Rol van psychische stoornissen bij alcoholmisbruik
De Nederlandse overheid wil het alcoholgebruik van de bevolking en de negatieve gevolgen ervan aanpakken. De gegevens voor een goede aanpak zijn echter beperkt. Er zijn nauwelijks gegevens over hoe valt te voorspellen wie wel en niet een traject naar alcoholmisbruik en alcoholafhankelijkheid doorloopt. Ook is er weinig bekend over de rol van psychische stoornissen, zoals ADHD, op het beloop van alcoholmisbruik.

Doel
Daarom wordt er in dit project onderzoek gedaan naar het ontstaan en het beloop van alcoholmisbruik en alcoholafhankelijkheid. Specifieke aandacht is er voor de rol die psychische stoornissen, zoals ADHD, daarin spelen. Daarnaast wordt er ook gekeken naar het zorggebruik van mensen die alcohol misbruiken. Mogelijk leidt dit project tot aanbevelingen voor een betere behandeling van combinaties van alcoholafhankelijkheid en psychische stoornissen.

Eindverslag

Het verminderen van riskant alcoholgebruik en de gevolgen daarvan in de algemene populatie is een prioriteit voor de Nederlandse overheid. Daarvoor is gedetailleerde informatie over (problematisch) alcoholgebruik nodig. Belangrijke vragen zijn: wie heeft een grotere kans op het ontwikkelen van (langdurige) alcoholproblemen? Wie komt in zorg voor deze problemen? Om hier inzicht in te krijgen werden gegevens gebruikt van de ‘Netherlands Mental Health Survey and Incidence Study’ (NEMESIS-2), met daarin 6.646 volwassenen (18-64 jaar) op de eerste meting en 5.303 volwassenen bij de vervolgmeting na drie jaar.

• Allereerst werd het ontstaan van alcoholgebruik en alcoholstoornis onderzocht, met bijzondere aandacht voor de rol van ADHD en gedragsstoornis.
• De overlap tussen excessief drinken en een alcoholstoornis werd bekeken. Vervolgens werden drie groepen (alléén excessief drinken, alléén alcoholstoornis, beide) vergeleken op demografie, mentale gezondheid, functioneren en zorggebruik.
• Het driejarig beloop van een alcoholstoornis werd onderzocht; zowel het voortduren als terugkerende problemen. Voorspellers daarvan werden geïdentificeerd met specifieke aandacht voor de mate van alcoholgebruik.
• Mensen met een alcoholstoornis die wel en geen zorg gebruikten werden vergeleken. Daarbij werd een uitsplitsing gemaakt in zorg specifiek voor alcoholproblemen en meer algemene zorg voor emotionele/drugs problemen.

Het effect van ADHD en gedragsstoornissen op het ontstaan en beloop van alcoholstoornissen in de algemene bevolking. Resultaten van de tweede Netherlands Mental health Survey and Incidence Study (NEMESIS-2)

De Nederlandse overheid is al enkele decennia bezig om riskant alcoholgebruik en de daarmee gepaard gaande problemen in de bevolking te verminderen. Om dit effectief te kunnen doen mist zij echter basale informatie over veranderingen in alcoholgebruik: welke groepen hebben een grotere kans om alcoholproblemen te ontwikkelen, en als mensen deze problemen hebben wat maakt dat de één op termijn wel en de ander niet zijn riskante drinkgedrag kan veranderen? Dergelijke vragen over het ontstaan en beloop van alcoholgebruik, alcoholmisbruik en alcoholafhankelijkheid kunnen alleen worden beantwoord met behulp van longitudinaal bevolkingsonderzoek. Dit type onderzoek is echter schaars, vooral omdat het met nogal wat kosten gepaard gaat. Dit betekent dat vooralsnog essentiële informatie niet voldoende voorhanden is om de behandeling en preventie van riskant alcoholgebruik en de daarmee gepaard gaande gevolgen effectief vorm te geven.
Deze studie beoogt een beter begrip te krijgen van de effecten van ADHD, gedragsstoornissen en andere psychische aandoeningen op het ontstaan en beloop van alcoholgebruik, alcoholmisbruik en alcoholafhankelijkheid, alsook sekseverschillen in ontstaan en beloop ervan. Daarvoor wordt gebruik gemaakt van gegevens van de tweede Netherlands Mental Health Survey and Incidence Study (NEMESIS-2), een representatief longitudinaal onderzoek onder 6646 volwassenen in de algemene bevolking.

