Nutraceuticals as potential modulators of Mitochondrial Energy Metabolism: a Translational Study

Mitochondriële aandoeningen, stoornissen in de cellulaire energie productie, behoren tot de meest voorkomende stofwisselingsziekten. Het zijn zeer ernstige aandoeningen waarbij een gegeneraliseerd  tekort aan energie leidt tot het disfunctioneren van met name organen met een hoge energiebehoefte, zoals hersenen, spieren en het hart.  Hoewel er steeds meer bekend wordt over deze ziekten, is er op dit moment nog geen curatieve behandeling mogelijk. In dit project zal er gekeken worden naar het effect van bepaalde voedingsextracten (nutraceuticals) op specifieke karakteristieken van de celstofwisseling. Met dit onderzoek hopen wij de gevolgen van de verstoorde energiehuishouding te corrigeren en daarmee  symptomen te verlichten en progressie te voorkomen.

Mitochondriële ziekten zijn ernstige, nu nog onbehandelbare ziekten. Om het effect van toekomstige behandelingen aan te tonen, moeten gevoelige meetinstrumenten gebruikt worden, welke kleine verschil tussen de uitgangssituatie en de situatie na de behandeling kunnen detecteren. Het doel van dit project was om meetinstrumenten te selecteren voor kinderen met mitochondriële ziekten.
Binnen dit project hebben wij een aantal pilotstudies uitgevoerd met de instrumenten die wij uit de literatuur hebben geselecteerd. Wij selecteerden deze instrumenten op basis van de meest belangrijke klachten van patiënten.
Wij voerden zes pilotstudies uit. We bekeken de waarde van een ondersteunde 6 minuten fietstest, welke voor zwakke kinderen de 6 minuten looptest zou kunnen vervangen; we onderzochten of 3D accelerometrie in staat was om de fysieke activiteit van de kinderen in hun thuissituatie in kaart te brengen en we ontwikkelden een nieuw scoringssysteem om de ziekte-ernst van kinderen te bepalen. Daarnaast testten we nog drie biomarkers uit.
Met deze studies hebben we de eerste stappen gezet in het selecteren van uitkomstmaten voor kinderen met mitochondriële ziekten en kunnen we enkele aanbevelingen doen over welke test het meest veelbelovend zijn. Op dit moment worden de veelbelovende meetinstrumenten verder ontwikkeld.

Samenvatting van de aanvraag

--Background Mitochondrial disease belong to the most frequently encountered inborn errors of metabolism. Most affected patients with such disorders harbor a defect in the oxidative phosphorylation system (OXPHOS) of which the incidence is estimated to be 1:5000 live births. Disturbances in cellular energy production frequently present with a multi-system disorder affecting tissues and organs with a high energy demand like brain, heart and skeletal muscle. No cure is available for mitochondrial disease and managing of these patients is limited to preventing complications, minimizing disability and providing genetic counseling. In the past years we have i) described the clinical phenotypes of complex I disease, ii) developed a scoring system to determine the probability of existence of having a mitochondrial disease, iii) contributed to the development of a clinical scale to monitor progression and treatment of mitochondrial disease, iv) cloned most of the 38 nuclear-encoded structural complex I genes, v) described the first mutations in several of these genes, vi) described the first complex I assembly model, vii) characterized new complex I assembly genes, viii) described the first pathological mutations in these genes, ix) studied the cell biological consequences of complex I deficiency in detail, and x) developed cellular read-outs to study energy metabolism in living cells. In children, we completed a pilot-study on the influence of nutritional optimization on clinical outcome and OXPHOS function, which revealed improved clinical parameters like growth and motor development and an increase in ATP production capacity measured in muscle. Preliminary results of studies performed in collaboration with the Palmiter group (Washington University Seattle, USA) show that the symptoms observed in complex I deficient patients harboring mutations in one of the structural complex I proteins (the NDUFS4 subunit) are also present in the first complex I mouse model: the NDUFS4-KO mice. This project aims at studying the effect of nutraceuticals on specified mitochondrial parameters in an evidence-based manner. Nutraceuticals , a contraction of “nutrition” and “pharmaceutical”, is used for food components (such as vitamins, polyhenols, benzoquinones, etc) claimed to have a beneficial effect on health or medical conditions. Several of these nutraceuticals have shown to have a beneficial effect on mitochondrial parameters like reactive oxygen species production and complex I expression. Experiments will be performed in isolated and combined complex I deficient cell models, animals and patients, with the final goal to positively modulate mitochondrial metabolism. Selection of nutraceuticals will be based on their proposed cell biological effects including radical scavenging and controlling redox-balance.
