The discovery of lymphoid tissue inducer cells as key players in Crohn’s disease

De ziekte van Crohn is een chronische darmontsteking. Deze ziekte wordt veroorzaakt door een combinatie van genetische aanleg, de omgeving en een niet goed functionerend afweersysteem. Onlangs zijn nieuwe ce  en gevonden binnen het afweersysteem: innate lymphoide cellen. Tijdens de ontwikkeling van de mens maken deze cellen lymfknopen en na de geboorte beschermen zij de darm  tegen infecties. Het is mogelijk dat deze cellen een belangrijke rol spelen in de chronische darmontsteking bij mensen met Crohn. In dit project wordt de rol van innate lymphoide cellen bij de ziekte van Crohn onderzocht. Hiermee kunnen mogelijk nieuwe therapieën ontwikkeld worden en nieuwe testen om patiënten adequater te kunnen behandelen.

Eindverslag

Met behulp van dit AGIKO stipendium hebben wij dit project succesvol kunnen afronden en is arts-onderzoeker Charlotte Peters op 25 april 2014 gepromoveerd op het proefschrift genaamd: “Crohn's disease: mucosal immunology and immune modulating therapy”. Zoals beschreven in voorgaande samenvatting, was het doel van dit project om kenmerken van ILCs in de darm van Crohn patiënten te identificeren. Dit leidde tot interessante bevindingen, met name een nieuw soort ILC dat waarschijnlijk ontstekingen bevorderd en mogelijk het chronische karakter van de ziekte van Crohn in stand houdt. Dit hebben wij gepubliceerd in het tijdschrift Nature Immunology. Met deze kennis kunnen in de toekomst hopelijk nieuwe therapieën ontwikkeld worden. Nieuwe therapieën zijn nodig aangezien maar een deel van de patiënten goed reageert op de huidige behandelingen. Daarnaast is bij behandelend artsen niet goed bekend hoe de respons op therapie vroeg gemeten kan worden om op tijd een inefficiënte behandeling te staken of juist een succesvolle behandeling niet te vroeg te stoppen. Dit probleem doet zich ook voor in klinisch onderzoek, waar tot recent alleen subjectieve maten voorhanden waren om respons te meten, door middel van vragenlijsten. Daarvoor is in dit proefschrift een begin gemaakt in de ontdekking van zogenaamde biomarkers, die te meten zijn in het bloed en al na 1 week behandeling een onderscheid kunnen maken tussen goede of slechte respons op hierop. Deze biomarkers lijken vooralsnog niet geassocieerd met de ILC markers die tot nu toe bekend zijn, en veel van de geïdentificeerde genen zijn nog niet eerder beschreven in de ziekte van Crohn. Verder werd ook een duidelijk verschil gezien in eiwitexpressie tussen responders en non-responders al vroeg tijdens de behandeling van actieve ziekte.

Dit project heeft als doel, om nieuwe afweercellen te vinden bij de ziekte van Crohn. Hiermee komen er mogelijk nieuwe inzichten in de behandeling en monitoring van patiënten met Crohn. Deze cellen (lymphoid tissue inducer cellen: LTi cellen) zijn recent voor het eerst beschreven en blijken in de darm aanwezig te zijn, met name voor de geboorte. Na de geboorte, en met name bij chronische ontsteking van de darm (ziekte van Crohn), blijken niet alleen deze LTi cellen aanwezig te zijn, maar ook een niet eerder beschreven familielid van LTi cellen. LTi cellen behoren namelijk tot de familie van innate lymphoid cellen (ILCs), waarvan een IL17 en IL22 producerend subtype bekend waren. Echter, zoals bij de T helper cel familie, heeft ook deze familie naast de IL17 en IL22 producerende leden, een IFN gamma producent: ILC type 1. In de darm van patienten met Crohn blijkt een duidelijk overwicht van ILC type 1 te bestaan. Mogelijk veroorzaken, of onderhouden deze cellen de chronische darmontsteking. Dit nieuwe celtype produceert ontstekingseiwitten (IFN gamma) en blijkt ook in diermodellen darmontstekingen te veroorzaken. Wij beschreven dit nieuwe celtype in een recent geaccepteerd artikel in Nature Immunology, een hoogstaand wetenschappelijk tijdschrift. Deze cellen, en alles wat een interactie heeft, kunnen nieuwe inzichten geven in de behandeling, maar ook monitoring voor therapie respons. Op dit moment zijn analyses (RNA arrays en luminex) gaande, waarmee wij hopen de handtekening van succesvolle behandeling te kunnen onderscheiden van onvoldoende therapie respons. Ook op celniveau in het bloed zullen wij, zoals in de darm met ILCs, verdere typeringen uitvoeren. De huidige en aankomende inzichten zullen nieuwe mogelijkheden voortbrengen in de behandeling en monitoring van de ziekte van Crohn.

