Pulsed Electromagnetic Fields in the treatment of fresh scaphoid fractures. A multi centre, prospective, double-blind, placebo controlled, randomized trial.

Het schippersbotje is het meest gebroken handwortelbeentje en is erg belangrijk voor de functie van het polsgewricht. Een breuk in het schippersbotje kan ernstige gevolgen hebben. Genezingsstoornissen met blijvende beperkingen komen voor in 40% van de gevallen.

Omdat de genezing van een breuk van het schippersbotje heel veel tijd kan kosten en een operatie niet altijd het gewenst resultaat oplevert willen wij onderzoeken of de behandeling met electromagnetische botgroeistimulatie (PEMF) de genezing kan versnellen. Ook de kosteneffectiviteit van deze behandeling wordt onderzocht.

In 7 Nederlandse ziekenhuizen worden 110 mensen willekeurig verdeeld over 2 groepen. De ene groep wordt met een werkende botgroeistimulator behandeld en de andere groep met een niet-werkende. Zowel patiënten alsook onderzoekers weten niet welke apparaten werken.

Alle patiënten worden volgens een vast schema gecontroleerd waarbij genezing wordt onderzocht middels scans van het schippersbotje. Verder worden bij alle patiënten functie-onderzoeken uitgevoerd en vragenlijsten afgenomen.

Alle patiënten worden tot 1 jaar na start van de behandeling gecontroleerd.

Eindverslag

Het gebruik van PEMF (pulsed electromagnetic fields) botgroeistimulatie wordt in de literatuur voorgesteld als alternatief voor de chirurgische behandeling van scaphoid nonunions maar het effect ervan is nooit eerder onderzocht bij acute fracturen.
In Nederland betreft 90% van alle carpale fracturen een fractuur van het scaphoid (schippersbotje). Het scaphoid speelt een essentiële rol in de beweeglijkheid van pols omdat het met vijf omringende handwortelbotjes een gewricht vormt. Gefaalde behandeling van fracturen van het scaphoid kan leiden tot avasculaire necrose, nonunion en versnelde artrose. Bovendien leidt vertraagde genezing van het scaphoid tot langere gipsimmobilisatie en uitgebreid functieverlies wat in de jonge, voornamelijk werkende patiëntpopulatie leidt tot een langere arbeidsongeschiktheidsduur met significante economische en sociale consequenties. Uit voorgaand onderzoek is bekend dat zelfs de ongecompliceerde behandeling van scaphoid fracturen kan leiden tot een gemiddelde arbeidsongeschiktheidsduur van 21 weken. De huidige bekende behandelstrategieën bieden hiervoor geen oplossing.

We hebben de hypothese geformuleerd dat het gebruik van PEMF botgroeistimulatie bij acute scaphoid fracturen de tijd tot volledige genezing met 30% kan versnellen. Dit hebben we onderzocht in een gerandomiseerde, dubbel-blinde, placebo-gecontroleerde multicentrische trial.
De volgende onderzoeksvragen werden hiertoe geformuleerd:

1. Kan het gebruik van PEMF botgroeistimulatie bij scaphoid fracturen de genezing versnellen zowel gekeken naar radiologische alsook klinische uitkomstmaten ?
2. Kan het gebruik van PEMF botgroeistimualtie bij scaphoid fracturen het aantal nonunions en gevallen van avasculaire necrose terugdringen en derhalve het aantal secundaire chirurgische interventies reduceren ?
3. Was is de invloed van PEMF botgroeistimulatie bij scaphoid fracturen gekeken naar functionele uitkomstmaten ?
4. Is het gebruik van PEMF botgroeistimulatie bij scaphoid fracturen kosteneffectief vanuit maatschappelijk perspectief bekeken als we het vergelijken met de als standaard beschouwde conservatieve behandeling ?

Tussen 1 januari 2010 en 31 december 2011 werden in 5 verschillende centra 102 geschikte patiënten met een eenzijdige, onverplaatste scaphoid fractuur geïncludeerd in de studie en werden ze willekeurig verdeeld over een groep die behandeling met een PEMF botgroeistimulator ontving en een groep die werd behandeld met een placebo. Zowel de behandel- alsmede de placebogroep bestond uit 51 patiënten. Exclusie criteria waren presentatie na 5 dagen na oplopen van de fractuur, zwangerschap, aanwezigheid van een IED (implanted electronic device), additionele fracturen van de hand(wortel) en de aanwezigheid van vooraf bestaande beperkingen in handfunctie.
Klinische, radiologische, functionele en sociaal-economische uitkomsten werden bepaald op 6, 9, 12, 24 en 52 weken na start van de behandeling. Radiologische uitkomsten werden bepaald middels multiplanar reconstruction (MPR) CT scans op 6, 12, 24 en evt. 52 weken. Tevens werden PRWHE, SF36 en Euroqol 5d vragenlijsten gebruikt om functionele uitkomstmaten te bepalen.

