Bestaand medicijn voorkomt SARS-CoV-2 infectie

Er is een zeer urgente noodzaak voor preventieve medicijnen tegen SARS-CoV-2 om de COVID-19 pandemie te stoppen. Wij hebben ontdekt dat de infectie van cellen met SARS-CoV-2 geblokkeerd wordt door het antistollingsmedicijn laag moleculaire heparine. Dit medicijn blokkeert de binding van het virus aan de cellen wat infectie sterk vermindert of zelfs voorkomt.

Inhalatie laag moleculair gewicht heparine

In dit project gaan wij onderzoeken of inhalatie van laag moleculair gewicht heparine als preventief medicijn gebruikt kan worden tegen SARS-CoV-2. Wij hebben hiervoor proefdiervrije innovaties ontwikkeld om de effectiviteit van de interventie te bepalen en om het werkingsmechanisme van dit medicijn te ontrafelen. Indien succesvol zal dit leiden tot een klinische studie met gezonde personen die een hoog risico lopen op SARS-CoV-2 met uiteindelijk doel de ontwikkeling van een inhalatie preventiemiddel om kwetsbare groepen te beschermen en de COVID-19 pandemie een halt toe te roepen.

Samenvatting van de aanvraag

We are currently in the COVID-19 pandemic and although new infections, ICU admissions and deaths are decreasing, there is an urgent need for quick and simple prophylactic interventions that prevent or limit SARS-CoV-2 infections. Our exciting preliminary data show that low molecular weight heparins (LMWH) are potent inhibitors of SARS-CoV-2 infections in vitro. Our data suggest that SARS-CoV-2 targets heparan sulfates to infect epithelial cells. However, we need to understand the mechanism and efficacy of LMWH to block SARS-CoV-2 in vivo. Animal models for COVID-19 are of limited use and therefore we have designed human volunteer studies and an innovative animal-free ex vivo human 3D respiratory flow cell model to address these questions. Healthy volunteers will be exposed to LMWH via nose drops or inhalation, and cells will be isolated via nasal brushes and bronchoalveolar lavages (BAL), respectively. We will measure SARS-CoV-2 binding to and infection of the isolated epithelial and immune cells ex vivo using innovative SARS-CoV-2 binding and infection assays. These studies will show whether in vivo LMWH-exposure protects against SARS-CoV-2 infection. We will use the ex vivo 3D respiratory flow model to identify the molecular mechanisms underlying LMWH inhibition of SARS-CoV-2 infection. As inhalation of LMWH is considered safe, we expect to rapidly translate our findings into a clinical trial for healthcare workers at risk with LMWH inhalation as a relatively cheap and safe prophylaxis against COVID-19. We will have several meetings with stakeholders to discuss our findings and develop the trial. Moreover, our study will also promote the use of our innovative animal-free models for COVID-19 research. Given that many respiratory viruses target heparan sulfate proteoglycans to attach to respiratory cells, this strategy may also be important in curtailing future respiratory (corona)virus outbreaks.
