COVID-19 and ischemic stroke - How to tame a dozing monster

Patiënten met een COVID-19-pneumonie kunnen, zeker als ze op de intensive care worden opgenomen, trombotische complicaties krijgen, die kunnen bijdragen aan een ernstiger beloop van de COVID-19-pneumonie.
Longembolie (stolsel in de long) is de meest voorkomende complicatie, maar ook herseninfarcten kunnen optreden. De hypothese in deze studie is dat deze beroertes kunnen worden veroorzaakt door een stollingsactivatie en vaatwandontsteking, of dat ze het resultaat zijn van een bloedstolsel dat vanuit de been- of bekkenvaten via een open verbinding tussen de rechter- en linkerkant van het hart in de hersenvaten terechtkomt.

Onderzoek en verwachte uitkomsten

In drie werkpakketten wordt kennis verkregen over:

  1. een nauwkeurige schatting van het vóórkomen van herseninfarcten bij COVID-19 IC-patiënten.
  2. de oorzaken en gevolgen van herseninfarcten aan de hand van beeldvormende onderzoeken en lab bepalingen.
  3. de optimale preventie van de herseninfarcten door het effect na te gaan van verschillende behandelingen.

Samenvatting van de aanvraag

COVID-19 pneumonia is complicated by a high risk of thrombotic complications, occurring with an incidence of 41% in intensive care unit (ICU) patients. These thrombotic complications are strongly associated with poor clinical outcome and death. Pulmonary embolism is the most common complication, but an emerging problem is the increase in number of reported ischemic strokes of currently unknown magnitude and impact. Our hypothesis is that these ischemic strokes are either caused by intensive coagulation-induced (local) thrombosis and intracranial vessel wall inflammation, and/or the result of a paradoxical embolism through a patent foramen ovale. In three work packages we will: 1) get an accurate estimate of the incidence of symptomatic ischemic stroke in all COVID-19 ICU patients (admitted to 20 Dutch hospitals) as well as the prevalence of silent brain lesions as detected by MRI in a random sample of COVID-19 ICU survivors (n=200); 2) unravel its causes and impact on the basis of imaging studies, biomaterial and measures of functional outcomes, anxiety and depression; and 3) establish the optimal prevention of this unwanted thrombotic complication in COVID-19 patients by comparing incidences of ischemic stroke under different thrombosis prophylaxis strategies. Our multidisciplinary team, with complementary key expertise in all relevant aspects of the field, who were the first to report on the very high risk of thrombotic COVID-19 complications, is very capable of rapidly addressing this urgent unresolved cerebrovascular problem of ischemic stroke in COVID-19.
