De UPGRADE-studie heeft onderzocht of ursodeoxycholzuur (UDCA) het optreden van symptomatisch galsteenlijden verminderd na bariatrische chirurgie (Roux-en-Y gastric bypass of gastric sleeve).

Onderzoek

In totaal hebben 985 patiënten uit drie ziekenhuizen deelgenomen aan de studie. Na de bariatrische operatie heeft de helft van de patiënten gedurende 6 maanden UDCA gekregen en de andere helft placebo. Alle patiënten hebben voorafgaand aan de operatie een galblaasecho ondergaan om reeds aanwezige galstenen vast te stellen en zijn gedurende 24 maanden gecontroleerd.

Resultaten

Deze studie heeft aangetoond dat in de groep patiënten die een gastric bypass ondergaan en die geen galstenen voorafgaand aan de operatie hebben, het risico op symptomatisch galsteenlijden significant wordt verlaagd door het gebruik van UDCA.
Of ditzelfde effect bestaat bij patiënten die alleen een gastric sleeve ondergaan kan niet worden gezegd, gezien het kleine percentage hiervan in de onderzoekspopulatie.
 

Samenvatting bij start

Een Roux-en-Y gastric bypass (maagomleiding) is een operatie voor ernstig overgewicht. Het snelle gewichtsverlies na de operatie is een risicofactor voor het ontwikkelen van galstenen. Van de patiënten die nog een galblaas hebben, krijgt ruim 10% in de eerste 2 jaar na de gastric bypass last van galstenen.

Ursodeoxycholzuur (UDCA) is een medicijn dat effectief is in het voorkomen van galsteenvorming en weinig bijwerkingen heeft. In deze studie zal onderzocht worden of standaard voorschrijven van UDCA aan alle patiënten die een gastric bypass ondergaan en nog een galblaas hebben het risico op galsteenlijden kan verkleinen. Hiertoe worden 980 patiënten verdeeld over twee groepen. Eén groep krijgt tot 6 maanden na de gastric bypass UDCA, de andere groep een placebo. Hierna worden zij gecontroleerd tot 24 maanden na de operatie. We verwachten dat het vóórkomen van galsteenlijden met 50% zal verminderen in de groep die UDCA krijgt. Ook wordt de kosteneffectiviteit onderzocht.

Eindverslag

Snel gewichtsverlies na bariatrische chirurgie is een risicofactor voor het ontwikkelen galstenen. De UPGRADE-studie heeft onderzocht of ursodeoxycholzuur (UDCA) het optreden van symptomatisch galsteenlijden verminderd na bariatrische chirurgie. Patiënten die een Roux-en-Y gastric bypass (maagomleiding) of gastric sleeve (maagverkleining) ondergingen zijn geïncludeerd voor dit onderzoek. In totaal hebben 985 patiënten in drie Nederlandse ziekenhuizen deelgenomen aan de studie. Na de bariatrische operatie heeft de helft van de patiënten voor 6 maanden UDCA gekregen en de andere helft placebo. Patiënten zijn gedurende 24 maanden gecontroleerd op de polikliniek om het ontstaan van galstenen en galsteen gerelateerde klachten te evalueren.

Samenvatting van de aanvraag

The number of bariatric interventions for morbid obesity is increasing rapidly in the Netherlands. Rapid weight loss is a major risk factor for gallstone development. Approximately eleven percent of patients who underwent Roux-en-Y gastric bypass develop symptomatic gallstone disease. Gallstone disease can lead to severe complications and often requires hospitalization and surgery. Ursodeoxycholic acid (UDCA) prevents the formation of gallstones after bariatric surgery. However, randomized controlled trials with symptomatic gallstone disease as primary endpoint have not been conducted. Currently, major guidelines make no definite statement about postoperative UDCA prophylaxis and most bariatric centres do not prescribe UDCA. We will conduct a randomized, placebo-controlled, double-blind multicentre trial. The study population consists of consecutive patients undergoing Roux-en-Y gastric bypass in the MC Slotervaart, OLVG West and MC Zuiderzee Lelystad. Patients will receive a preoperative ultrasound, randomisation will be stratified for patients already having gallstones. The intervention group will receive UDCA 900mg once daily for six months. The placebo group will receive similar-looking placebo tablets. The primary endpoint is symptomatic gallstone disease after 24 months, defined as admission or hospital visit for symptomatic gallstone disease. Secondary endpoints consist of the development of gallstones on ultrasound at 24 months and side-effects of UDCA. Cost-effectiveness, cost-utility and budget impact analyses will be performed with costs per patient with poor outcome, costs per quality adjusted life year, respectively total reimbursement as primary outcomes.
