Evaluation of PET and Laparoscopy in STagIng advanced gastric Cancer: a multicenter prospective study (PLASTIC-study)

De PLASTIC-studie evalueerde de toegevoegde waarde van FDG-PET/CT en stadieringlaparoscopie (SL) bij stadiering van gevorderde maagkanker, welke volgens de huidige nationale richtlijn standaard wordt verricht. Inzet van deze diagnostische modaliteiten kan leiden tot detectie van niet-operabele ziekte (tumordoorgroei in omringende organen of uitzaaiingen) en daarmee niet-zinvolle maagoperaties voorkomen, evenals daarmee gerelateerde kosten. SL leidde tot een substantiële vermindering in het aantal niet-zinvolle maagoperaties en kan daarmee tot een significante jaarlijkse kostenbesparing, terwijl de toegevoegde waarde van FDG-PET/CT beperkt was en kosten verhoogt. Derhalve is het aanbevolen om niet routinematig FDG-PET/CT te verrichten, en wel standaard SL uit te voeren bij stadiering van gevorderde maagkanker. In de toekomst kan op nationaal niveau en vooral op patiëntniveau een verbeterde afweging worden gemaakt van de baten en kosten van FDG-PET/CT en SL voor patiënten met maagkanker.

NEDERLANDS - DOEL Het evalueren van de toepasbaarheid en kosteneffectiviteit van FDG-PET/CT (PET) en diagnostische laparoscopie (DLS) te evalueren, in aanvulling op endoscopie en CT, bij hoog-risicopatiënten. - HYPOTHESE De hypothese van deze studie is dat het verrichten van PET en DLS bij hoog-risicopatiënten leidt tot betere patientselectie en betere kosteneffectiviteit. Op basis van literatuurgegevens zou hierdoor bij 27% van de patiënten een zinloze maagoperatie voorkomen kunnen worden, en zou de jaarlijkse nationale kostenreductie ca. €916.438 zijn. - STUDIE OPZET De opzet van de studie is een prospectieve, observationele, cohort studie. - STUDIEPOPULATIE De studiepopulatie bestaat uit patiënten met een chirurgisch resectabel, hoog-risico (cT3-4), adenocarcinoom van de maag, die na initiële stadiering met endoscopie en CT in aanmerking komen voor een curatieve behandeling. - INTERVENTIE De interventies die in deze studie wordt onderzocht zijn de toevoeging van PET en DLS op initiële diagnostiek met endoscopie en CT bij hoog-risicopatiënten. - GEBRUIKELIJKE ZORG / VERGELIJKING Zowel PET als DLS zijn recent in de Nederlandse richtlijn “Maagcarcinoom” opgenomen en gelden dus als standaard zorg bij patiënten met hoog-risico tumoren. De kosten van de studiepopulatie zullen worden vergeleken met een dataset van patiënten die geen PET en DLS hebben ondergaan. - UITKOMSTMATEN De primaire uitkomstmaat van deze studie omvat het percentage patiënten waarbij PET en/of DLS leiden tot een wijziging in het behandelplan. De waarde van PET en DLS zal onafhankelijk geanalyseerd worden. Secundaire uitkomstmaten zijn de diagnostische waarden, morbiditeit, mortaliteit, kwaliteit van leven, kostenreductie en kosteneffectiviteit. - SAMPLE SIZE BEREKENING Gebaseerd op een schatting dat ten minste 22% van de patiënten een verandering in beleid krijgen, zijn er minstens 239 patiënten nodig voor deze studie om te bewijzen dat de modaliteiten break-even zijn. Gedurende 3 jaar komen circa 543 patiënten in aanmerking voor deze studie. - KOSTENEFFECTIVITEITSANALYSE Een state-of-the-art kosteneffectiviteitsanalyse en budget impact analyse zullen worden verricht op de toegevoegde waarde van de combinatie van PET en DLS via zowel prospectieve als retrospectieve data verzameling - TIJDPAD De studie duurt in totaal 36 maanden en eindigt op 1 augustus 2020. De eerste 30 maanden zullen bestaan uit inclusie en follow-up, de laatste 6 maanden uit follow-up en analyses. ENGLISH - OBJECTIVE(S) To evaluate the impact and cost-effectiveness of FDG-PET/CT (PET) and diagnostic laparoscopy (DLS) in addition to initial staging by CT and gastroscopy in patients with advanced gastric cancer. - HYPOTHESIS The study hypothesizes that performing DLS and PET for advanced gastric adenocarcinomas results in a reduction in the number of futile gastrectomies performed and a favorable cost-effectiveness. According to the literature, in 27% of patients a futile gastrectomy can be prevented, and the annual cost-reduction is an estimated €916.438. - STUDY DESIGN The study design is a prospective observational study. - STUDY POPULATION The study population consists of patients with a surgically resectable, advanced gastric adenocarcinoma (cT3-4a,N0-3,M0), that are scheduled for treatment with curative intent after initial staging with gastroscopy and CT. - INTERVENTION The intervention to be investigated in this study is the addition of both PET and DLS to the initial staging with gastroscopy and CT of patients with an advanced tumor (cT3-4) - USUAL CARE / COMPARISON Both PET and DLS were recently included as staging modalities in addition to initial staging in the new Dutch guidelines for the treatment of gastric cancer of advanced (T3-4) tumors. The costs of the study population will be compared to retrospective data of patients who underwent curative surgery (gastrectomy) after initial staging with CT alone. - OUTCOME MEASURES The primary outcome of this study will be the proportion of patients in whom the PET or DLS lead to a change in treatment strategy. The accuracy of each modality will be analyzed separately. Secondary outcomes parameters will be diagnostic performances, morbidity and mortality, quality of life, cost-reduction and cost-effectiveness. - SAMPLE SIZE CALCULATION Based on the expectation that at least 22% of patients will have a change in treatment strategy, at least 239 patients will be needed for this study to demonstrate that the diagnostic modalities in the new guideline are break-even. Approximately 543 patients will be eligible for the study in 36 months. - COST-EFFECTIVENESS ANALYSIS A state-of-the-art cost-effectiveness analysis and budget impact analysis will be performed on the additive value of PET and DLS by both prospective and retrospective data collection. - TIME SCHEDULE The study will take 36 months and will be finished by 01-08-2020. The first 30 months will consist of inclusion and follow-up, the last 6 months of follow-up and analyses.
