Repair of monogenetic defects in adult human liver stem cells

Gedurende dit project hebben onderzoekers een nieuwe methode ontwikkelt om leverstamcellen functionele levercellen voort te laten brengen. Deze methode faciliteert de productie van een grote hoeveelheid ontwikkelde humane levercellen die gebruikt kunnen worden voor medicijn ontwikkeling, toxiciteit testen en uiteindelijk regeneratieve geneeskunde. Daarnaast ontwikkelde de onderzoekers een nieuwe kweekmethode waarin niet alleen stamcellen maar ook functionele cellen uit kunnen groeien tot mini-levers in een kweekschaal. De DNA-sequenties in de mini-levers kan specifiek worden aangepast om de genetische defecten te repareren in mini-levers van patiënten met een genetische leveraandoening. Tenslotte konden de onderzoekers verifiëren dat de lever organoïden de aangedane lever konden penetreren en integreren om de integriteit van het orgaan te herstellen. Om deze redenen heeft het project grote stappen gemaakt voor de applicatie van patiënt specifieke lever organoïden bij de behandeling van leveraandoeningen.

Samenvatting van de aanvraag

The liver, the main metabolic organ in our body, shows a remarkable ability to regenerate. However, this regenerative capacity can be depleted by chronic tissue damage. At that point, transplantation of healthy liver tissue is the only therapeutical long-term option. Unfortunately, only few patients will receive an organ, due to the significant shortage of donor livers. Many chronic liver diseases such as alpha-1-antitrypsin deficiency, Wilson’s disease or Tyronsinemia are caused by mutations in a single gene. Repair of these mutations in the patient’s own liver cells, would not only remove the source of damage and thus cure the patient, but would abolish the need for life-long immunosuppression after donor organ transplantation. My research group recently developed a culture system that allows isolation and expansion of human liver stem cells in vitro. This human liver organoid technology allows me to grow the equivalent cell mass of an adult human liver from a single stem cell within 3-4 months. These adult tissue stem cells display high genetic stability and can be differentiated in vitro to hepatocytes. Most importantly, my preliminary results show that these cells can engraft in damaged liver tissue and contribute to organ function. Here I propose to combine the genetic stability and high expandability of our in vitro culture system with the most recent advances in genome editing to repair and transplant patient-derived liver cells into chronically damaged livers. To this end I will establish the optimal conditions for differentiation and engraftment, set up a novel method to repair monogenetic defects in vitro using the CRISPR/Cas9 system and transplant repaired patient-derived cells into chronically damaged mouse livers. Successful completion of this project will not only lay the foundation for future clinical applications, but will also provide essential insights into mechanisms of liver stem cell maintenance and differentiation.