Samenvatting van de aanvraag

Inflammatory bowel disease (IBD) consists of Crohn’s disease (CD) and ulcerative colitis (UC). Patients suffer from chronic intestinal inflammation leading to bloody diarrhea, weight loss and fatigue. The major hurdle to unravel the immunopathology of these diseases has been the identification of the relevant inflammatory pathways. Genome wide association studies have identified genetic polymorphisms that are associated with CD or UC. Several independent studies have found a highly significant association between CD and the interleukin receptor IL23R gene on chromosome 1p31, which encodes a subunit of the receptor for the pro-inflammatory cytokine interleukin 23. IL23R is expressed on T helper 17 cells (Th17) which are thought to be important in IBD. Recently, it has been found that IL23R is also expressed on a novel innate lymphoid cell population that was discovered in our lab. These cells show features of lymphoid tissue inducer (LTi) cells and natural killer (NK) cells and are present in human lymph nodes, tonsils, Peyer’s patches in the ileum and the lamina propria of the gut. NK receptor positive (NKR+) LTi-like cells (also called NK22 cells) can be stimulated by IL23 to produce IL17 and IL22, cytokines known to be increased in the gut and blood of patients with CD. Since evidence in the mouse indicates that cells related to LTi are pathogenic in models for IL23-dependent colitis, we hypothesize that LTi and NKR+ LTi-like cells are involved in the pathophysiology of CD. Next to IL17 and IL22, high amounts of tumor necrosis factor (TNF) are produced by LTi cells. Anti-TNF therapy is effective in treatment of CD, but the working mechanism is not yet revealed. We hypothesize that the TNF targeted by anti-TNF therapy is produced predominantly by LTi cells in CD intestine. Therefore markers on the LTi cells in effectively treated CD patients may serve as biomarkers for accurate therapy response. Recently, we extensively characterized LTi cells in peripheral blood of healthy individuals. These peripheral blood LTi cells appear to reflect their intestinal state. Pathogenic LTi cells from the intestine might be present in CD peripheral blood and may differ from those in healthy individuals. Also markers in blood differently expressed in responders and non-responders to therapy can be used in clinical care, as early objective therapy response markers. To test these hypotheses we will compare the phenotype and functional characteristics of LTi cells in inflamed and non-inflamed intestine of patients with CD. We will also characterize LTi cells in fetal intestine as well as in healthy intestine to obtain insight into the frequencies and possible function of these cells in the gut before and after colonization with microbes. The characteristics of LTi cells in CD intestine will be compared to those from non-diseased intestine. With these experiments, the role of LTi cells in CD will be elucidated. To proof the concept of LTi cells being involved in the immunopathology of CD, LTi’s in different disease activity states within the same patient will be investigated. Biopsies and blood samples are obtained before and after therapy and the alterations that LTi cells undergo during treatment are investigated. Efficient therapy might induce different phenotypes, function or amount of LTi cells compared to ineffective therapy in non-responders. If this hypothesis is correct, patient’s peripheral LTi and LTi-like cells might serve as markers for disease activity and therapy response.