De tijd tot op de CT scan bevestigde volledige genezing van het scaphoid bleek korter in de interventie (PEMF) groep, echter de verschillen waren niet statistisch significant.
Er bleek geen significante invloed te bestaan op de arbeidsongeschiktheidsduur en het sociale functioneren van de proefpersonen door toevoeging van PEMF botgroeistimulatie.
Daarnaast bleek PEMF in acute scaphoid fracturen het aantal nonunions niet terug te kunnen dringen. Met uitzondering van een toegenomen knijpkracht bij fracturen aan de dominante hand bleek de PEMF behandeling niet van invloed te zijn op functionele uitkomstmaten.
Verder bleek uit een kosteneffectiviteitsanalyse waarbij de effectiviteit van PEMF ten opzichte van een placebo behandeling in QALY’s (quality adjusted life years) wordt uitgedrukt dat PEMF een inferieure behandel

Van alle handwortelfracturen is de scaphoid fractuur de meest voorkomende. In Nederland is 90 % van alle handwortelbreuken een scaphoid fractuur. Het scaphoid vervult een sleutelrol bij de beweeglijkheid van de pols en stoornissen in genezing kunnen leiden tot zeer slechte functionaliteit van hand en pols. Ondanks het feit dat de exacte incidentie van scaphoid fracturen in Nederland onbekend is en diagnostiek naar deze fractuur vaak lastig is worden er naar schatting 24.000 per jaar behandeld met het vermoeden op een fractuur. Waarschijnlijk bestaat 15 tot 20 % van deze groep uit ware fracturen.
De scaphoidfractuur is lastig te behandelen en falen kan leiden tot avasculaire necrose (tot 40%), non-union (5 tot 21 %) en juveniele arthrose (tot 32%) waardoor polsfunctie ernstig geïnvalideerd kan raken. Verder leidt vertraagde genezing van de scaphoidfractuur tot langere gipsimmobilisatie met als gevolg langer ziekteverzuim en significante financiële en psychosociale consequenties. Studies tonen aan dat zelfs na ongecompliceerde genezing het arbeidsproces gemiddeld 155 dagen is onderbroken.
Huidige behandelstrategieën zijn niet in staat om met deze problemen af te rekenen omdat het voorspelde functionele eindresultaat na genezing, non-union en andere complicaties zoals avasculaire necrose onduidelijk is. Zelfs na operatieve behandeling zijn de resultaten variabel.

Wij willen derhalve onderzoeken of botgroei stimulatie door middel van pulsed electromagnetic fields (PEMF) bij verse scaphoid fracturen de genezing kan versnellen en de incidentie van invaliderende complicaties zoals non-union en avasculaire necrose kan terugdringen. Tevens willen we onderzoeken wat de invloed is van deze behandeling op de duur van het ziekteverzuim en het hervatten van de voorgaande werkzaamheden.
Het betreft een multicentrisch, prospectief, dubbelblind, placebo gecontroleerde gerandomiseerde trial. Studie populatie bestaat uit alle patiënten met een unilaterale, verse scaphoid fractuur. Zwangere vrouwen en personen met een pacemaker of andersoortig IED worden van inclusie uitgesloten alsmede patiënten met additionele fracturen van pols, handwortel of middenhandsbeentjes en patiënten met een tevoren bestaande beperking in polsfunctie.
De scaphoidfractuur wordt gediagnostiseerd door een combinatie van lichamelijk en aanvullend radiodiagnostisch onderzoek.
Alle patiënten met een bewezen scaphoidfractuur worden behandeld met gipsimmobilisatie en een klein PEMF apparaatje dat op het gips wordt gemonteerd. De helft van alle PEMF apparaatjes wordt willekeurig onklaar gemaakt in de fabriek. Zowel de patiënten alsmede de onderzoekers zijn onwetend betreffende de werkzaamheid van de apparaten.
Follow-up zal plaatsvinden na 6, 9, 12, 24 en 52 weken na diagnose. Deze follow-up bestaat uit lichamelijk onderzoek, radiodiagnostisch onderzoek (CT scan) en analyse door middel van 4 verschillende vragenlijsten. (PRWE, SF-36, Euroqol 5d en PRODISQ-vragenlijst)
Parameters die bestudeerd worden zijn: klinische genezing, radiologische genezing door middel van CT-scans, functie van de hand en dagelijks gebruik (o.a. d.m.v. de PRWE) alsmede kwaliteit van leven (SF-36 en Euroqol 5d vragenlijsten). Tevens zal er een economische analyse worden verricht. Deze economische analyse zal bestaan uit een kosten-effectiviteits analyse alsmede een kosten-utiliteits analyse vanuit sociaal perspectief. Het totaal aantal verloren werkdagen zal gemeten worden door gebruikmaking van PRODISQ (Productivity and Disease Questionnaire) modules bij inclusie in de studie, na 12, 24 en 52 weken.

