De onderzoeker wil bewijzen dat subtiele veranderingen in dag-en-nachtritme leiden tot een verlaagde insulinegevoeligheid en een verandering in de glucoseregulatie. Eindverslag

De hypothalamus speelt een centrale rol in de regulatie van processen als eetgedrag, basaal energieverbruik en de glucose- en vetstofwisseling. De hypothalamus stuurt via de nucleus suprachiasmaticus ook het dag en nachtritme van vele biologische processen aan. Dit project had tot doelstelling nader inzicht te krijgen in de effecten van hypothalame ontregeling of beschadiging op glucoseregulatie met behulp van een aantal klinische en preklinische studies. In een diermodel werden de effecten van beschadiging van de nucleus suprachiasmaticus onderzocht. In patienten die een periode van chiasmacompressie door een hypofysetumor hadden doorgemaakt, werden aanwijzigingen gevonden voor een veranderde slaap en veranderd activiteitspatroon.
Een belangrijke bevinding is dat al na een enkele nacht met verkorte slaapduur, namelijk slaap tussen 1 en 5 uur snachts, effecten op insulinegevoeligheid worden gemeten, zowel in gezonde controlepersonen als in patienten met diabetes mellitus type I. Deze interventie representeert alledaagse verstoringen van slaap. Slaap en dag- en nacht ritme zijn belangrijke determinanten voor de insulinegevoeligheid.

Introductie: De hypothalamus speelt een centrale rol in de regulatie van complexe processen als eetgedrag, het basale energieverbruik en de glucose en vetstofwisseling. Bij preklinisch onderzoek is naar voren gekomen dat de hypothalamus erg belangrijk is voor de weefselspecifieke regulatie van de insuline gevoeligheid van de glucose stofwisseling, waarschijnlijk via veranderingen in de activiteit van het autonome zenuwstelsel. De hypothalamus stuurt via de nucleus suprachiasmaticus ook het dag-nacht ritme van vele biologische processen aan.
Wij hebben bij sommige patiënten, behandeld voor hypofyse tumoren, aanwijzingen gevonden voor veranderingen in deze biologische klokfunctie. Sommige grote hypofyse tumoren, mogelijk door druk op het chiasma opticum, leiden kennelijk tot hypothalame schade met een gestoord dag-nacht ritme, hetgeen tot nu toe niet onderkend is.
In een eerdere studie hebben we gevonden dat er bij patiënten behandeld voor grote hypofyse tumoren een veel hogere prevalentie is van metabole cardiovasculaire risicofactoren, als insuline resistentie en dyslipidemie in vergelijking met gezonde personen, ondanks een optimale hormonale substitutie van de hypofyse uitval.
Voor deze aanvraag richt ik mij daarom op de relatie tussen de regulatie van het dag-nacht ritme door de hypothalamus, en de regulatie van de insulinegevoeligheid van de glucose stofwisseling.
Mijn doelstelling is daarom om aan te tonen dat subtiele stoornissen in dag-nacht ritmes
in het algemeen leiden tot een verminderde insuline gevoeligheid en een gestoorde glucoregulatie.

In deze aanvraag willen we het effect van verstoring van dag-nacht ritmes door endogene oorzaken, door schade aan de hypothalamus, en door exogene oorzaken, nl door korte slaapdeprivatie, op de glucoregulatie bestuderen. Wij verwachten dat de effecten op het glucosemetabolisme zelfs al optreden na een nacht wat slecht slapen, zoals dat in het dagelijks leven ook wel eens voorkomt.

Mijn vraagstellingen zijn:
1.Wat is het effect van een gestoord dag-nacht ritme door hypothalame schade op insuline gevoeligheid en glucoregulatie?
2.Wat is het effect van verstoring van het dag-nacht ritme door subtiele slaapdeprivatie op insuline gevoeligheid en glucoregulatie? Het effect van een normale slaap op de glucoregulatie wordt vergeleken met het effect van een korte slaapdeprivatie (slaap toegestaan alleen tussen 01.00 en 05.00 u). Meetmethoden: In alle studies wordt de glucoregulatie steeds gemeten in de basale toestand en vervolgens gedurende een euglycemische hyperinsulinemische clamp studie met isotopen dilutie van 6,6-D2 glucose. Hierdoor kan zowel de hepatische als de perifere insuline gevoeligheid gemeten worden.

