Hunting down the cause of cerebral infarcts

In het Vidi project 'Hunting down the cause of cerebral infarcts' zijn nieuwe methodes ontwikkeld om de dunne vaatwanden van de kleine slagaders binnen de schedel af te beelden. In de eerste jaren van het project zijn de basis methodes ontwikkeld waarmee de vaatwanden in een klein gebied zichtbaar gemaakt worden. In het vervolg van het project zijn methodes ontwikkeld voor het afbeelden van een veel groter gebied binnen de schedel. Er wordt bij deze methode ontwikkeling gebruik gemaakt van sterke en zeer sterke MRI scanners om voldoende details zichtbaar te maken. Omdat we nu voor het eerste met zoveel detail met MRI naar vaatwanden kijken is het belangrijk om te leren wat we zien. Dit laatste doen we door de afbeeldingen van de vaten met MRI na overlijden onder de microscoop te bekijken. Doordat we de methodes ook beschikbaar
hebben gemaakt voor MRI’s die gebruikt worden in de dagelijkse patiëntenzorg zijn er vele nieuwe toepassingen gekomen.

Achterhalen van oorzaken

Zo worden nu ook de dunne vaatwanden van aneurysmata afgebeeld en wordt er onderzocht of we hiermee kunnen voorspellen welke aneurysmata een hoog risico hebben om te barsten. Bijna dagelijks worden de ontwikkelde MRI scans nu ingezet om verschillende vragen bij patiënten te beantwoorden zoals het achterhalen van de oorzaak van een herseninfarct. Naast bredere
toepassing van de methodes in ons eigen ziekenhuis worden de methodes ook toegepast in andere ziekenhuizen in Nederland en daarbuiten. Binnen dit Vidi project hebben we ook onderzoek uitgevoerd in samenwerking met ziekenhuizen in Nederland, Azië (Hong Kong) en de VS. Binnen het project is een makkelijke methode ontwikkeld om afwijkingen te scoren zodat deze
methode makkelijker in de dagelijks praktijk gebruikt kan worden. Door de grote mate van details die zichtbaar zijn blijken de MRI methodes ook andere verrassende toepassingsgebieden te hebben zoals het afbeelden van hele kleine tumoren en zoals hierboven vermeld het afbeelden van de dunne wanden van aneurysmata.

Samenvatting van de aanvraag

Current stroke classification methods (using clinical symptoms, infarct location and stroke risk factors) often fail to determine the cause of cerebral infarcts in an individual patient. The detection of the exact cause of cerebral infarcts will make individualized stroke treatment possible. As a single patient example: if a presumed large-artery atherosclerotic stroke is caused by an intracranial plaque the risk associated with operating an asymptomatic (coexisting) carotid stenosis can be avoided. In this VIDI proposal I will develop MR intracranial vessel wall and plaque imaging methods that will hunt down the cause of a stroke. The success of the proposed MR techniques depends on increasing their sensitivity with high resolution imaging. Therefore, I will develop new MR methods and use these for stroke applications at recent ultrahigh field strength MRI scanners (7 Tesla) which will make 3D imaging possible with ~5 times shorter scan times compared to 3 Tesla. Proof of principle studies have been proven successful in >50 stroke patients (published: Stroke 2011). With my novel approach the vessel wall of arteries feeding infarcted brain tissue will be scrutinized to detect and characterize plaque/thrombotic/embolic material which can explain the presence of ischemia. MR contrast will be injected to detect plaque/vessel wall enhancement. My novel methods will make it possible to step beyond imaging a luminal stenosis of intracranial arteries. Proof of principle studies will be performed in different stroke patient populations including patients with a yet undetermined cause of stroke. Furthermore, intracranial MRI plaque findings will be validated with specimen-pathology studies. With my expertise in the fields of neuroradiology, MR sequence developments (including a series of projects at 7Tesla), cerebral vasculature and MR patient studies, I provide the expertise in this VIDI project to develop these innovative MR techniques and make them suitable for clinical care.
