Het DEPAR project is in 2013 geïnitieerd door de reumatologen in Zuidwest Nederland. Doel van het project was het opzetten van een volautomatisch dataverzamelingssysteem over (de behandeling van) Artritis Psoriatica (APs).
APs is een vaak voorkomende reumatologische aandoening. Naast het huidprobleem (psoriasis) is bij APs sprake van chronische invaliderende gewrichtsontstekingen. Alhoewel er veel bekend is over de verschillende vormen van deze APs, de toegepaste behandelingen en de effecten ervan, is de kennis niet afdoende. Met name de patiëntgerapporteerde uitkomsten en de socio-economische gevolgen van de aandoening zijn onderbelicht.

Resultaten

Het systeem heeft ervoor gezorgd dat een automatische gegevensverzameling van ruim 500 items per meetmoment voor meer dan 500 patiënten mogelijk is tegen lage kosten. De verzamelde gegevens dienen als basis voor analyse naar de meest (kosten)effectieve behandelingen en zijn voldoende gedetailleerd om een Decision Support systeem te bouwen voor een behandeladvies in de dagelijkse praktijk.

Samenvatting bij start

Artritis psoriatica is een chronische ontstekingsziekte gekarakteriseerd door gewrichtsontstekingen en rode, schilferende huidafwijkingen. De ziekte heeft veel impact op het dagelijks leven van de patiënt. Het is de 2e meest voorkomende chronische artritis na reumatoïde artritis. Er is echter nog onvoldoende bekend over het voorkomen van verschillende subtypes en optimale behandelstrategieën. Om meer inzicht te krijgen in het beloop van artritis psoriatica en de behandeling van de ziekte te verbeteren, is het Artritis Psoriatica Register (DEPAR-R) opgezet. Patiënten uit ziekenhuizen in Zuidwest Nederland worden 3 jaar gevolgd. Door middel van vragenlijsten, lichamelijk-, bloed- en röntgenonderzoek worden gegevens verzameld over onder andere de ziekteactiviteit, medicatiegebruik, kwaliteit van leven en werk. Uniek in dit onderzoek is het beveiligde digitale netwerk dat is ontwikkeld voor DEPAR-R. Het geautomatiseerde systeem zal het volgen van grote patiënten aantallen mogelijk maken.

DEPAR project is geïnitieerd in 2013 door de reumatologen in Zuidwest Nederland. Doel: het opzetten van een volautomatisch data verzameling systeem over (de behandeling van) Artritis Psoriatica (APs). APs is een vaak voorkomend reumatologische aandoening. Naast het huid probleem (psoriasis) is bij APs sprake is van chronische invaliderende gewrichtsontstekingen. Alhoewel er veel bekend is over de verschillende vormen van deze APs, de toegepaste behandelingen en de effecten ervan, is de kennis niet afdoende. Met name de patiënt gerapporteerde uitkomsten en de socio-economische gevolgen van de aandoening zijn onderbelicht. Het opgezette systeem heeft ervoor gezorgd dat automatische gegevens verzameling van meer dan 500 items per meetmoment voor 500+ patiënten (alleen in het eerste studiejaar zijn er 5 meetmomenten) mogelijk werd tegen lage kosten.
De verzamelde gegevens dienen als basis voor analyse naar het meest (kosten)effectieve behandelingen en zijn voldoende gedetailleerd om een Decision Support systeem te bouwen voor een behandeladvies in de dagelijkse praktijk.

Samenvatting van de aanvraag

Psoriasis is an immune-mediated skin disease affecting 1.5% to 3% of the population in North America and Europe. Psoriatic arthritis (PsA) is a chronic, disabling inflammatory disease, associated with psoriasis, one the most common rheumatologic disease. Approximately 30% of the patient suffering from psoriasis will develop PsA on average 7 years after the onset of the skin disease. As a chronic disease with estimated lifetime risk of 0.5% for women and 0.6% for men, the disease is responsible for a fair share of the work load of rheumatologists and it has a significant financial burden on the health care system. The presented proposal describes the development and implementation of the infrastructure needed for data collection on psoriatic arthritis patients in order to describe the incidence en prevalence numbers of the disease in secondary care in South-West of the Netherlands (representing an area of 3.000.000 inhabitants). As such, the databases will provide the exact numbers for the region as well as the detailed information on the current treatment strategies. The results will include the main outcome disease activity, and the secondary outcomes such as the cost effectiveness of the applied treatment and drug survival. Furthermore, work participation and general well being of the patients will be the subject of investigation. An extensive data set will be generated by combining the existing hospital information management systems (up to date demographics, laboratory data and radiological investigations as well as drug records) with additional information from questionnaires (electronically filled in by the patients and stored in the database). Also the information gathered by Research Nurses during regular scheduled patients’ visits at the outpatients clinics for the data which is not readily available from the patients records will be included. The emphasis will be put on generation of algorithms to provide the physicians with (ideally real-time) feedback for an individual patient based on the treatment outcomes of the whole group. In particular, the choice of the medical therapy used in the daily practice will be put to scrutiny with regard to safety, efficacy and cost-effectiveness. The information extracted from the database will be made available (sufficiently protected to assure the privacy of the involved parties) to policy makers, pharmaceutical companies and patient organization. In addition, given the representative combination of urban and rural areas in the region, the acquired local data will most likely be generally applicable the entire country. Last but not least, the data mining on the genetic determinants of the disease may be used for data comparison/results verification with the existing databases worldwide.
