Longstanding active physical therapy versus usual care in patients with rheumatoid arthritis and severe functional disability

Reumatoïde artritis (RA) is een vorm van chronisch ontstekingsreuma, gekenmerkt door ontstekingen in meerdere gewrichten. Uit onderzoek blijkt dat met kortdurende oefentherapie (≤12 weken) de conditie, spierkracht en het dagelijks functioneren van mensen met RA verbeteren. Hierna kunnen de meeste mensen zelfstandig blijven oefenen, bewegen en/of sporten.

Een kleine groep mensen met RA ervaart zodanige beperkingen (dagelijkse activiteiten, werk, vrijetijdsbesteding) dat kortdurende oefentherapie onvoldoende is. Of langdurige oefentherapie in deze groep (kosten)effectief is, is echter nog onbekend. Daarom is een wetenschappelijk onderzoek opgezet, waarin bij mensen met RA en ernstige functionele beperkingen een langdurig, actief oefenprogramma, aangeboden door speciaal getrainde fysio/oefentherapeuten, wordt vergeleken met gebruikelijke zorg. Na een jaar worden dagelijks functioneren, kwaliteit van leven en kosten tussen de groepen vergeleken.

Samenvatting van de aanvraag

DOEL: Vergelijken van de effectiviteit van langdurige, actieve fysiotherapie versus usual care bij patienten met reumatoide artritis (RA) en ernstige functionele beperkingen. Ook zal onderzocht worden of langdurige, actieve fysiotherapie in vergelijking met usual care kosten-effectief is. HYPOTHESE: Langdurige, actieve fysiotherapie is, in vergelijking met usual care, effectief ten aanzien van dagelijkse activiteiten en leidt tot vermindering van het gebruik van tweede en derdelijnszorg. STUDIE DESIGN: Randomised, controlled trial. STUDIEPOPULATIE: RA en ernstige, functionele beperkingen; De beperkingen zijn gerelateerd aan de RA en eventueel aanwezige comorbiditeit; Geen fysiotherapie of multidisciplinaire revalidatie in de afgelopen 3 maanden. INTERVENTIE EN CONTROLEGROEP: Interventie: Langdurige fysiotherapie (tenminste 26 w) gericht op verminderen of stabiliseren van individuele functionele beperkingen. Hierbij wordt gebruik gemaakt van gestandaardiseerde, bewezen effectieve fysiotherapeutische behandelstrategieen voor complexe problematiek. De behandelend fysiotherapeuten dienen specifieke deskundigheid te hebben en ontvangen een daarnaast een aanvullende training. Na 26 weken wordt de interventie tot 52 weken voortgezet. Controle: Care as usual. Alle patienten in de controlegroep krijgen na 26 weken 52 weken langdurige fysiotherapie aangeboden. UITKOMSTMATEN: Primair: individueel niveau van functioneren (activiteiten en participatie) gemeten met de PSK (patiënt specifieke klachten); Secundair: fysiek functioneren gemeten met de 10-item Patient Reported Outcomes Measurement Information System (PROMIS) for Physical Function, de Health Assessment Questionnaire- Disability Index (HAQ), de 6 min wandeltest. Zorggebruik tweede en derde lijn, kwaliteit van leven (RAQOL, SF-36)en de EQ5D; ervaren verbetering, tevredenheid met behandeling. Follow-up: 12, 26, 54 en 104 weken. SAMPLE SIZE BEREKENING/DATA ANALYSE: Uitgaande van een power van 0.90, Alpha 0.05, een populatie effect size van 0,5 voor de primaire effectmaat PSK op 26 weken en 20% uitval zijn in totaal 215 patienten nodig. Primaire analyse vindt plaats volgens het intention-to-treat principe. KOSTENEFFECTIVITEITS ANALYSE De economische analyse zal vanuit een maatschappelijk perspectief worden uitgevoerd volgens daarvoor geldende richtlijnen. Het doel ervan is het bepalen en vergelijken van de totale kosten van de interventie en controlegroep en deze kosten te relateren aan de gezondheidseffecten. TIJDPAD: Mnd 0-6: Voorbereiding; Mnd 6-18: Inclusie; Mnd 9-36 Follow-up; Mnd 36-42: Analyse en verslag. SUMMARY AIM: To compare the effectiveness of longstanding, active physical therapy versus usual care in patients with rheumatoid arthritis (RA) and severe,functional disability. In addition, it will be determined whether longstanding active physical therapy is cost-effective. HYPOTHESIS: longstanding, active physical therapy is, in comparison with usual care, effective with respect to functional ability en leads to a reduction of secondary and tertiary care health care usage. STUDY DESIGN: Randomised, controlled trial. STUDy POPULATION: RA and severe, functional disability; The functional disability is related to the RA and possible comorbidity; No physical therapy or multidisciplinary rehabilitation in the past three months. INTERVENTION AND CONTROL: Intervention: Longstanding physical therapy (at least 26 weeks) aiming to decrease or stabilize individual functional disabity. For this purpose, proven effective standardized physical therapy strategies for complex problems will be used. The treating physical therapist have specific expertise and will take part in additional education. Control: Care as usual. All patients in the control group are offered longstanding physical therapy after 26 weeks. OUTCOME MEASURES: Primary: individual level of functioning (activities, participation) over time, as measured with the Patient-Specific Complaints instrument (PSC); Secundary: Secondary and tertiary health care usage, physical functioning (the 10-item Patient Reported Outcomes Measurement Information System (PROMIS), the Health Assessment Questionnaire- Disability Index (HAQ-DI)and the 6 minute walk test), quality of life (the RAQol and SF-36)and the EuroQol 6-D for health valuation. Follow-up: 12, 26, 52 en 104 weeks. SAMPLE SIZE CALCULATION/DATA ANALYSIS: Taking into account a power of 0.90, Alpha 0.05, a population effect size of 0,5 for the primary endpoint PCS at 26 weeks and 20% drop-out, in total 215 patients will be needed. The primary analysis takes place according to the intention-to-treat principle. COST-EFFECTIVENESS ANALYSIS The economic analysis will be performed from the societal perspective, according to relevant guidelines. The aim is to determine and compare the costs of the intervention and to relate these to the health effects. TIME SCHEDULE: Month 0-6: Preparation; Month 6-18: Inclusion; Month 9-36 follow-up; Month 36-42: Analysis, reports and publications.
