Hypogonadotropic hypogonadism in CHARGE syndrome: clinical characterisation and mouse studies in order to unravel the link with Kallmann syndrome

Er zijn diverse syndromen die worden veroorzaakt door genetische veranderingen in het CHD7 gen. Dat maakt een eiwit dat is betrokken bij de ruimtelijke organisatie van het erfelijk materiaal en de activatie van genen. De mutaties in CHD7 veroorzaken aangeboren afwijkingen aan ogen, oren, neus, keel en hart. Evenwichtsproblemen, doofheid en het ontbreken van reukzin en het niet tot stand komen van de puberteit, zijn vaak voorkomende verschijnselen. Er zijn patiënten opgespoord met mutaties in het CHD7 gen en in muizen is geprobeerd genetische veranderingen aan te brengen zodat ze als model kunnen dienen voor onderzoek van deze complexe syndromen bij mensen. De variatie aan symptomen die de muizen ontwikkelen blijkt groot, daardoor is het waarschijnlijk dat het effect van een CHD7 mutatie in mensen en muizen wordt beïnvloed door subtiele omgevingsvariaties tijdens de embryonale ontwikkeling.

Eindverslag

In ons project getiteld ‘Hypogonadotroop hypogonadisme in CHARGE syndroom: klinische karakterisering en muisstudies ter ontrafeling van de link met Kallmann syndroom’ hebben we de klinische overlap tussen CHARGE en Kallmann syndroom bestudeerd.
CHARGE syndroom wordt gekenmerkt door een wisselende combinatie van aangeboren afwijkingen, waaronder Colobomen van het oog, Hartafwijkingen, Atresie van de choanen (= blokkade van de neus-keel doorgang), Retardatie van groei/ontwikkeling, Genitale hypoplasie en Oorafwijkingen/doofheid. Evenwichtsproblemen en een gespleten lip/gehemelte kunnen ook voorkomen. CHARGE syndroom komt voor bij ongeveer 1/10.000 pasgeborenen en wordt veroorzaakt door veranderingen (mutaties) in het CHD7 gen. De functie van het CHD7 eiwit is niet goed bekend, maar men denkt dat het de expressie van andere genen tijdens de vroege embryonale ontwikkeling beïnvloedt. Sinds de ontdekking van CHD7 is gebleken dat sommige CHARGE patiënten slechts milde verschijnselen vertonen. Uit recente studies is gebleken dat hypogonadotroop hypogonadisme (HH, afwezige puberteit door Gonadotrofine Releasing Hormoon, GnRH, tekort) en anosmie (afwezige reukzin door afwijkende reukhersenen) vaak worden gezien bij CHARGE syndroom.
Kallmann syndroom is de combinatie van HH en anosmie. Daarnaast kunnen gespleten lip/gehemelte, nierafwijkingen en gehoorsverlies voorkomen. Deze kenmerken kunnen ook aanwezig zijn bij CHARGE syndroom. Bij slechts 30% van de Kallmann patiënten worden mutaties gevonden in de bekende Kallmann genen (KAL1, FGFR1, PROK2 en PROKR2). In een pilot studie hebben we bij enkele Kallmann patiënten CHD7 mutaties gevonden. Het zou kunnen dat de Kallmann eigenschappen in CHARGE syndroom het gevolg zijn van de invloed van CHD7 op de expressie van de Kallmann genen.
We hebben de overlap tussen CHARGE en Kallmann syndroom op 3 manieren onderzocht; (1) we hebben de reuk en puberteitsontwikkeling bestudeerd in CHARGE patiënten, (2) we hebben het CHD7 gen geanalyseerd bij Kallmann patiënten en (3) we hebben de reuk en voortplanting onderzocht bij muizen met een CHD7 mutatie.
(1) We hebben de reuk en puberteitsontwikkeling van 27 CHARGE patiënten in kaart gebracht. HH en anosmie waren aanwezig in de meerderheid. HH en anosmie bleken altijd samen voor te komen; 9 patiënten hadden anosmie en HH, terwijl bij 4 patiënten sprake was van een normale reukzin en normale puberteit. Een reuktest kan daarom voorspellen of een CHARGE patiënt HH zal krijgen en dus niet spontaan in de puberteit zal komen. Vroege diagnostiek van anosmie bij CHARGE patiënten met een reuktest draagt eraan bij dat zij op een normale leeftijd in de puberteit kunnen worden gebracht. Dit zal leiden tot minder sociaal-emotionele problemen en een verlaagd risico op osteoporose en hart- en vaatziekten.
(2) Momenteel zijn we bezig om Kallmann patiënten te screenen op CHD7 mutaties. Tot dusverre hebben we 8 Kallmann patiënten getest en hebben we 1 CHD7 mutatie gevonden. De CHD7 positieve patiënte bleek ook andere CHARGE kenmerken te hebben (gespleten lip/gehemelte, evenwichtsproblemen en gehoorsverlies). De patiënte is door ons gecounseld en weet dat zij 50% kans heeft om haar CHD7 mutatie door te geven aan haar nageslacht, waarbij een kind ook ernstiger aangedaan kan zijn (er is sprake van een grote variatie in ernst bij CHARGE syndroom, ook binnen families).
(3) We hebben de Kallmann kenmerken bestudeerd in een muismodel voor CHARGE syndroom om meer inzicht in de pathogenese te krijgen. Chd7 kwam tijdens de embryonale ontwikkeling tot expressie in de hersendelen betrokken bij reuk en voortplanting. De mutante muizen presteerden slechter op de reuktest en een deel had afwijkende reukhersenen. Enkele mannetjes hadden te kleine zaadballen en de vrouwtjes hadden allen afwijkende baarmoeders. Er bleken minder GnRH neuronen aanwezig te zijn in de hersenen van de vrouwtjes. Ook was het voortplantingsvermogen licht verminderd in de mutante muizen. Opvallend was dat de muizen m

