Is endoscopic trans-sphenoidal selective adenoma resection superior and cost-effective compared to medical treatment with cabergoline? A cohort multiple randomized controlled trial for treatment of microprolactinomas.

Projectomschrijving

Een prolactinoom is een tumor aan de hypofyse en kan behandeld worden met medicijnen of met een operatie. Veel patiënten ervaren bijwerkingen van deze medicijnen. Hypofyseoperaties zijn de laatste jaren verbeterd waardoor er minder complicaties optreden en patiënten vaker genezen. Momenteel is niet duidelijk welke behandeling beter is: medicijnen of operatie. Om dit te onderzoeken zijn er 2 onderzoeken opgezet: 1) Een cohort studie waarbij patiënten met alle vormen van prolactinoom de standaard zorg krijgen; 2)  Patiënten met een bepaalde vorm van prolactinoom worden o.b.v. behandelfase (diagnose, na kort medicatie of na lang medicatie) verdeeld in 3 RCTs, waarbij  de standaard zorg wordt vergeleken met een operatie. De studie is voortijdig beëindigd doordat de inclusie van patiënten in de RCT zeer moeizaam verliep, voornamelijk doordat patiënten een behandelvoorkeur hadden. Desondanks is en veel belangstelling voor de studie. De studie zal met andere middelen worden voortgezet.

Deze studie is voortijdig gestopt. De onderzoeksvragen zijn niet (volledig) beantwoord.

Samenvatting van de aanvraag

VRAAGSTELLING: Wat is de beste behandeling voor patiënten met een duidelijk, niet te groot prolactinoom qua kans op genezing, kwaliteit van leven (HR-QOL), negatieve effecten (zoals bijwerkingen van medicatie en complicaties) en welke behandeling is kosteneffectief? ACHTERGROND EN HYPOTHESE: Genezing door operatie (90%) geeft betere kwaliteit van leven dan langdurig onderdrukking van hormoonoverproductie met medicijnen (dopamine-agonisten, Cabergoline) en is tevens kosteneffectief. STUDIE OPZET: Een pragmatische studie met modern design: groot cohort patiënten met standaard behandeling, waaruit patiënten geselecteerd worden voor drie gecontroleerde gerandomiseerde trials (RCTs). In de drie RCT wordt operatie met medicatie vergeleken: 1) nieuwe patiënten, niet eerder voor prolactinoom behandeld, 2) nieuwe patiënten, die eerst zes maanden met cabergoline behandeld worden, 3) bekende patiënten, die tenminste 2 jaar met cabergoline behandeld zijn. STUDIEPOPULATIE: Cohort: Nieuwe en bestaande patiënten, > 18 jaar met op MRI scan een prolactinoom > 4 mm. RCTs: Tevens goede kans op chirurgische curatie (4 mm – 1.5 cm) zonder contra-indicaties voor operatie. INTERVENTIE: 1. Cabergoline oraal, dosisescalatie tot genormaliseerd prolactine, stop na 2 jaar. 2. Endoscopische trans-sphenoidale selectieve resectie van prolactinoom GEBRUIKELIJKE ZORG/ VERGELIJKING: Cabergoline, een dopamineagonist, is eerste keus, operatie is tweede keus behandeling in geval van bijwerkingen of resistentie. Patiënten geven aan last te hebben van (milde) bijwerkingen van Cabergoline, met name stemmings- en soms gedragsveranderingen. Volgens een patiënten enquête (180 patiënten) en eigen onderzoek is dit een reëel probleem. In de afgelopen 15 jaar zijn operaties aanzienlijk beter en veiliger geworden dankzij de endoscoop. Operaties worden meestal goed verdragen en er is korte opnameduur. UITKOMSTMATEN: Curatie na 36 maanden; HR-QOL na 12 maanden; bijwerkingen, veiligheid, kosten. SAMPLESIZE BEREKENING/ DATA ANALYSE: Cohort 2000 patiënten, RCTS: 200 patiënten per RCT. Er is rekening gehouden dat circa 30% van de patiënten na voorlichting toch niet voor operatie kiest. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE: In de studie worden de EQ5D, directe en indirecte kosten gemeten. Een budget impact analyse wordt uitgevoerd voor de verschillende scenario’s qua timing van de operatie. TIJDSPAD: Inclusie 2 jaar (2018 -2020), medicamenteuze behandeling 2 jaar (2018-2022), follow-up na staken medicatie 1 jaar (2021-2023). --- RESEARCH QUESTIONS: Which treatment is better with respect to quality of life (HR-QOL), cure rate and cost effectiveness for patients with a distinct prolactinoma: trans-sphenoidal endoscopic surgery or cabergoline? HYPOTHESIS: Cure by surgery (90%) has higher HR-QOL than long-term suppression of hormone overproduction by medication (dopamine agonist, Cabergoline) and is cost-effective. STUDY DESIGN: A cohort multiple randomized controlled trial will be conducted in which all new and prevalent patients can be included in the cohort and subsets are randomized to (continue with) standard treatment with cabergoline versus (counselling for) trans-sphenoidal surgery. Three independent RCTs are conducted: 1) new drug naïve patients, 2) patients after 6 months pre-treatment with cabergoline, and 3) patients after 2 years or more treatment with cabergoline. STUDY POPULATION(S)/DATASETS: Patients, > 18 years, with biochemically and radiologically proven prolactinoma. For the RCT the prolactinoma should have a high chance to be cured by surgery, i.e. a size of 4 -1.5 cm and no invasive growth and the patient should be fit for surgery. INTERVENTION: 1. Standard treatment with cabergoline for 2 years with a normalized prolactin. 2. Endoscopic trans-sphenoidal surgery with selected removal of the adenoma. USUAL CARE /COMPARISON: Cabergoline is current standard treatment, with very high control rate, but (mild) psychological side effects of the drug. The cure rate after 2 years is 20-40 %, so long-term treatment is needed in the majority. Endoscopic surgery is currently second line treatment for this indication but is a well-established treatment for all other pituitary adenoma. Innovations have improved the safety and tolerance of the procedure and the cure rates of small adenomas are considered very high in expert hands. OUTCOME MEASURES: Primary outcome: Cure at 36 months, HR-QOL at 12 months; secondary outcome: negative effects (side effects, complications) and costs. SAMPLE SIZE CALCULATION/DATA ANALYSIS: Cohort 2000 patients, RCT 200 patients per RCT taking into account a subset of patients that prefers not to be operated after counselling. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS: Evaluation of direct and indirect health costs, and HR-QOL to estimate QALY. TIME SCHEDULE: inclusion 2 years (2018-2020), medical treatment 2 years (2018-2022), follow-up after cessation of medical treatment 1 year (2021-2023).
