CLEAR-CAD Trial: Clinical outcomes and cost-effectiveness of a diagnostic and treatment strategy of upfront CTCA plus selective non-invasive functional imaging compared with standard care in patients with chest pain and suspected Coronary Artery Disease

Projectomschrijving

Patiënten met pijn op de borst ondergaan doorgaans diagnostisch onderzoek om kransslagaderlijden te beoordelen. Dit kan uit verschillende onderzoeken bestaan, waaronder een inspanningsonderzoek, verschillende soorten scans, of een hartkatheterisatie. Echter beoordeeld een deel van deze onderzoeken de aanwezigheid van kransslagaderlijden niet optimaal, waardoor patiënten onderbehandeld worden en risico lopen op een hartinfarct. De zorgkosten voor dit onderzoek zijn erg hoog.

Onderzoek

In dit onderzoek, uitgevoerd in meer dan 30 Nederlandse ziekenhuizen, vergelijken we een nieuwe strategie met de gebruikelijke zorg. In deze nieuwe strategie wordt er primair een CT-scan van de kransslagaders gemaakt, waarna verder beleid volgt. Door loting worden ±6500 volwassenen met pijn op de borst toegewezen aan een van deze strategieën.

Verwachte uitkomsten

We evalueren of de CT-gedreven strategie leidt tot betere uitkomsten voor patiënten, en of het leidt tot een zorgkosten besparing.

Samenvatting van de aanvraag

VRAAGSTELLING Wat is het effect op de klinische uitkomsten en de kosteneffectiviteit van een diagnostische en behandelingsstrategie van primair CTCA met selectieve niet-invasieve functionele beeldvorming bij patiënten met verdenking op coronarialijden (CAD), in vergelijking met hedendaagse standaard zorg? HYPOTHESE Deze strategie zal de incidentie van overlijden en niet fatale myocardinfarcten verlagen en doelmatig blijken PATIENT (P) Poliklinische patiënten met pijn op de borst en verdenking op CAD, zonder bekend obstructief CAD of verdenking op acuut coronair syndroom. INTERVENTION (I) Primair CTCA, beoordeeld middels CAD-RADS-classificatie. Bij CAD-RADS 0 wordt CAD uitgesloten. Bij CAD-RADS 1&2 wordt obstructief CAD uitgesloten en optimale preventieve medicamenteuze therapie (OMT) gestart. Bij CAD-RADS =3 wordt obstructief CAD gediagnosticeerd en OMT met antiangineuze medicatie (OMT+) gestart. Bij aanhoudende klachten onder OMT + zal aanvullende niet-invasieve functionele beeldvorming worden verricht, waarna revascularisatie volgt bij >10% myocardischemie in overleg met het hartteam. COMPERATOR (C) Standaard zorg. De diagnostiek en behandeling worden bepaald door de behandelend cardioloog. OUTCOME (O) 1. Overlijden en niet fataal myocardinfarct (MACE) 2. Kosten per voor kwaliteit gecorrigeerd levensjaar (QALY) FOLLOW-UP TIME 3 jaar STUDIE OPZET Een prospectieve gerandomiseerde gecontroleerde studie om klinische superioriteit van de interventie aan te tonen SAMPLE SIZE BEREKENING/DATA-ANALYSE We verwachten een MACE-incidentie van 2.82% (standaard zorg) en 1.70% (primair CTCA). Uitgaande van 80% power zijn 3,061 patiënten per arm nodig. Inclusief 5% uitvalpercentage zijn in totaal 6454 deelnemende patiënten nodig (+10 voor analyse tijd tot MACE). KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Kostenutiliteitsanalyse van de combinatie van routinematig CTCA, selectieve niet-invasieve functionele beeldvorming en gerichte behandeling vanuit maatschappelijk perspectief en een middellange tijdshorizon in vergelijking met standaardzorg. Budget impact analyse vanuit de perspectieven van overheid, zorgverzekeraar en zorgaanbieder. Analyse van informatiewaarde van vervolgonderzoek ter reductie van resterende onzekerheid van modeluitkomsten. TIJDPAD 01-2022 Project start/ voorbereiding 04-2022 Start Inclusie (12 maanden inclusie periode) 04-2024 Voltooiing 1 jaar follow-up 04-2026 Voltooiing 3 jaar follow-up 04-2027 Project einde RESEARCH QUESTION What is the clinical effect and cost-effectiveness of a combined diagnostic and treatment strategy in patients with suspected coronary artery disease (CAD)? This strategy comprises of upfront computed tomography coronary angiography (CTCA) with selective non-invasive functional imaging and patient tailored therapy. HYPOTHESIS This strategy will result in a reduction reduce the incidence of major adverse cardiovascular events (MACE) defined as death or non-fatal myocardial infarction and appear more efficient than care as usual. PATIENT (P) Outpatients with chest pain and suspected CAD. It does not concern patients with known CAD or suspected acute coronary syndrome. INTERVENTION (I) Upfront CTCA with standardized reporting through CAD-RADS classification. CAD-RADS 0 rules-out CAD. CAD-RADS 1 & 2 rules-out obstructive CAD with high certainty and optimal preventive drug therapy (OMT) is then initiated. In patients CAD-RADS =3, obstructive CAD is assumed and OMT will be started with additional anti-anginal medication (OMT +). In this latter group, additional non-invasive functional imaging will be performed if anginal complaints persist under OMT +. Patients with> 10% myocardial ischemia on imaging will undergo revascularization in consultation with the cardiac team. COMPERATOR (C) Standard of care. Diagnosis and treatment are at the discretion of the attending cardiologist. OUTCOME (O) 1- Occurrence of MACE, defined by mortality or non-fatal myocardial infarction 2- Costs per quality adjusted life year (QALY) FOLLOW-UP TIME 3 years STUDY DESIGN A prospective randomized controlled trial to demonstrate clinical superiority of the intervention SAMPLE SIZE CALCULATION / DATA ANALYSIS We expect a MACE incidence of 2.82% (standard care) and 1.7% (upfront CTCA). For 80% power, 3061 patients are needed per group. Including 5% dropout rate, 6454 participating patients are required (+10 to include time to MACE). COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS Cost-utility analysis of the combination of upfront CTCA, selective non-invasive functional imaging and targeted therapy from a societal perspective and with a mid-term horizon in comparison with standard care. Budget impact assessment from governmental, health insurer and health care provider perspectives.
