Point of care diagnostic testing in patients with chest pain/dyspnoea to rule out serious acute cardiopulmonary conditions in out-of-hours primary care

Huisartsen vinden consulten bij patiënten met klachten van de borstkas, welke op een acute long- of hartaandoening kunnen duiden, uitdagend en staan positief tegenover aanvullende tests om ernstige acute aandoeningen uit te sluiten. Point of care testen zijn aanvullende diagnostische testen op hulpstoffen in het bloed die huisartsen kunnen ondersteunen om op dat moment de juiste beslissing te nemen of de patiënt acuut naar het ziekenhuis moet. Momenteel is er onvoldoende wetenschappelijk bewijs dat het inzetten van deze sneltesten uit een druppel bloed uit de vinger een effect heeft op uitkomsten die relevant zijn voor patiënten. Enkele sneltesten op hulpstoffen zijn veelbelovend, maar dan vooral wanneer de arts deze inzet in combinatie met een beslisregel waarin de huisarts de bevindingen uit het gesprek met de patiënt en het lichamelijk onderzoek kan samenvoegen. In dit onderzoek werd de Marburg Heart Score, een veelbelovende beslisregel om acute zuurstofschade van het hart (hartinfarct) uit te sluiten door middel van een innovatief flashmob onderzoek onderzocht. Tijdens dit onderzoek, de Huisarts Hart Week genaamd, konden alle huisartsen in Nederland patiënten met pijn op de borst registreren. De onderzochte klinische beslisregel bleek uiteindelijk onvoldoende in staat om een hartinfarct. Wij stellen dat toekomstig onderzoek naar het bepalen van de diagnose bij patiënten met acute hart- en longklachten zich moet richten op een strategie waarbij een klinische beslisregel wordt gecombineerd met een snelle test.

Samenvatting van de aanvraag

Chest pain and dyspnoea can be due to common causes like stable angina, gastro-intestinal disease, anaemia, anxiety, viral infections and musculoskeletal pain. However, they can also be symptoms of serious cardiovascular and pulmonary conditions including acute coronary syndrome (ACS), acute decompensated heart failure (HF), and thromboembolic events such as pulmonary embolism (PE) and related deep venous thrombosis. Symptoms and signs are often an incomplete guide to diagnosis for general practitioners (GPs). Three biomarkers – troponin, (NT-pro)BNP and D-dimer - were found to have a high sensitivity and negative predictive value for rapidly ruling out potential serious aetiologies. The development of mobile point of care tests (POCT) opens up opportunities for primary care because POCTs are non-invasive, easy-to-use, fast and can be used during home-visits as well. The aim of this research programme is to assess whether these biomarkers are valuable as part of the diagnostic work-up in cases with low or intermediate risk of serious events in primary care. We propose an individually randomised controlled clinical trial in out-of-hours general practice to evaluate clinical and cost-effectiveness of management of patients with chest pain/dyspnoea using these additional POCTs to rule out serious acute cardiopulmonary conditions. We will assess diagnostic accuracy of GP diagnosis at the recruitment consultation as well as the number of out-of-hours(ambulance) referrals and visits to secondary care, serious cardiopulmonary events during 3 month follow-up, patient quality of life and satisfaction with care, resource utilisation and costs. The trial will be preceded by a needs assessment among >1000 GPs with regard to POCT in daily practice and out-of-hours care and a series of preparatory substudies using mixed methods to detect POCT facilitators and barriers, to define and pilot clinical pathways and train GPs how to use POCT in their consultation, and check POCT devices for robustness. After the trial quantitative and qualitative methodologies will be used to thoroughly evaluate the interventions and subsequently achieve the best understanding of effect and to consider further steps for further improvement and implementation.
