Optimal antibiotic use: an 'antibiotic checklist' for all bacterial infections in the hospital

Verbetering van antibioticagebruik is noodzakelijk om de ontwikkeling van antibioticaresistentie te remmen. In deze studie is een ‘antibioticachecklist’ ontwikkeld, bedoeld als hulpmiddel om de arts te herinneren aan de belangrijkste stappen in antibiotische behandeling. De checklist is in negen Nederlandse ziekenhuizen geïntroduceerd en het effect is geëvalueerd.

Resultaten

Door het gebruik van de checklist verbeterde de kwaliteit van antibioticagebruik significant. Hoewel de kwaliteit van het antibioticagebruik geassocieerd was met de opnameduur van de patiënt, leidde het checklistgebruik zelf niet tot een verkorte opnameduur. Reden hiervan was dat artsen regelmatig wel een vinkje zetten, maar het punt zoals genoemd op de checklist in de werkelijkheid niet uitvoerden. Uiteraard is het daadwerkelijk uitvoeren noodzakelijk om effect te bereiken.
De antibioticachecklist bleek (kosten)effectief en bovendien makkelijk inzetbaar. Desalniettemin moet introductie van de checklist samengaan met een gedetailleerd implementatieplan, uit deze studie bleek namelijk dat de checklist voor maar 19% van de patiënten werd ingevuld waarvoor hij had kunnen worden ingevuld.
 

Samenvatting bij start

De inspectie voor de Volksgezondheid heeft alle ziekenhuizen een brief gestuurd met de opdracht een plan te maken om de toename in antimicrobiële resistentie te verminderen. Goed antibioticagebruik is één van de kernpunten. Wij hebben kwaliteitsindicatoren ontwikkeld voor het meten van ‘goed antibioticagebruik’ bij infecties in het ziekenhuis. Het doel van dit project is om deze indicatoren te vertalen in een checklist voor alle bacteriële infecties in het ziekenhuis, welke goed antibioticagebruik zal bevorderen door gezondheidsmedewerkers in de dagelijkse praktijk te herinneren aan de aanbevolen antimicrobiële therapie. De checklist wordt samengesteld, in de praktijk geïntroduceerd, getest en indien nodig aangepast. De uitkomsten voor en na introductie worden vergeleken en zijn o.a. goed antibioticagebruik, gemeten met de indicatoren en opnameduur in het ziekenhuis. De studiepopulatie bestaat uit 800 volwassen patiënten (uit 8 ziekenhuizen) met een bacteriële infectie.

Eindverslag

In deze stepped-wedge cluster gerandomiseerde studie hebben wij een antibioticachecklist (AB-checklist)geïntroduceerd in 9 Nederlandse ziekenhuizen, op zowel snijdende- als niet-snijdende afdelingen. De checklist is gebaseerd op gevalideerde kwaliteitsindicatoren, die adequaat antibioticagebruik definiëren, en is bedoeld als hulpmiddel voor de arts bij het voorschrijven van therapeutische antibiotica die intraveneus worden gestart. We hebben geëvalueerd wat het effect is van checklist gebruik op de opnameduur van de patiënt, adequate antibiotische therapie (= opvolgen van de checklist items), en kosten.

De antibioticachecklist (AB-checklist) is gebaseerd op gevalideerde kwaliteitsindicatoren. Deze checklist is bedoeld als hulpmiddel voor de arts bij het voorschrijven van therapeutische antibiotica die intraveneus worden gestart. In dit project wordt de effectiviteit van de AB-checklist getest in een stepped-wedge cluster randomized studie, waarin 9 ziekenhuizen participeren.
In elk ziekenhuis is er eerst een voormeting gedaan naar opnameduur en het huidige antibioticagebruik; vervolgens is in elk ziekenhuis de checklist geïntroduceerd. De primaire uitkomstmaat is de opnameduur van de patiënt die intraveneuze antibiotische therapie ontvangt. Secundaire uitkomstmaten zijn het opvolgen van de kwaliteitsindicatoren, het gebruik van antibiotica, en kosten. Op dit moment worden de data betreffende deze eindpunten verzameld. In de voormeting zijn in totaal 816 patiënten geïncludeerd, en in de nameting zijn er 1073 checklists ingevuld. De berekende sample size is hiermee ruim gehaald. De analyse van de resultaten moet nog volgen, waarin wordt bepaald of deze checklist inderdaad effectief is.
Als dat zo blijkt te zijn, zou de checklist mogelijk in de toekomst in alle Nederlandse ziekenhuizen gebruikt kunnen worden, en bovendien ingebouwd kunnen worden in het elektronisch patiëntendossier.

Samenvatting van de aanvraag

BACKGROUND In order to address the steadily increasing resistance problem, the Dutch Healthcare Inspectorate recently ordered all hospitals to install an Antibiotic Stewardship Program (ASP), which should be functioning in 2014. Improving appropriate antibiotic use is one of the cornerstones of this program. Appropriate antibiotic use can be defined, and measured, by guideline-based quality indicators (QIs). We recently developed QIs for antimicrobial treatment of all bacterial infections in the hospital. OBJECTIVE The goal of the present proposal is to translate these QIs into an innovative “antibiotic checklist” for all bacterial infections in the hospital. This checklist will guide daily practice by functioning as a reminder for all Health Care Workers. The checklist will be constructed, and thereafter introduced in clinical practice, validated, and when needed adapted. At the end of the project, the checklist will be ready for national implementation. DESIGN AND ENDPOINTS The “antibiotic checklist” will be introduced and evaluated in a multicenter, prospective study comparing outcomes before and after introduction using a stepped wedge design. Primary endpoints are appropriate antibiotic use measured by adherence to the QIs and the duration of hospital stay,. Secondary endpoints are antibiotic use, mortality and costs. SAMPLE SIZE The study population consists of all adult patients with a bacterial infection in the hospital (8 hospitals x 100 =800 patients). COST-EFFECTIVENESS ANALYSIS Costs associated with introduction and use of the checklist in clinical practice will be offset to the effectiveness.
