Samenvatting van de aanvraag

Background - Test evaluation is dominated by the diagnostic accuracy paradigm, where tests are evaluated by comparing their results with those of a reference standard. Yet many practical diagnostic problems involve a comparison of two or even more tests. In recent years, several results have become available for the comparison of two tests in a paired design. For problems with more than two tests, there are no valid methods available to identify optimal strategies based on effectiveness and costs. We want to bridge the gap between paired designs for two tests and multivariable modelling. Objectives - To develop, extend and evaluate methods for building optimal diagnostic algorithms with multiple tests, where not all patients have to undergo all tests, and accuracy is optimized within cost constraints. Study design - Data-analysis of large, existing datasets from multiple test evaluation with fully paired designs Intervention - An empirical comparison of existing techniques and algorithms and newly developed variations for multiple test comparisons Data-analysis - Using the results of an exhaustive comparison of all possible strategies in a decision analysis as the reference standard, we will compare the validity and efficiency of techniques and algorithms based on information-to-cost ratio and related measures, decision-trees, multivariable logistic regression, and other classification and regression trees. Outcome measures - A comparison of validity (identification of members of set of optimal solutions) and efficiency Power/data analysis - The existing datasets hold from 300 to over 1000 patients and will allow the comparison of strategies based on 3 to 10 tests and patient characteristics. Time schedule - 24 months. The project will run during the first two years of the OPTIMA project. ---------------------------------------------------- Achtergrond - Het evaluatieonderzoek van diagnostische tests wordt gedomineerd door het accuratesse paradigma, waarbij de vergelijking van de resultaten van één enkele test met die van de referentiestandaard centraal staat. Recent is de methodologie verder ontwikkeld voor het vergelijken van de accuratesse van twee tests. Het ontbreekt ons echter aan valide methoden voor het vinden van een optimaal diagnostische algoritme voor meer dan twee tests, waarbij niet alle patiënten alle tests hoeven te ondergaan. Doel - Het verfijnen en evalueren van methoden voor het ontwikkelen van diagnostische algoritmes, waarbij naar de beste accuratesse tegen aanvaardbare kosten wordt gezocht en niet alle patiënten alle tests hoeven te ondergaan. Opzet - Methodologische ontwikkeling; empirische vergelijking van methoden in bestaande datasets. Interventie - Bestaande heuristieken en methoden, aangevuld met verdere verfijningen Data-analyse - De resultaten van een uitputtende vergelijking van alle mogelijke diagnostische strategieën, uitgevoerd in een beslissingsanalyse, wordt als het criterium gebruikt. Daarna vergelijken we de resultaten van technieken en algoritmes, waaronder de informatie-kost verhouding, multivariable logistic regression, en classification and regression trees. Uitkomstmaat - Validiteit (identificeren van optimale oplossingen) en efficiëntie Power/data analyse - De bestaande datasets (o.a. FIT, ANTELOPE) bevatten gegevens over 300 tot meer dan 1000 patiënten, zodat van 3 tot 10 tests en patiëntenkenmerken kunnen worden vergeleken. Tijdsplanning - Het project wordt tijdens de eerste twee jaar (24 maanden) van het OPTIMA project uitgevoerd, zodat de bevindingen in dat project kunnen worden ingezet.
