Dosis-individualisatie van Beta-lactam antibiotica op de IC, verbetering voor de patiënt?

Patiënten op de Intensive Care (IC) hebben frequent infecties, waarbij veel antibiotica worden gebruikt. De antibioticadosis die nu gebruikt wordt leidt vaak tot een niet optimale behandeling bij IC-patiënten. Een inadequate behandeling leidt tot verhoogde mortaliteit en morbiditeit.

Studie

In de DOLPHIN studie is onderzocht of door het meten van de antibioticaconcentratie in het bloed (TDM) de dosis zo spoedig mogelijk kan worden aangepast en zo de blootstelling te verbeteren.
De patiënten werden in twee groepen verdeeld; een groep kreeg de standaarddosering en de andere groep kreeg eerst standaarddosering die vervolgens werd aangepast met TDM.
Het onderzoek is op 8 IC’s in Nederland uitgevoerd bij 390 patiënten.

Resultaten

De DOLPHIN studie heeft laten zien dat het niet effectief is om bij alle patiënten op de IC een persoonlijke antibioticadosis met TDM te geven. Echter, patiënten met minder ernstige ziekte of een TDM-interventie binnen 24 uur hadden wel baat bij TDM; de ligduur op de IC nam af.
 

Samenvatting bij start

Intensive Care (IC) patiënten hebben frequent infecties, waarvoor veel antibiotica worden gebruikt. De dosis die nu gebruikt wordt leidt vaak tot een niet optimale behandeling. Inadequate behandeling kan hogere mortaliteit en langer ziekenhuisverblijf tot gevolg hebben.

Doel

Het doel van deze multicenter studie is het verbeteren van de behandeling van volwassen IC-patiënten door geïndividualiseerd te doseren. Hiervoor krijgt de helft van de patiënten de standaardbehandeling en wordt deze vergeleken met de andere helft van de patiënten waarvoor de dosering in een vroeg stadium wordt aangepast op geleide van een concentratiemeting van het antibioticum in het bloed. Dit laatste wordt ook wel TDM (Therapeutic Drug Monitoring) genoemd. Het primaire eindpunt is verkorting van de opnameduur.
Voor de patiënt met de TDM gestuurde dosering wordt de antibioticum-concentratie ingevoerd in een model en dezelfde dag wordt een advies gegeven aan de IC-arts. In de standaard zorggroep wordt de dosering niet aangepast.

Samenvatting van de aanvraag

Mortality in critically ill patients with infection is a global health problem. Traditional antibiotic dosing is not designed for the complexity of critically ill intensive care unit (ICU) patients. Sub optimal dosing will lead to longer ICU and hospital stay and eventually higher health costs. ,A 'one-dose-fits-all' approach seems inadequate to reach the pharmacodynamic target in ICU patients. Therapeutic drug monitoring(TDM), determination of drug levels with the application of pharmacokinetic and pharmacodynamic principles to optimize dosage regimens, is likely to increase reachment of pharmacodynamic target. Beta-lactam antibiotics are the cornerstones of anti-infective therapy on the ICU. In several international studies it has been shown that beta-lactams are highly underdosed in ICU patients and we have found similar results in our EXPAT study. Given the problems described above, it is surprising that dose individualization of beta-lactams is hardly practised. However, randomized controlled clinical studies with primary patient outcomes showing the benefit of TDM are lacking so far. We propose a randomized study comparing active TDM versus standard dosing of beta-lactams. The study is performed in 5 large hospitals, 422 patients will be included receiving antibiotics, according to standard clinical care. Blood levels of antibiotics are determined at defined time points and are combined with microbiology data to set an individualized TDM advise for the individual ICU patient in the active TDM group. In the standard group blood levels will be analyzed in bulk later. Primary objective: To show that the length of stay at the ICU decreases significantly in the group with active TDM (intervention group) in comparison to the group with no TDM (control group). Length of IC stay is associated with less mortality and less post-IC syndrome. Secondary objectives; to show that active TDM results in improved clinical outcome, cost-effectiveness and quality of life
