De gebruikelijke behandeling voor urine-incontinentie bij kinderen is blaastraining. Dit kost veel tijd en inzet. Regelmatig worden daarbij hulpmiddelen gebruikt. De SENS-U is een nieuw hulpmiddel.

De SENS-U is een klein kastje met een ingebouwde echo dat op de buik wordt geplakt. Het meet voortdurend de hoeveelheid urine en trilt bij een volle blaas. Anders dan bestaande hulpmiddelen geeft de SENS-U een seintje vóórdat er urineverlies optreedt. Dit helpt kinderen om sneller te begrijpen hoe een volle blaas aanvoelt.

Blaastraining met de SENS-U leidt mogelijk tot een kortere behandelduur. De resultaten zijn bruikbaar voor huisartsen, jeugdgezondheidsartsen, kinderartsen, urotherapeuten, kinderfysiotherapeuten en urologen.

Samenvatting van de aanvraag

DOEL/VRAAGSTELLING: Verbetert de SENS-U (continue monitoring van blaasvulling met draagbaar echografisch apparaat) de kosteneffectiviteit van standaard urotherapie en specifieke urotherapie (3e lijn)? HYPOTHESE Urotherapie met de SENS-U is 15% effectiever (15% meer continente kinderen na behandeling) dan urotherapie zonder de SENS-U. STUDIE OPZET Multicenter gerandomiseerde studie waarbij urotherapie, urotherapie met placebo (sham device) en urotherapie met de SENS-U vergeleken worden. Randomisatie per centrum, met deelname van 4 centra. STUDIEPOPULATIE/ DATABRONNEN Kinderen van 6 t/m 16 jaar die in aanmerking komen voor urotherapie vanwege functionele urine incontinentie overdag. INTERVENTIE Urotherapie met de SENS-U. De SENS-U is een draagbaar echografisch apparaat dat continue de blaasinhoud meet en alarmeert als een vooraf ingesteld blaasvulling bereikt wordt (bijv. 80% vulling). De SENS-U geeft biofeedback waardoor het kind leert bij welk gevoel van de blaas (bijv. het gevoel bij 80% vulling) het moet gaan plassen. GEBRUIKELIJKE ZORG/ VERGELIJKING Urotherapie zonder SENS-U of met een sham apparaat (random alarm). UITKOMSTMATEN Primaire uitkomstmaat is het percentage continente kinderen na 3 maanden urotherapie. Secundaire uitkomstmaten zijn lange termijn uitkomsten,subjectieve ervaringen, therapie trouwheid, mictiefrequentie en -volumina, uroflowmetrie curven en residumetingen en kwaliteit van leven. SAMPLE SIZE BEREKENING/ DATA ANALYSE De effectiviteit van urotherapie is 54% continente kinderen, het verwachte effect van de sham is identiek. De SENS-U voegt naar verwachting 15% effectiviteit toe (69% continente kinderen). De sample size is berekend op 3 paarsgewijze vergelijkingen van de proporties continentie in de 3 groepen. Met a = 0,05, power = 0,80, verwachtte uitval van 33% komt dit uit op 160 kinderen per groep (1:1:1 randomisatie) met een totaal van 480 kinderen verdeeld over 4 centra (120 kinderen per centrum). KOSTEN EFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE Met een incidentie van 8.600 kinderen per jaar met functionele incontinentie overdag verwachten wij bij 80% implementatie van de SENS-U een kostenbesparing van €29.446.400,-. TIJDSPAD In de 1e 4 maanden wordt personeel van de deelnemende centra geïnformeerd en getraind. Inclusie nadien is 24 maanden. Behandeling en follow-up na inclusie is in het totaal 33 maanden. In de laatste 11 maanden vindt data analyse, presentatie en publicatie plaats. Bij bewezen kosteneffectiviteit vindt gedurende de studie implementatie plaats. OBJECTIVE/ RESEARCH QUESTION Does SENS-U (continuous ultrasonic bladder monitoring) improve the cost-effectiveness of standard urotherapy and specific urotherapy (3rd line). HYPOTHESIS Urotherapy with additional SENS-U is 15% more effective (15% more continent children after treatment) than urotherapy without SENS-U. STUDY DESIGN Multicenter RCT comparing urotherapy alone, with SENS-U, and with sham device. Children are randomized per center. There are four participating centers. STUDY POPULATION/ DATASETS Children aged 6 to 16 years with functional daytime urinary incontinence eligible for urotherapy. INTERVENTION Urotherapy with the SENS-U. The SENS-U is a wearable ultrasound device that continuously measures bladder filling and gives an alarm at a preset bladder filling (e.g. 80% filling). The SENS-U provides biofeedback that teaches the child at which feeling of the bladder (e.g. the feeling corresponding with 80% filling) the child has to void. USUAL CARE/COMPARISON Urotherapy without SENS-U or with sham device (random alerts). OUTCOME MEASURES Primary outcome is the percentage of continent children after 3 months of urotherapy. Secondary outcomes are long term outcomes, subjective experiences, adherence, voiding frequencies, post-void residuals and volume, uroflowmetry curves, and Quality of life. SAMPLE SIZE/ DATA ANALYSIS The effectiveness of urotherapy is 54% (continent children). The sham device is believed to have the same effect. We expect 15% additional effect with the SENS-U. Sample size is based on three pairwise comparisons of continence proportions between the 3 groups. With a = 0.05, power = 0.80, and 33% lost to follow-up we need 160 children per group (1:1:1 randomization).This results in a total of 480 children divided over 4 centers (120 children each center). COST-EFFECTIVINESS/BUDGET IMPACT ANALYIS With an estimated incidence of 8.600 children a year with functional daytime urinary incontinence and 80% implementation of the SENS-U, we expect a potential saving of €29.446.400,-. TIME SCHEDULE In the first 4 months, personnel of participating centers are informed and trained. The 24 months thereafter are for inclusion. Treatment and follow-up after inclusion will take 33 months in total. In the last 11 months, data are analyzed, presented, and published. If the SENS-U is cost-effective, implementation will take place alongside the study.