Relevantie: Deze studies zullen aantonen in welke mate stoornissen in dag-nacht ritmes leiden tot insuline resistentie. Dit is van belang voor het begrip van de onderliggende pathofysiologische concepten van insuline resistentie. Deze inzichten zijn niet alleen van belang voor patiënten met hypofysaire ziekten en type 1 diabetes mellitus maar ook van groot belang voor het begrip van de effecten van slaapduurverkorting, westerse leefstijl, en ploegendiensten op cardiovasculaire risicofactoren.

Samenvatting van de aanvraag

Introduction: The hypothalamus plays a central role in the complex regulation of metabolic processes such as eating behavior, basal energy expenditure, glucoregulation and lipid metabolism. From animal experiments we have learned that the hypothalamus is important for the tissue specific regulation of insulin sensitivity of the glucoregulation, likely via changes in the activity of the autonomic nerve system. In addition, the suprachiasmatic nucleus in the hypothalamus dictates the diurnal variations of many biologic processes. In some patients treated for large pituitary tumors, we observed disturbances in diurnal rhythms. We hypothesize that these large tumors with suprasellar extension and surgical and radiotherapeutic interventions close to the hypothalamus lead to hypothalamic damage, and consequently to disturbed diurnal rhythmicity. In another study we found a high prevalence of metabolic risk factors, such as dyslipidemia and insulin resistance, in patients treated for large pituitary tumors in comparison with the general population, despite optimal replacement therapy of hypopituitarism(1-4). Our hypothesis is that these alterations in diurnal regulation and the high prevalence of cardiovascular risk factors are related. In this proposal I want to evaluate the relation between disturbed diurnal regulation of the hypothalamus and the regulation of glucose metabolism, with a special focus on the effects on hepatic and peripheral insulin-sensitivity. Patients with large pituitary tumors with suprasellular extension are a unique model to investigate this potential role of the hypothalamus on glucoregulation in humans. The aim is to proof that subtle changes in the day-night rhythm in general lead to decreased insulin sensitivity and alter glucoregulation. We will study glucoregulation in patients and rats with an altered day-night rhythm due to an endogenous cause, i.e a damaged hypothalamus in humans and rats, and due to exogenous causes, i.e. short sleep deprivation. My objectives are to answer the following questions: 1: What is the effect of a disturbed day-night rhythm caused by hypothalamic damage on insulin sensitivity and glucose metabolism? 1a: in patients with large pituitary tumors, 1b: in rats with hypothalamic damage due to electric bilateral coagulation of the suprachiasmatic nucleus. 2: What is the effect of disturbance in day-night rhythm induced by subtle sleep deprivation on insulin sensitivity and glucose metabolism? The effects of normal sleep will be compared to those of sleep deprivation (sleep only between 1:00 and 5:00 hr)? 2a: in healthy controls 2b: in patients with type I diabetes mellitus who are treated with a continuous stable sc infusion pump, and cannot compensate for altered insulin sensitivity by increasing endogenous insulin production. Methods: In all studies we will measure the characteristics of glucose metabolism in the basal, postabsorptive state followed by a euglycemic, hyperinsulinemic clamp experiment with the use of isotope dilution of 6.6 2H2 glucose. Using this method both hepatic and peripheral insulin sensitivity can be measured. Relevance: These studies will proof the concept that day-night rhythm disturbances induce insulin resistance. This concept is important for the understanding of pathophysiology of insulin resistance. The findings are not only important for patients with pituitary disease and type 1 diabetes mellitus, but also for the understanding of shortening of sleep duration in general, western life style, and the effects of night-shifts on cardiovascular risk profile.
