Vraagstuk

De pensioengerechtigde leeftijd is de afgelopen jaren gestegen. De vraag is nu: hoe kunnen meer mensen langer gezond aan de slag blijven? Om werknemers daarbij te ondersteunen, bieden werkgevers diverse activiteiten aan, zoals gezondheidsbevorderende programma’s. Wat is de bijdrage van deze programma’s aan arbeidsdeelname op oudere leeftijd? Kunnen bedrijven zelf de potentiële effecten berekenen?

Onderzoek

Na vooronderzoek naar de invloed van een slechte gezondheid en ongezond gedrag van werknemers, is onderzocht wat de positieve gevolgen zijn van leefstijlgerelateerde interventies op de arbeidsparticipatie.

Uitkomst

Er is een programma ontwikkeld waarmee bedrijven zelf kunnen rekenen. Via een spreadsheet krijgen zij inzicht in de winst in arbeidsjaren en financiële baten voor de bedrijfspopulatie, door het aanbieden van leefstijlinterventies. De grootste winst zit in stoppen met roken-interventies. Op nationaal niveau blijkt wel een grote kloof tussen theoretische en werkelijke winst, vooral door het beperkte bereik van interventies.

De pensioengerechtigde leeftijd stijgt de komende jaren. Om werknemers gezond aan het werk te houden, bieden werkgevers diverse activiteiten aan. De afgelopen decennia bieden steeds meer bedrijven gezondheidsbevorderende programma’s aan hun werknemers aan. Dit project brengt de bijdrage van dergelijke programma’s aan arbeidsdeelname in kaart. Er is een model gemaakt dat inzicht geeft in de zogenaamde working life expectancy: het aantal jaren dat iemand op een bepaalde leeftijd verwacht wordt nog te (kunnen) werken. Hieruit blijkt dat er grote verschillen bestaan naar opleidingsniveau en geslacht. Mannen met een laag opleidingsniveau hebben op 25 jarige leeftijd een ruim drie jaar kortere working life expectancy dan mannen met een hoog opleidingsniveau. Voor vrouwen is het verschil in arbeidsduurverwachting nog groter, namelijk 10 jaar.

Daarnaast is de invloed van een slechte gezondheid en ongezond gedrag op werkloos worden, eerder met pensioen gaan en arbeidsongeschiktheid onderzocht. Uit dit onderzoek blijkt dat een verminderde gezondheid, zwaar overgewicht en een ongezonde leefstijl risicofactoren zijn voor uitstroom uit werk, voornamelijk via arbeidsongeschiktheid. Vervolgens is onderzocht wat de positieve gevolgen zijn van leefstijlgerelateerde interventies op de arbeidsparticipatie. Dit geeft inzicht in het belang van gezondheidsbevorderende programma’s in bedrijven. Er blijkt een grote discrepantie te bestaan tussen de theoretische en werkelijke winst. Deze discrepantie is toe te wijzen aan het beperkte bereik van interventies, en de beperkte effectgrootte. De grootste winst is te behalen door effectieve interventies gericht op stoppen met roken.

Ten slotte is een applicatie ontwikkeld waarmee bedrijven zelf kunnen berekenen wat gezondheidsinterventies positief kunnen bijdragen aan arbeidsparticipatie. Het is mogelijk de spreadsheet te gebruiken om inzicht te krijgen in de winst in arbeidsjaren en de financiële baten voor de bedrijfspopulatie door het aanbieden van leefstijlinterventies. Het model stelt organisaties in staat de baten van primaire preventieve interventies kunnen worden geëvalueerd met behulp van de ontwikkelde spreadsheet.

Dit geeft inzicht in het belang van gezondheidsbevorderende programma’s in bedrijven. Daarna wordt een applicatie ontwikkeld waarmee bedrijven zelf kunnen berekenen wat gezondheidsinterventies positief kunnen bijdragen aan arbeidsparticipatie.

Samenvatting van de aanvraag

The overall aims of this project are threefold: (1) to estimate the direct and indirect effects of ill health and lifestyle factors on working life expectancy; (2) to evaluate the theoretical and actual gains in working life expectancy through primary preventive interventions on lifestyle factors in the workforce; (3) to develop and field test a spreadsheet application that allows the evaluation of the benefits of primary preventive interventions in particular occupational populations. Part 1: Effects of ill health and lifestyle factors on working life expectancy The following activities will be conducted: * an updated review with available studies published up to December 2010. * a formal meta-analysis to describe the influence of ill health on the transition probabilities of quitting the workforce due to unemployment, early retirement, and permanent disability, with specific attention for the influence of age, sex, and education. * a systematic review on the effect of ill health on re-entering paid employment after unemployment and long-term disability. * a subsequent meta-analysis to estimate the transition probabilities on re-entering paid employment after unemployment and long-term disability, whereby is will also be analysed whether stratification by age, sex, and education is needed. * secondary data-analysis in the Survey on Health and Ageing in Europe (SHARE study), the European Community Household Panel (ECHP), and the Longitudinal Study on Socio-Economic Health Differences (LS-SEHD) in the Netherlands. These analyses will estimate transition probabilities and adds to previous work (a) a detailed analysis of time lag effects, and (b) detailed analysis for effects of ill health and unhealthy behaviour (both direct and indirect through ill health) Part 2: Theoretical and actual gains in working life expectancy through primary preventive interventions on lifestyle factors and on ill health in the workforce A disease - labour force participation model will be constructed in a spreadsheet application that allows an impact assessment of primary preventive intervention on health and sustained labour force participation. The multitistate work status table consists of the proportion of labour force participation for each age year and the annual transition probabilities from paid employment to unemployment, disability, and (early) retirement and vice versa. Working life survival curves will be constructed to calculate the working life expectancy for each age year, stratified by sex and education. Based on the results from Part 1, the annual transition from and into paid employment will be decomposed into the proportions due to lifestyle and ill health and due to other factors. This decomposition is derived from Population Attributable Fractions, calculated for significant determinants of work force exit. Basically, the effect of ill health on transitions will be partly attributed to unhealthy behaviour, whereas there also may be a direct effect of unhealthy behaviour on transitions (as was observed for obsity in our previous analyses). The population impact fraction (PIF) is used to calculate the proportion of ill health in the workforce which can be eliminated by reducing the influence of ill health and unhealthy behaviour. The benefits of the primary preventive interventions will be limited to the indirect costs associated with loss of productivity (productivity loss at work, productivity loss due to sickness absence, and productivity loss due to leaving the workforce). In the societal perspective displacement from the workforce will only result in indirect costs during the friction period. The 'company' perspectives will address costs for the employer, based on current regulations with respect to payment of social beneifts (ww, wao). Part 3: Development of a spreadsheet application to assess the benefits of primary preventive interventions for health and sustained labour force participation Part 3 is dedicated to making a user-friendly application that can be used by occupational health professionals and human resource managers in companies. This user-friendly application will contain: - an input module: * the constitution of the occupational population of interest, based on available information on age, sex, and education. * a menu of possible primary preventive interventions * an estimate of the total uptake in the occupational population of interest - a calculation module: * transition probabilities in labour force participation that are constant across the total workforce (given age, sex, and education). * proportions of quitting or re-entering the labour force due to ill health and lifestyle factors * prevalence of lifestyle factors across the total workforce (given age, sex, and education) * population impact fractions - an output module; * working life expectancy, by sex and education * cost-benefits at societal and company perspective.
