Projectomschrijving
Kinderen met een laag gewicht bij de geboorte hebben een hoger risico op hart- en vaatziekten en diabetes type 2 op volwassen leeftijd. De hypothese voor dit project was dat ongunstige blootstellingen in de foetale periode en vroege kinderjaren leiden tot permanente aanpassingen van foetale cardiovasculaire en metabole orgaansystemen, die gunstig kunnen zijn voor de korte termijn, maar het individu vatbaar maken voor ziekten in het latere leven. De resultaten toonden aan dat verschillende maternale leefstijl- en voedingsgerelateerde factoren en foetale groeipatronen vanaf de vroege zwangerschap van invloed zijn op de cardiovasculaire en metabolische ontwikkeling. We identificeerden ook verschillende genetische en epigenetische markers met betrekking tot zowel groei in het vroege leven als cardio-metabole ziekten op volwassen leeftijd. De resultaten hebben geleid tot een belangrijke bijdrage aan inzichten rond het vroege programmeren van ziekten op latere leeftijd.

Children with a low weight at birth have higher risks of cardiovascular disease and type 2 diabetes in adulthood. These remarkable associations have consistenly been demonstrated in different populations and strongly suggest that cardiovascular diseases and type 2 diabetes have at least part of their origins in fetal life. Form both an etiological and preventive perspective, it is important to unravel the mechanisms underlying these associations. It is clear that low bith weight is not the causal factor per se, leading to cardiovascular disease and type 2 diabetes. The main hypothesis for this study is that adverse fetal and early childhood exposures induce permanent developmental anatomical and functional adaptations of fetal cardiovascular and meabolic organ systems, which may beneficial for short term survival but predispose the individual to cardiovascular and metabolic dysfunction in childhood and diseases in later life. To address this hypothesis, I will use an integrated epidemiological, molecular and clinical life course approach form early fetal life onwards. This study will specifically be focused on the role of critical exposure periods, longitudinal fetal and childhood growth patterns, childhood adiposity measures, early markers of cardio-metabolic dysfunction, and persistent DNA methylation and expression changes. The study will make use of successful ongoing prospective cohort studies with high quality research infrastructures in Rotterdam, the Netherlands, and Bristol, United Kingdom. This project notably extends my previous work on fetal and early childhood origins of adult diseases.
