In het kader van behandeling van experimentele colitis hebben wij onderzocht of breed- en smalspectrum antibiotica ontsteking in de diverse regionen van het colon van IL-10-/- muizen zou kunnen beïnvloeden. Ciprofloxacine en vancomycine-imipenem bleken specifiek de ontsteking te remmen in het caecum. Daarnaast bleken metronidazol en vancomycine-imipenem de ontsteking in het colon te reduceren. Concluderend blijkt dat in het kader van behandeling van experimentele colitis ciprofloxacine het meest effectief is in het caecum, metronidazol in het colon, en vancomycine-imipenem is effectief in beide regionen.
Hierna is onderzocht of Lactobacillus GG danwel Lactobacillus plantarum fulminante colitis in specifiek pathogeen-vrije transgene ratten kon behandelen en een recidief kon voorkomen na het stoppen van antibiotische behandeling. Lactobacillus GG kon colitis niet voorkomen in B. vulgatus-co-geassocieerde transgene ratten, en was evenmin in staat om fulminante colitis te behandelen. Lactobacillus GG kon echter een recidief colitis voorkomen in ratten behandeld met antibiotica, dit gold niet voor Lactobacillus plantarum.
Vervolgens was het doel te onderzoeken of orale prebiotica de luminale concentraties van probiotische bacteriën kunnen verhogen en colitis kunnen voorkomen. De prebiotische combinatie inuline/oligofructose voorkwam gedeeltelijk colitis in HLA-B27 transgene ratten. Dit correleerde met een veranderde darmflora, en tevens vonden wij een afname van intestinale pro-inflammatoire cytokinen en een toename van caecale TGF-β. Deze resultaten bevestigen de veelbelovende mogelijkheden van prebiotica als potentiële onderhoudstherapie bij patiënten met IBD.

Om de pathogenese van onstekignsreacties in experimentele colitis op te helderen toonden wij aan dat wanneer ongefractioneerde cellen van mesenteriale lymfeklieren uit HLA-B27 transgene ratten gestimuleerd worden met caecale bacteriële lysaten een grote toename van de pro/anti-inflammatoire cytokine ratio gezien wordt in vergelijking met non-transgene cellen. De IFN-g reacties verdwenen en de IL-12 productie minimaliseerde na depletie van T cellen, terwijl B cel depletie de IL-10 en TGF-β productie elimineerde. Deze resultaten suggereren dat colitis in HLA-B27 transgene ratten samenhangt met de activatie van IFN-γ producerende CD4+ T cellen die reageren op commensale bacteriën.
Daarnaast werd onderzocht of B. vulgatus colitis kan veroorzaken in monogeassocieerde HLA-B27 transgene rnu/rnu ratten en rnu/+ ratten die T cellen bevatten. In dit onderzoek hebben wij aangetoond dat rnu/+ ratten met normale T cel ontwikkeling, en ook rnu/rnu transgene ratten na transfer van CD4+ cellen van B. vulgatus rnu/+ transgene donor ratten, colitis ontwikkelden. Echter, rnu/rnu ratten zonder T cellen bleven ziekte-vrij. Samenvattend blijkt uit deze data dat T cellen vereist zijn en CD4+ T cellen voldoende voor het mediëren van colitis door de non-pathogene bacterie B. vulgatus in HLA-B27 transgene ratten.
Vervolgens werd een studie ontworpen om het relatieve aandeel van antigeen-presenterende cellen en T cellen in de ontwikkeling van colitis te bepalen. HLA-B27 transgene rnu/rnu ontvangers van non-transgene danwel transgene mesenteriale lymfeklier cellen inclusief T cellen ontwikkelden ernstige colitis vergeleken met rnu/rnu non-transgene ontvangers van non-transgene danwel transgene cellen. Concluderend induceren T cellen van non-transgene en transgene donoren colitis in rnu/rnu transgene maar niet in non-transgene ontvangers. Dit suggereert dat activatie van T cellen door antigeen-presenterende cellen die HLA-B27 tot expressie brengen cruciaal is in de pathogenese van colitis in dit model.
De intracellulaire mechanismen van ontregelde IL-12 reacties in de afwezigheid van IL-10 in IL-10-/- muizen zijn onbekend. Daarom hebben wij de intracellulaire signaaltransductie cascades voor NF-κB en STAT3 onderzocht in relatie tot LPS-geinduceerde IL-12 p40 gen expressie in den

