Eerder onderzoek heeft uitgewezen dat het uitvoeren van echo-onderzoek bij baby’s op het consultatiebureau naar aangeboren heupafwijkingen (heupdysplasie) leidt tot een sterke afname van het aantal onterechte doorverwijzingen naar het ziekenhuis. In Utrecht is aansluitend de kosteneffectiviteit van de echometing op het consultatiebureau onderzocht. Dit met het oog op landelijke invoering van deze meting. De studie wijst uit dat het invoeren van de echometing kosteneffectief is mits het personeel dat de echometing uitvoert goed getraind is. Hierbij kan gekozen worden voor training van het personeel van het consultatiebureau of het inhuren van reeds getrainde echogebruikers, zoals radiolaboranten of verloskundigen. Opzetten van een landelijk systeem voor kwaliteitscontrole is daarom wenselijk. De kostprijs van de echoapparaten kan de kosteneffectiviteit beïnvloeden.

. objective(s) / research question(s): Implementation requires assessment of numerous potential scenarios. Frequently, local interest and constraints shape the final scenario, which may as a result lack external validity. Through a simulation study using data recorded in a multi center implementation study already representing various scenarios, potentially viable scenarios as a function of specifications of local settings will be identified. In addition, costs and cost-drivers relevant to local decision makers will be identified. The research question reads: 'which parameters of organisation drive local possibilities for implementation and associated costs?' . study design: Discrete Event Simulation study (DES). . study population(s)/ datasets: Data observed in 'Leidse Rijn' child health centres and two or more child health centres affiliated with Twente University and CHC Oost . intervention: Implementation of Ultrasound screening for developmental dysplasia of the hip (DDH) under variable constraints, demands and possibilities for professional organisations . outcome measures: Incremental costs per screening examination performed and incremental costs per child diagnosed correctly. . power/data analysis: Not applicable. . economic evaluation: Costs will be based in the estimates obtained in the principal study. The DES model also enables calculating the relevant iCERs as all outcomes of interest, i.e., costs, number of children screened and number of children with accurate diagnosis, are part of the output. . time schedule: In parallel with the implementation study a DES model will be developed reflecting all relevant parameters and outcomes. This will take 0.5 FTE junior researcher and 0.2 FTE senior for a period of 2 year. In the final year the model will be extensively validated and several viable scenarios will be identified through simulation. The last 6 months will be used for final analyses and reporting. In the last year 1.0 junior and 0.3 senior will be required. . doel / vraagstelling: Implementatie is veelal gebaseerd op locale interessen en mogelijkheden, hetgeen ertoe kan leiden dat generaliseerbaarheid beperkt is. Het onderzoek richt zich op het identificeren van realistische scenarios binnen de mogelijkheden en beperkingen van de lokale professionele organisaties. De onderzoeksvraag luidt: 'welke organisatieparameters bepalen de lokale mogelijkheden voor implementatie en ermee samenhangende kosten?'. . studie-opzet: DES . studiepopulatie / databronnen: Hoofdstudie gericht op implementatie van screening naar DDH met behulp van ultrageluidsonderzoek in de jeugdgezondheidszorg. . interventie: Echografische screening naar DDH . uitkomstmaten: kosten per gescreend kind en kosten per kind met een correcte diagnose . power-/data-analyse: niet van toepassing . economische evaluatie: kosten op basis van de hoofdstudie. Het DES model wordt tevens gebruikt om alle relevante uitkomsten, waaronder kosten, aantal gescreende kinderen en correcte diagnoses onder diverse scenarios te voorspellen en met elkaar in verband te brengen. Aldus worden iCERs geraamd. . tijdplanning: 36 maanden. 24 maanden modelontwikkeling. 6 maanden validatie en simulatie. Definitieve analyse en rapportage eveneens 6 maanden.
