Immunohistochemische markers voor CD44s, p27-kip1 en Ki-67 (MIB-1) kunnen effectief worden bepaald binnen een radicale prostatectomie preparaat in patienten met prostaatkanker. Dit is zowel mogelijk in opgeslagen weefsel van radicale prostatectomie preparaten alsook in vers verkregen materiaal. De bovenbeschreven tumormarkers geven een aanvullende prognostische waarde naast PSA, graad van de ziekte (Gleason score), pathologisch tumorstadium en tumorvolume

Immunohistochemische markers kunnen worden bepaald in vers verkregen prostaatnaaldbiopten, zeggen iets over de expressie hiervan in de prostaat, en dienen hiertoe als een surrogaat voor de expressie in de prostaat zelf. In beperkte mate kan de expressie van CD44s, Ki-67 (MIB-1) en p27-kip1 iets zeggen over de uiteindelijke prognose van een patient die een radicale prostatectomie ondergaat (t.w. PSA terugkeer na radicale prostatectomie).

Het onderzoek heeft behalve de beoogde doelstellingen ook een andere wending gekregen. Zo werd er onderzoek gedaan naar de prognostische waarde van het rectaal onderzoek voor de opsporing van prostaatcarcinoom, de betrouwbaarheid van prostaatnaaldbiopsien via de transrectale en de perineale route, de prognostische waarde van voorloper lesies van prostaatcarcinoom in het biopt alsook in het radicale prostatectomie preparaat, en de de rol en de expressie van CD44s op circulerende tumorcellen in het bloed.

Later onderzoek in de eindfase van de opleiding tot uroloog spitste zich toe op de prognostische waarde van het positief chirurgisch snijvlak m.b.t. PSA recidief, locaal recidief, en de uiteindelijke overleving van patient, in een hersenspinsel omtrent het afschaffen dan wel blijven erkennen van de huidige graderingsmethose voor prostaatkanker (d.i. de Gleason score).

Aanbevelingen
Verder onderzoek naar de waarde van de immunohistochemische expressie van deze belangrijke prognostische factoren is aan te bevelen, zowel in prostaatnaaldbiopten alsook in de prostaat zelf. Wellicht zou de expressie in gemetastaseerde ziekte kunnen worden gemeten. Methoden om de betrouwbaarheid van de expressie in het prostaatnaaldbiopt als maat voor de expressie van tumormarkers in de prostaat zelf kunnen worden onderzocht en de criteria verscherpt. In deze tijd van een verhoogde detectie van prostaatcarcinoom t.g.v. screening en een mogelijke overdiagnsoe en overbehandeling van prostaatkanker noopt naar een meer betrouwbare voorspelling van het uiteindelijk gedrag van prostaatkanker. De expressie van bepaalde eiwitten (tumormarkers) in de prostaatkanker en in prostaatnaaldbiopten kan wellicht iertoe bijdragen, naast die van de reeds bekende prognostische factoren (graad, tumorstadium, tumor volume).

Samenvatting van de aanvraag

Early detection of prostate cancer by the use of the tumour marker prostate specific antigen (PSA) has led to a considerable increase of the incidence of prostate cancer. This rise in incidence of prostate cancer is not paralleled by increased mortality, indicating that a large proportion of men with prostate cancer might undergo overtreatment. A major problem is the lack of markers that can distinguish aggressive from more indolent prostate cancers. Currently, a few promising progression markers for prostate cancer have been reported, i.e. loss of the adhesion molecule CD44, the cell cycle markers MIB-1, p34cdc2 and p27, and genetic alterations in the recently isolated tumour suppressor gene PTEN. The purpose of the project is to examine whether any of these markers can be employed to distinguish on prostatic needle biopsies, i.e. prior to therapy, aggressive from more indolent cancers. The availability of a large biorepository of prostatic needle biopsies of participants to the European Randomised Screening program of Prostate Cancer (ERSPC) and corresponding radical prostatectomy specimens will allow us to perform this study. Initially, a series of about 70 needle biopsies will be assayed for expression of CD44, MIB-1, p34 cdc2 and p27, in addition to conventional parameters as Gleason score and proportion of needle biopsies involved by tumour. On the basis of prostate maps of radical prostatectomy specimens the tumour focus, represented in the corresponding needle biopsy, will be identified, and the same set of markers will be tested on these tumours. Pathological stage of the tumours will be determined as intermediate endpoint and biochemical (i.e. rise of PSA) and clinical progression as end-point after an average follow-up of 6 years. In a multivariant analysis the prognostic significance of each of the parameters will be determined. In a consecutive series of needle biopsies the most promising marker(s), selected on the basis of the foregoing study will be tested for prognostic significance. Since currently no well characterised antibodies against PTEN are available that can be applied on archival formalin fixed, paraffin embedded tissues we will attempt to generate hybridomas producing monoclonal antibodies against PTEN. Subsequently, the immuno-reactivity of these antibodies will be assayed on a set of prostatic tissues either with intact or with mutant PTEN. Dependent on the outcome of these experiments, anti-PTEN antibodies will be used to examine the frequency of PTEN loss in early detected prostate cancers. Association with other prognostic parameter's and pathological stage might demonstrate its potential use as a prognostic marker in early detected prostate cancer.
