines (geneticists, pathologists, bio-infomatics, cancer biologists, oncologists, health economists and ethical/legal experts). This approach will provide evidence for decision making on genetic testing for optimizing health and economic outcomes for current and future personalized cancer care.
