Cost effectiveness of treatment strategies in couples with unexplained subfertility

Koppels met onverklaarde infertiliteit worden meestal, na een afwachtend beleid van 6 tot 12 maanden, behandeld met intra-uteriene inseminatie (IUI) en in vitro fertilisatie (IVF). Wij hebben onderzocht wanneer het kosteneffectief is om met IUI en met IVF te beginnen. Zowel IUI in een gestimuleerde cyclus als IVF leiden tot meer levendgeborenen dan een afwachtend beleid. Wij vonden dat IUI met gonadotrofinen leidt tot meer levendgeborenen, maar ook tot meer meerlingen dan wanneer CC wordt gebruikt. Het verschil in levendgeborenen en meerlingen tussen gonadotrofines en CC viel weg bij gebruik van een lage startdosering en strikte cancelcriteria Bij gebruik van strikte cancelcriteria was IUI met CC goedkoper dan IUI met FSH. Wanneer paren bereid zijn 2 jaar een afwachtend beleid te volgen is het voor de maatschappij waarschijnlijk goedkoper om direct met IVF te beginnen. Patiënten zelf hebben duidelijke voorkeuren voor hun behandelingen die hier goed bij aansluiten.

INTRODUCTIE Paren met onverklaarde subfertiliteit worden meestal, na een afwachtend beleid van 6 tot 12 maanden, behandeld met achtereenvolgens IUI en IVF. DOEL/VRAAGSTELLING Wanneer is het kosten-effectief om met IUI en met IVF te beginnen? HYPOTHESE IVF is duurder dan een afwachtend beleid of IUI maar wel effectiever. Wij nemen aan dat, afhankelijk van onder meer de leeftijd van de vrouw, IVF kosten-effectief zal zijn na een afwachtperiode van 1 tot 3 jaar gevolgd door minimaal 6 cycli IUI. STUDIE OPZET Economische besliskundige analyse op basis van bekende effectiviteit met behulp van netwerk en individuele patiëntengegevens metanalyse van bestaande gerandomiseerde studies. STUDIEPOPULATIE/DATABRONNEN Paren met onverklaarde subfertiliteit INTERVENTIE Afwachtend beleid, intra-uterine inseminatie met en zonder ovariële stimulatie en IVF GEBRUIKELIJKE ZORG /VERGELIJKING Er worden verschillende behandelingsscenario's vergeleken. UITKOMSTMATEN Levend geboren kind, doorgaande zwangerschap, meerling, cancelen cyclus, gebruikte volumina, kosten SAMPLE SIZE BEREKENING/DATA-ANALYSE Er zijn op dit moment 20 trials met in totaal 3232 paren die gedurende 3 tot 12 maanden werden behandeld met afwachten, IUI en/of IVF. Deze populatie heeft voldoende power om ook te onderzoeken wat het effect van bijvoorbeeld leeftijd van de vrouw is op de kosteneffectiviteit. We gebruiken de data van een netwerk meta-analyse om de effectiviteit van de behandelingen vast te leggen en de IPD om het effect van confounders en effect modificatoren in beeld te brengen en de volumina voor het bepalen van de kosten vast te leggen. KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE De kosteneffectiviteitsanalyse zal voor de verschillende behandelingsstrategieën de incrementele kosten relateren aan de incrementele gezondheidseffecten binnen een tijdshorizon van 36 maanden. In de BIA zullen kosten en effectiviteit worden gecombineerd met nationale prevalentie en incidentie gegevens om de resultaten te extrapoleren naar een tijdshorizon van 5 jaar. TIJDPAD 0-2 maanden: afronden netwerk meta-analyse 0-6 maanden: verkrijgen databases en opslaan in safe haven. 6-12 maanden:analyse IPD gegevens. 10-14 maanden: in kaart brengen resource use in de betreffende landen waar de studies plaatsvonden. 14-16 maanden:in kaart brengen Nederlandse kosten. 16-20 maanden: de kosten-effectiviteit analyses. 20-24 maanden: opschrijven en afronden project. INTRODUCTION Couples with unexplained subfertility are, after an expectant policy of 6 to 12 months, usually treated with IUI followed by IVF. OBJECTIVE(S)/RESEARCH QUESTION(S) When do IUI and IVF become cost-effective? HYPOTHESIS IVF is more expensive than an expectant policy or IUI but it is more effective in terms of a live birth. We assume that, depending on the age of the woman, IVF will be more cost-effective after an expectant policy of 1 to 3 years followed by minimally 6 cycles of IUI. STUDY DESIGN Economic decision analyses on basis of known effectiveness as determined by network and individual patient data meta-analyses of existing randomised trials. STUDY POPULATION(S)/DATASETS Couples with unexplained subfertility INTERVENTION Expectant policy, intra-uterine insemination with and without ovarian stimulation and IVF USUAL CARE /COMPARISON We will compare different treatment scenarios. OUTCOME MEASURES Live birth, ongoing pregnancy, multiple pregnancy, cancellations, used resources, cost SAMPLE SIZE CALCULATION/DATA ANALYSIS There are at this moment 20 trials with a total of 3232 couples that fulfil the inclusion criteria. This population has adequate power to also evaluate the effect of female age on the cost-effectiveness of different treatment scenarios. We use the data of a network meta-analysis to study the effectiveness of the treatments and the IPD to determine the effect of confounders and effect modifiers and the resource use required to calculate the costs. COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS The cost-effectiveness analysis will, for the different treatment strategies, relate the incremental costs to the incremental health effects within a time horizon of 36 months. For the budget impact analysis, cost and effectiveness data will be combined with national prevalence and incidence data to extrapolate the findings to a time horizon of 5 years. TIME SCHEDULE Months 0-2: finalise the network meta-analysis Months 0-6: acquire databases and safe them in safe heaven. Months 6-12 merge and analyse the IPD databases. Months 10-14 map resource use in the trials countries of origin. Months 14-16 Determine resource costs in the Netherlands Months 16-20 Cost-effectiveness analysis Months 20-24 Writing up results, finalising project
