Herhaalde GON-injecties als preventieve behandeling voor chronische clusterhoofdpijn
Patiënten met chronische clusterhoofdpijn hebben vrijwel dagelijks meerdere aanvallen van extreme pijn rondom één oog. Een aanval duurt tot 180 minuten. Patiënten weten zich dan geen raad van de pijn. De klachten kunnen zo heftig zijn dat dit ook wel ‘zelfmoordhoofdpijn’ wordt genoemd.
Onderzoek
Sinds 2020 wordt een effectieve maar kostbare operatieve vorm van neurostimulatie van de achterhoofdzenuw in de nek (greater occipital nerve; GON) vergoed. Een relatief nieuwe, goedkope en waarschijnlijk veilige behandeling is een injectie met corticosteroïden rondom de GON. Een eenmalige injectie geeft een snelle, weken tot maanden durende, aanmerkelijke of niet zelden zelfs gehele aanvalsvermindering.
Vanuit de patiëntengroep is de vraag ontstaan of herhaalde GON-injecties effectief en veilig zouden kunnen zijn bij chronische clusterhoofdpijn.
Dit onderzoek richt zich op de werkzaamheid, belasting, veiligheid, economische voordelen en het optimale toedieningsinterval van herhaalde GON-injecties.

Samenvatting van de aanvraag
Chronic cluster headache is a rare, devastating, disabling, and very difficult-to-treat brain disorder, typically characterised by unremitting, usually daily, multiple (up to 12) attacks of excruciating unilateral periorbital pain for 15 minutes to 3 hours, accompanied by restlessness and autonomic facial features. It is considered one of the most painful conditions and is not rarely referred to as ‘suicide headache’. The efficacy of currently available prophylactic medication is usually only moderate with a slow onset of efficacy. Attack freedom is rarely achieved. Moreover, these medications are often associated with serious adverse events, significantly hampering their use. Since 2020 an effective, but costly invasive form of neurostimulation of the greater occipital nerve (GON) is reimbursed in The Netherlands. A relatively new, cheap and probably safe treatment is injection of corticosteroids around the GON. A single injection can result in a fast reduction in the number of attacks, even attack-freedom, weeks till months lasting. Because of the good effectivity, convenience and duration of effect after a single administration, single GON-injection seems like a very good treatment for episodic cluster headache. For chronic cluster headache, however, GON-injections should be repeated to achieve prolonged efficacy. Little if anything is known, however, about (i) whether repeated GON-injections also are repeatedly effective; (ii) what the optimal injection interval is; (iii) how often injections can be effectively and safely repeated; (iv) what the gain in quality of life is; and (v) whether the procedure is cost-effective? Because of lack of answers to these seminal questions, repeated GON-injections are hardly used in in the Netherlands in chronic cluster headache. Our study aims to provide solid answers to the crucial questions raised above.
