Most, if not all, of the major neuropsychiatric disorders are cognitive-affective in nature. For example symptoms of aggressive and impulsive disorders may reflect aberrant or conversely deficient emotional control of learned behavioural responses. Cognitive-affective processes traditionally refer to covert processes that have to be inferred from behaviour. However, recent advances in functional neuroimaging (fMRI) and computational modeling have inspired biological psychiatrists worldwide to define neuropsychiatric abnormalities in terms of their cognitive-affective mechanisms. Insight into these mechanisms is increasingly recognized to be crucial for guiding diagnosis and optimizing treatment development. This is pertinent because there is currently no consensus regarding the origin or treatment of neuropsychiatric disorders. This lack of understanding likely reflects the heterogeneity of the disorders, the poor definition of their symptoms and underlying mechanisms. We aim to contribute to resolving these issues by adopting a mechanistic and neuroimaging approach focusing on well-defined affective deficits, their neural mechanism and their treatment across multiple aggressive and impulsive disorders. Specifically, we will investigate the neural and cognitive-affective mechanisms underlying aberrant versus deficient emotional control of behaviour in two forms of aggressive/impulsive disorder: proactive aggression, i.e. psychopathy, versus reactive impulsivity, i.e. borderline personality disorder. To this end, the use of a sophisticated cognitive-affective paradigm will be combined with fMRI and computational modelling. Critically, this approach will be applied in order to predict and quantify the effects of a successful and increasingly established psychotherapeutic treatment option for borderline personality disorder: dialectical behaviour therapy.
