Ieder jaar ondergaan circa 12,000 Nederlanders een lumbale hernia operatie. Uit de resultaten van wetenschappelijk onderzoek blijkt echter dat de lange termijn effecten van deze operaties gelijk zijn aan die van conservatieve behandelingen, zoals injecties en (specifiek op de patiënt afgestemde) fysiotherapie. Daarom hebben we onlangs een pilot studie uitgevoerd naar de werkzaamheid van een combinatietherapie waar injecties en speciale vorm van fysiotherapie worden gecombineerd. Het bleek dat veel mensen die in aanmerking kwamen voor een hernia operatie, deze na de combinatietherapie uiteindelijk toch niet nodig hadden.
De ‘PLUS studie’ (Preventing LUmbar Disc Surgery) onderzoekt de effectiviteit en kosteneffectiviteit van die combinatietherapie, voor mensen met een lumbale (lage rug) hernia, met als doel het aantal hernia operaties te laten dalen. De ‘PLUS studie’ wordt gecoördineerd door het EMGO Instituut van het VU/VU Medisch Centrum.
Deze studie is voortijdig gestopt. De onderzoeksvragen zijn niet (volledig) beantwoord.

NEDERLANDS VRAAGSTELLING Is een combinatietherapie (Mechanische Diagnose & Treatment) [MDT] & Transforaminale Epidurale Steroïde Injecties [TESI]) kosteneffectief vergeleken met usual care bij patiënten met een lumbale hernia? HYPOTHESE De combinatie therapie voorkomt minimaal 30% van de lumbale hernia operaties. STUDIE OPZET RCT met een kostenutiliteits- en kosteneffectiviteitsanalyse en een budget impact analyse. PATIENTEN Patiënten met een indicatie voor een operatie vanwege lumbale hernia INTERVENTIE De combinatietherapie (MDT & TESI) wordt uitgevoerd door een anesthesioloog en een fysiotherapeut getraind in MDT. Eerst worden patiënten onderzocht volgens MDT principes om de pijnresponse van de patiënt op het MDT test protocol te classificeren als "centraliserende pijn" of "non-centraliserende pijn". Op basis van deze classificatie krijgt de patiënt een op maat gesneden behandeling. Deze behandeling betreft een combinatie van injectie(s) en specifieke MDT oefeningen en gericht advies behorende bij de classificatie. GEBRUIKELIJKE ZORG Een lumbale hernia operatie UITKOMSTMATEN -Primaire uitkomstmaat: aantal lumbale hernia operaties binnen een termijn van 12 maanden -Secundaire uitkomsten: pijn (been en rug), functioneren en kwaliteit van leven. Gemeten op 2, 4, 6, 9 & 12 maanden. -Kosten: Interventiekosten, alle relevante kosten binnen gezondheidszorg, kosten voor informele zorg, en alle arbeid gerelateerde kosten worden gemeten op dezelfde tijdstippen met een kostenvragenlijst. SAMPLE SIZE 146 patienten zijn nodig om een verschil van minimaal 30% in de primaire uitkomstmaat te detecteren (95% power, alpha 0,05, 20% drop out, en een ICC van 0.15). ANALYSE De intention-to-treat analyse (linear mixed model & generalized mixed model, inclusief multiple imputation techniques) analyseert de data gebruikmakend van alle meetmomenten (baseline, 2, 4, 6, 9 & 12 months) (levels: ziekenhuis, patient & meetmoment) KOSTENEFFECTIVITEITSANALYSE / BUDGET IMPACT ANALYSE De economische evaluatie wordt uitgevoerd vanuit een maatschappelijk en gezondheidszorg perspectief. Onzekerheid rondom ICERs wordt geschat d.m.v. bootstrapping. De budget impact analyse gebruikt Nederlandse epidemiologische data & Markov modellen. Het maatschappelijk, gezondheidszorg en verzekeraars perspectief worden meegenomen. TIJDSPAD Totaal 36 (maanden): voorbereiding, METC goedkeuring, (0-5); selectie & inclusie patiënten (6-22); follow-up (6-32); analyse en rapportage (32-36) IMPLEMENTATIEPLAN & PROBLEEMANALYSE Een implementatie plan wordt gemaakt o.a. op basis van een probleemanalyse-studie voor implementatie ENGELS RESEARCH QUESTION Is a combination therapy (Mechanical Diagnosis & Treatment [MDT] & Transforaminal Epidural Steroid Injections [TESIs]) cost-effective compared to usual care for herniated lumbar discs? HYPOTHESIS Combination therapy prevents 30% of lumbar disc surgeries DESIGN RCT with economic evaluation PATIENTS Patients with leg pain and an indication for lumbar herniated disc surgery INTERVENTION Combination therapy delivered by anesthesiologists and physiotherapists trained in MDT. Patients are first classified, based on their response patterns to the specific MDT test procedures as ‘centralizing’ or ‘non-centralizing’ and receive targeted treatment (MDT exercises and/or TESIs) based on this classification. COMPARISON The control group will receive lumbar disc surgery OUTCOMES -Primary: proportion of patients undergoing surgery during 12 months follow-up. -Secondary: Pain (leg and back), functioning, return-to-work & health-related quality of Life outcomes will be measured at 2, 4, 6, 9 & 12 months. -Costs: costs of the intervention, other healthcare, informal care, and patient related and productivity costs will be measured using cost-questionnaires at 2, 4, 6, 9 & 12 months. SAMPLE SIZE 146 patients are needed to detect a difference of at least 30% with 95% power, alpha = 0,05, taking 20% drop out into account, and an ICC of 0.15. ANALYSIS The intention-to-treat analysis (linear mixed model/ generalized mixed model, using multiple imputation techniques) will use the responses at baseline and 2, 4, 6, 9 and 12 months (levels: hospital, patient and time) COST-EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS The economic evaluation will be conducted from the health care and the societal perspective. Resource utilization and costs will be measured using a cost questionnaires and valued using Dutch standard prices. Bias-corrected and accelerated bootstrapping with 5000 replications will be done to estimate the uncertainty surrounding the ICERs. The budget impact analysis uses Dutch epidemiological data and Markov models. The societal, healthcare, and insurer perspectives are considered. TIME SCHEDULE a total of 36 (months): preparation, approval METC (0-5); selection & recruitment (6-22); follow-up of patients (6-32); analysis and publication (32-36) IMPLEMENTATION PLAN & PROBLEM ANALYSIS An implementation plan will be developed based on a 'problem analysis study'