Samenvatting van de aanvraag

Introduction: Crohn&8217;s disease and ulcerative colitis, collectively referred to as inflammatory bowel disease (IBD), are chronic relapsing inflammations of the intestinal tract. There are over 35,000 IBD-patients in the Netherlands. As a result of the relatively poor understanding of the pathogenesis of IBD, therapeutic approaches are symptomatic and do not change the natural course of disease. In a well defined animal model of IBD, interleukin-10 deficient (IL-10-/-) mice develop intestinal inflammation when susceptible hosts are colonized with specific pathogen-free (SPF) bacteria. However, germ-free (GF) conditions prevent the development of inflammation in this model, emphasizing the essential role of commensal intestinal organisms. Bacterial products bind to toll-like receptors (TLR), followed by the subsequent activation of several intracellular signalling cascades, including NF&954;B/I&954;B and the MAP kinases. In the absence of IL-10, these proinflammatory pathways lead to IL-12 driven TH1-mediated colitis, whereas IL-10 prevents inflammation in wild-type (WT) mice. The STAT/SOCS pathway is activated by IL-10 and potentially counteracts proinflammatory responses. However, it is unknown how IL-10 specifically inhibits proinflammatory responses in the presence of IL-10 in WT mice, or how the absence of IL-10 in IL-10-/- mice leads to dysregulated IL-12 production. In addition, the host genetic background of IL-10-/- mice determines the severity of disease, indicating the presence of modifier genes that interfere with these processes. Currently, the intracellular mechanism leading to the wide variety of disease phenotypes in IL-10-/- mice is poorly understood.Aim: The present application aims to 1) determine the specific target for IL-10 in the cell-signalling pathways NF&954;B/I&954;B and the MAP kinases, as well as the role of the STAT/SOCS pathway, which will explain the mechanism of dysregulated IL-12 production in IL-10-/- mice, 2) investigate which of the intracellular pathways described in aim 1 determines the differences in disease phenotype in IL-10-/- mice with various genetic backgrounds, and 3) to specify which genes on incriminated genetic loci on chromosome 1 and 3 correlate with dysregulated intracellular pathways and expression of disease as mentioned in aim one and two.Methods: Aim 1: We will compare the expression of the TLR isoforms most relevant to bacterial stimulation in bone marrow derived macrophages from IL-10-/- and WT mice. We will asses the activation and translocation of NF&954;B, a transcription factor which activation can lead to proinflammatory cytokine production. Chromatin immunoprecipitation will allow us to asses the binding of NF&954;B to the IL-12 promoter. We will determine the role of MAP kinases using specific neutralizing antibodies. The inhibitory pathway STAT/SOCS that is activated by IL-10 will be evaluated by using inhibitory RNA specific for STAT-3. We will add physiological concentrations of recombinant murine IL-10 to cultures of IL-10-/- cells and block endogenous IL-10 in WT cells with neutralizing anti-IL-10 antibody. Aim 2: We will study whether these pathways are different between IL-10-/- mice on permissive and resistant backgrounds, using the techniques described in aim one. Aim 3: We will continue our efforts to identify the modifier genes on chromosomes 1 and 3 that have been shown to influence the expression of colitis in various mouse models of intestinal inflammation. This will be established by determining differential expression of genes in susceptible and resistant mouse strains by microarray analysis.Rationale: The approaches described in this grant application will help to unravel the mechanisms of the protective effect of IL-10 in innate immune responses to bacterial stimuli. This will facilitate the search for therapeutic drugs that specifically target dysregulated disease-inducing pathways in IBD patients.
