Towards Biological Joint Reconstitution

Gewrichtskraakbeen geneest slecht na beschadiging. Dat komt door de slechte doorbloeding van het kraakbeen en doordat kraakbeencellen zich moeilijk in het weefsel kunnen bewegen. Daardoor kunnen al kleine defecten leiden tot artrose. Er zijn nog geen medicamenteuze of chirurgische interventies die de voortgang van artrose kunnen remmen. Regeneratieve geneeskunde en vooral therapie met kraakbeencellen zou een optie kunnen zijn en deze is onderzocht. Daarbij is in het bijzonder aandacht besteedt aan het toedienen van signaalfactoren (interleukine 1β) bij slijtage van de knie. Die zouden het herstel van het kraakbeen kunnen versnellen. Hoewel deze stoffen de biologische reactie in het beschadigde kraakbeen verbeteren en patiënten ook wat beter presteren op tests die het functioneren van de knie meten, kan de therapie niet worden aanbevolen. De van te voren bepaalde verbetering in deze studie niet werd gehaald.

Eindverslag

Gewrichtskraakbeen heeft een beperkte capaciteit om te genezen na trauma en zijn functie te behouden bij toenemende leeftijd. Dit is het gevolg van de a-vasculariteit van het weefsel en de beperkte capaciteit van chondrocyten om te migreren in het weefsel. Defecten vanaf 2 millimeter in diameter kunnen leiden tot de ontwikkeling van artrose OA. De komende decennia zal de incidentie van artrose maar ook die van gewrichts schade door trauma toenemen als gevolg van toegenomen activiteiten en de continuerende vergrijzing. De huidige therapieën combineren medicamenteuze en niet-medicamenteuze behandelingen die symptoomvermindering tot doel hebben. Er zijn echter nog geen medicamenteuze of chirurgische interventies voorhanden waarvan bewezen is dat deze de progressie van artrose kunnen moduleren. Momenteel wordt intensief onderzoek verricht naar methoden om de ziekteontwikkeling te modificeren. Regeneratieve geneeskunde en in het bijzonder kraakbeen cell therapie is een ,ethode om defeceten in het gewrciht te herstellen en heeft tot doel het voorkomen van OA.Wij hebben de hypothese getoetst dat kraakbeen cel therapie een beter resultaat geeft dan traditionele chirurgische technieken. Verder hebben wij onderzocht welke factoren een rol spelen in het concept van gewrichts homeostase.

Samenvatting van de aanvraag

The success of biological joint reconstitution can be improved, by obtaining proof for the potential advantages and limitations of current cartilage tissue engineering strategies, and by defining and improving the parameters that limit the success of cartilage repair. Current results steer us towards developing methods that counteract the negative effects of catabolic cytokines and degeneration products released into the joint after trauma. Autologous chondrocyte transplantation (ACT) for repair of cartilage defects can restore articular cartilage with tissue that has hyaline cartilage characteristics. The results are not that good in all patients. Since articular defects do not heal cartilage degeneration is initiated. These conditions create a catabolic environment in the joint with a cascade of cytokines and inflammatory mediators. This "catabolic joint" constitutes an environment very different from the optimal laboratory conditions or healthy animal joints that demonstrated the feasibility of cartilage repair by tissue engineering. Further examination of this discrepancy between science and clinic is required if we want to improve biological joint reconstitution. The above clearly demonstrates an opportunity for research to improve clinical practice with direct and considerable impact on patient care.