Samenvatting van de aanvraag

The scaphoid bone is the most commonly fractured of the carpal bones. In the Netherlands 90% of all carpal fractures is a fracture of the scaphoid bone. The scaphoid has an essential role in functionality of the wrist, acting as a pivot and complications in healing can result in poor functional outcome. Although the exact incidence of scaphoid fractures is unknown, 24.000 carpal fractures are suspected and subsequently treated in the Netherlands annually. Probably 15 to 20% of this group consists of actual fractures. The scaphoid fracture is a troublesome fracture and failure of treatment can result in avascular necrosis (up to 40%), non-union (5-21%) and early osteo-arthritis (up to 32%) which may seriously impair wrist function. In addition impaired consolidation of scaphoid fractures results in longer immobilization and more days lost at work with significant psychosocial and financial consequences. Studies showed that even uncomplicated healing leads to a mean employment interruption of 155 days. Furthermore patient population mainly consist of young, active men and women aged 15 to 40 and therefore economic consequences of temporary or even permanent disability are substantial. Current treatment strategies are unable to deal with this problem since exact outcome after scaphoid union, non-union and other complications such as avascular necrosis remains unclear. Even after operative treatment results are variable. Furthermore operative treatment for complicated healing of scaphoid fractures (eg delayed or non-union) is often initiated in a late fase, most often months after the fracture occured. Initially PEMF was used in the treatment of tibial pseudoarthrosis and non-union. More recently there is evidence that physical forces can also be used in the treatment of fresh fractures, showing accelerated healing by 30 % and 71 % reduction in nonunion within 12 weeks after initiation of therapy. Nevertheless, until now no double blind randomized, placebo controlled trial has been conducted to investigate the effect of PEMF on the healing of fresh fractures of the scaphoid and investigate the effects of this treatment on consolidation and prevention or reduction of complications of healing, such as non-union. We therefore want to investigate whether the use of PEMF in fresh scaphoid fractures accelerates consolidation of this fracture and whether the use of PEMF reduces the incidence of disabling wrist conditions like scaphoid non-union or osteonecrosis and the consequences of this treatment on time off work and resuming of former professional activities. This is a multi centre, prospective, double-blind, placebo controlled, randomized trial. Study population consists of all patients with an unilateral fresh scaphoid fracture. Pregnant women and patients having a life-supporting implanted electronic device are excluded as well as patients with additional fractures of wrist, carpal or metacarpal bones and pre-existing impairment in wrist function. The scaphoid fracture is diagnosed by a combination of physical and radiographic examination. (CT-scanning) All patients having a proven scaphoid fracture are treated with cast immobilisation and a small PEMF device placed on the cast. Half of the PEMF devices will be disabled at random in the factory. The patients as well as the investigators will be unaware of the device's functionality. Study parameters are clinical consolidation, radiological consolidation by means of CT scanning, functional status of the wrist, including the patient rated wrist evaluation (PRWE) and quality of life, measured by SF-36 health survey questionnaire. Primary endpoint is number of scaphoid unions at six weeks, secondary endpoints are time interval to clinical and radiological consolidation, number of non-unions, functional status at 52 weeks and non-adherence to the treatment protocol. The estimated incidence of scaphoid fractures is about 35 per year in a hospital with an adherence of 250.000 patients a year. Studies performed using physical forces on fresh fractures show acceleration of the healing process of about 30 %. Based on a power analysis a study group of 100 to 110 patients is needed for inclusion. (alpha 0.05, beta 0.20, 81 % vs 55 % consolidation after 6 weeks, including 15 % dropout) Since 4 hospitals participate in this study with an adherence of 1.5 milion patients, inclusion will take less than 1 year. Since follow up is one year, the study can be completed within 2 years. If the results of this treatment are as we suspect based on former studies using physical forces to accelerate bone healing, a financial benefit is also created because of faster and enhanced recovery of troublesome scaphoid fractures in a young and productive patient population. This financial benefit is greater than the costst involved in this treatment, based on the number of patients (earlier) resuming their former professional activities.
