Effectiveness of continuous glucose monitoring during pregnancy in women with type 1 and type 2 diabetes.

Diabetes tijdens de zwangerschap geeft een hoog risico op complicaties. De GlucoMOMS studie heeft onderzocht of het aanvullend gebruik van een nieuwe techniek, de Continue Glucose Monitor (CGM) leidt tot een betere zwangerschapsuitkomst dan de standaard behandeling. Ruim 300 zwangeren met diabetes type 1 of 2 of met een insuline afhankelijke zwangerschapsdiabetes werden willekeurig geloot voor ofwel aanvullend gebruik van de retrospectieve CGM elke 6 weken ofwel standaard behandeling. Primair werd er gekeken naar de belangrijkste complicatie van diabetes in de zwangerschap: kinderen met een hoog geboortegewicht. Er werd geen verschil gezien tussen beide groepen. Behalve minder vaak een zwangerschapsvergiftiging bij CGM gebruik werd er geen verschil gezien in zwangerschapsuitkomsten van moeder en kind. Concluderend kan gezegd worden dat de behandeling van diabetes tijdens de zwangerschap mbv een retrospectieve CGM niet effectief is.

Diabetes tijdens de zwangerschap geeft een hoog risico op complicaties. De GlucoMOMS studie heeft onderzocht of het aanvullend gebruik van een nieuwe techniek, de Continue Glucose Monitor (CGM) leidt tot een betere zwangerschapsuitkomst dan de standaard behandeling. Ruim 300 zwangeren met diabetes type 1 of 2 of met een insuline afhankelijke zwangerschapsdiabetes werden willekeurig geloot voor ofwel aanvullend gebruik van de retrospectieve CGM elke 6 weken ofwel standaard behandeling.
Diabetes tijdens de zwangerschap geeft een hoger risico op complicaties voor moeder en kind zoals een hoog geboortegewicht van het kind, zwangerschapsvergiftiging en keizersnede. Een goede duiker regulatie tijdens de zwangerschap is van groot belang. De GlucoMOMS studie onderzoekt of het aanvullend gebruik van de Continue Glucose Monitor resulteert in minder complicaties.

Samenvatting van de aanvraag

OBJECTIVE: Hyperglycemia in pregnancy is associated with poor perinatal outcome. Even if pregnant women with diabetes are monitored according to current guidelines, they do much worse than their normoglycemic counterparts. The Continuous Glucose Monitoring System (CGMS) is an efficacious new method to optimize glucose control in pregnant women with diabetes. In the present proposal, we aim to assess the cost-effectiveness of the use of CGMS in type 1 and 2 diabetic pregnancies. STUDY DESIGN: Multicenter open label randomized clinical trial (RCT) with a decision and cost-effectiveness study alongside it. STUDY POPULATION: Pregnant women with type 1 and 2 diabetes. INTERVENTION: Consenting women will be randomly allocated to either additional use of CGMS or usual care. All women will determine their glycemic control by self-monitoring of blood glucose levels and HbA1c. In addition, women allocated to CGMS will use CGMS every month and adjust their insulin regimen based on their CGMS profile. OUTCOME MEASURES OF THE RCT: Primary outcome of the RCT will be macrosomia rate, defined as a birth weight above the 90th centile. Secondary outcomes will be birth weight, composite neonatal morbidity, maternal outcome and costs. The analyses will be according to the intention to treat principle. SAMPLE SIZE CALCULATION: We anticipate that a reduction of macrosomia from 45% to 30% will outweigh the costs of the additional use of the CGMS. Under these assumptions, we need to randomize 300 women (two groups of 150). ECONOMIC EVALUATION: As we expect a reduction of macrosomia but an increased use of resources in the intervention group, the economic analysis will be a cost-effectiveness analysis, with the costs per prevented case of macrosomia as the primary outcome measure, and a cost-benefit-analysis based on modeling, where also the economic consequences of (reduction of) complications and care-use due to macrosomia will be evaluated on the long term. TIME SCHEDULE: Total study time 24 months. As this study was first submitted to the program medical innovation, we were limited to a budget of €250,000 and 24 months, which is tight, as we have to extend the study with the economic modeling. Discussion with the ZonMW desk revealed that we were advised to stick to current budget and time phrame. SAMENVATTING DOEL: Hyperglycemie tijdens de zwangerschap is geassocieerd met een slechte perinatale uitkomst. Zelfs als zwangeren met diabetes worden gecontroleerd volgens de huidige richtlijnen, zijn de uitkomsten slechter vergeleken met normoglycemische vrouwen tijdens de zwangerschap. Het Continue Glucose Monitoring Systeem (CGMS) is een werkzame nieuwe methode om de bloedglucoseregulatie te verbeteren bij zwangeren met diabetes. In dit onderzoeksvoorstel willen wij de kosten-effectiviteit nagaan van het gebruik van CGMS tijdens type 1 en type 2 diabetische zwangerschappen. STUDIE OPZET: Multicentrische open label gerandomiseerde gecontroleerde trial (RCT), met aanvullend een beslis en kosten-effectiviteits studie. STUDIEPOPULATIE: Zwangere vrouwen met type 1 of type 2 diabetes. INTERVENTIE: Deelnemende vrouwen zullen willekeurig worden toegewezen tot of additioneel gebruik van CGMS of standaard zorg. Alle vrouwen zullen hun bloedglucose-regulatie controleren aan de hand van zelfcontrole (dagcurves) en HbA1c-waarden. Daarbij zullen de vrouwen in de CGMS groep aanvullend maandelijks de CGMS gaan gebruiken. Hun insulineschema zal worden aangepast op basis van de CGMS-profielen. UITKOMSTMATEN VAN DE RCT: Het primaire eindpunt is macrosomie (i.e. geboortegewicht boven de 90ste percentiel). Secundaire eindpunten zijn geboortegewicht, samengestelde neonatale morbiditeit, maternale morbiditeit en kosten. De analyses zullen worden uitgevoerd volgens het intention-to-treat principe. SAMPLE SIZE BEREKENING: Wij verwachten dat een reductie van macrosomie van 45% naar 30% in de interventiegroep zal opwegen tegen de additionele kosten van het gebruik van CGMS. Onder deze voorwaarden moeten 300 vrouwen worden gerandomiseerd (twee groepen van 150). ECONOMISCHE EVALUATIE: Omdat wij een reductie van macrosomie maar een groter gebruik van hulpmiddelen verwachten in de interventiegroep, zal de economische analyse bestaan uit een kosten-effectiviteits-analyse met kosten per voorkomen geval van macrosomie als uitkomstmaat, en een kosten-baten analyse op basis van modellering, waarin tevens de economische consequenties van(reductie van) complicaties en zorggebruik tgv macrosomie op langere termijn zullen worden geëvalueerd. TIJDPAD: Totale studieduur 24 maanden. Omdat deze studie in eerste instantie was ingediend voor het programma medische innovatie, waren we beperkt tot een budget van 250.000 euro en 24 maanden, hetgeen krap is, aangezien we de studie hebben uitbreid met economische modellering. In overleg met ZonMw werd geadviseerd om het oorspronkelijke budget en tijdpad aan te houden.