Samenvatting van de aanvraag

Since the last decades, the Dutch government has tried to reduce risky alcohol use and its consequences in the general population. To achieve this goal, detailed information about key changes in alcohol use is needed, e.g. which groups are more likely to develop alcohol problems, and if people develop such problems what determines that some can change their drinking behaviour and others not? These questions about onset and course of alcohol use, alcohol abuse and alcohol dependence can only be addressed in a longitudinal population study. However, such studies are scarce mainly because of their high cost (1). This means that essential information is missing to effectively organise treatment and prevention programmes aimed to reduce the negative consequences of risky alcohol use. The findings of this proposal will have practical implications by answering questions, like: how can alcohol disorders be detected in an early stage, do addiction care centres have to start treating other co-occurring psychiatric disorders among alcoholics as well, how can people with alcohol problems be motivated to seek professional treatment, and are there gender differences we have to consider in the prevention and treatment of alcohol use problems? Better understanding of the effects of ADHD, conduct disorders and other psychiatric disorders as well as gender differences in the onset and course of alcohol use, alcohol abuse and alcohol dependence is the central focus of this proposal. If the proposed study could demonstrate a strong influence of e.g. ADHD or conduct disorders in the onset and course of alcohol disorders, more effective prevention and intervention strategies could be developed. That is because ADHD can easily be diagnosed and effective treatment options for ADHD are available. Besides, if we could demonstrate that adolescents with conduct disorders experiment with substances at an earlier age, but are less able to recognise the negative consequences of risky alcohol use, a timely intervention could prevent a lot of problems. However, there exists much controversy about the role of ADHD, conduct disorders and other psychiatric disorders in shaping the onset and course of alcohol disorders. Determinants of alcohol remission in the population have hardly been studied yet (1). The main reason is that psychiatric-epidemiologic studies with a substantial follow-up period are very scarce. The proposed study will focus more thoroughly on predictors of alcohol remission, and possible gender differences in the rates of alcohol remission. Data will be used from the second Netherlands Mental Health Survey and Incidence Study (NEMESIS-2), a large longitudinal general population study among adults. In the proposed study the following questions will be addressed, using retrospectively and prospectively gathered data from this survey: Onset 1. What is the influence of ADHD and conduct disorders on the age of first full standard drink? 2a. What is the influence of ADHD and conduct disorders on the onset of alcohol abuse, among those who have ever had at least one full standard drink? 2b. What is the influence of ADHD and conduct disorders on the age of onset of alcohol abuse, among those who have ever had at least one full standard drink? 3a. What is the influence of ADHD and conduct disorders on the onset of alcohol dependence, among those who have ever met criteria for alcohol abuse? 3b. What is the influence of ADHD and conduct disorders on the age of onset of alcohol dependence, among those who have ever met criteria for alcohol abuse? Course 4. What is the 3-year course of alcohol disorders in the general population, in terms of rates of remission of alcohol disorders, and also specified to the amount of drinking (abstinence, low-risk drinking, and high-risk drinking)? 5. What are determinants of the 3-year course (remission versus chronicity) of alcohol disorders? Here, the effects of a wide variety of determinants will be analysed, such as socio-demographic characteristics, vulnerability indicators, functional limitations, characteristics of the alcohol use, clinical characteristics of the alcohol disorder as well as the presence of other psychiatric disorders, including ADHD and conduct disorders. Service use 6. How often do adults with alcohol disorders make use of professional services, and what are determinants of treatment-seeking for such problems?