Samenvatting van de aanvraag

CHARGE syndrome is a highly variable, multiple congenital malformation syndrome. The most frequent features are Coloboma, Heart defects, choanal Atresia, Retarded growth and development, Genital hypoplasia, Ear anomalies and deafness. But other congenital defects like semicircular canal hypoplasia, facial nerve palsy, cleft lip/palate, renal abnormalities and tracheo-esophageal fistula may also be present. CHARGE syndrome has a birth incidence of 1 in 8,000-10,000 and recently CHD7 on chromosome 8 has been discovered as the causative gene in this syndrome. Since this discovery it has become clear that the phenotypic spectrum of CHD7 mutations is very broad and that especially the mild end of this spectrum is not being fully recognised. The function of the CHD7 protein is not completely clear, but based on its functional domains it is likely that it has a role in chromatin modelling and thus might influence the accessibility of genes for transcription. It is hypothesised that CHD7 influences the expression of multiple early developmental genes, which could explain why so many different organs are affected in CHARGE syndrome. However, which target genes are involved and why the phenotype is so variable is not yet known. The mutations found in CHARGE syndrome are autosomal dominant loss-of-function mutations. Almost all patients are sporadic and due to de novo CHD7 mutations, while only a very few familial cases are known. This is surprising since at least 25% of the CHARGE syndrome patients have a normal cognitive development and should be able to rear children. The most likely explanation is that patients with CHARGE syndrome have a reduced fertility. Recent studies revealed that hypogonadotropic hypogonadism, anosmia and abnormal olfactory bulb development, the key symptoms in Kallmann syndrome, are common features of CHARGE syndrome. Kallmann syndrome is genetically very heterogeneous and several genes are known that together account for 30% of all cases. The two most important genes are KAL1, responsible for X-linked Kallmann syndrome and FGFR1 involved in autosomal dominant Kallmann syndrome. The knowledge that mutations in CHD7 can also occur in mildly affected CHARGE patients and the clinical overlap with Kallmann syndrome prompted us to sequence CHD7 in 40 patients diagnosed with Kallmann syndrome, but without mutations in KAL1 or FGFR1. We demonstrated a mutation in CHD7 in three of these patients of whom only one fulfilled the clinical criteria for CHARGE syndrome after careful re-evaluation. Thus it can be concluded that CHD7 mutations may be responsible for (atypical) Kallmann syndrome. On the other hand, one might speculate that the Kallmann features in CHARGE syndrome are due to the influence of CHD7 on the expression of KAL1 and/or FGFR1. In the proposed project we focus on the overlap of CHARGE syndrome with Kallmann syndrome by using both clinical and animal studies. The Kallmann features, abnormal puberty development and anosmia, will be studied in at least 25 Dutch children with CHARGE syndrome. This will enable us to formulate guidelines for surveillance and hormone replacement therapy. The other way round, the occurrence of undiagnosed CHARGE syndrome in children with (atypical) Kallmann syndrome will be investigated and subsequently criteria for CHD7 analysis in patients with Kallmann syndrome will be defined. In order to compare the embryological effects of Chd7 and Fgfr1 mutations on olfactory bulb development and outgrowth of GnRH neurons, two existing mouse models will be used: the Whirligig and Hush puppy mice. The Whirligig mouse has a mutation in Chd7 and shows many features in common with CHARGE syndrome. The Hush puppy mouse has a mutation in Fgfr1, a gene that plays a role in mouse olfactory bulb development. The effects of Chd7 and Fgfr1 mutations on the GnRH neurons and olfactory bulb development will be studied in both mutant mice at different stages of embryonic development. Besides, the influence of the Chd7 mutation on Fgfr1 and anosmin-1 like peptide (the KAL1 protein product) expression will be studied in the Whirligig mouse embryos. Smell will be tested in both mouse strains and in double heterozygote mice. The results of the mouse experiments will help us to understand the clinical overlap between both syndromes. Moreover, these results will be used to further explore CHD7 function in a C.elegans model within the group of dr. E. Nollen at our department. The proposed clinical and mice studies will help us to understand the embryologic link and thus clinical overlap between Kallmann syndrome and the highly variable CHARGE syndrome.
