Exercise therapy for patellofemoral pain syndrome: a randomised clinical trial in general practice and sports medicine

Sommige mensen krijgen, waarschijnlijk door overbelasting, aanhoudende pijn aan de onderkant van hun knieschijf (patellofemoraal pijnsyndroom). Rustig aan doen, aangevuld met lichte oefeningen en zo nodig een pijnstiller is de gebruikelijke behandeling van deze aandoening. In Rotterdam is onderzocht of gerichte oefeningen onder de hoede van een fysiotherapeut tot een sneller herstel leidt. Dit blijkt (gemiddeld) zo te zijn. Mensen die de gerichte oefeningen deden hadden na 3 en 12 maanden gemiddeld minder pijn en konden beter allerlei activiteiten uitvoeren dan mensen die het rustig aan hadden gedaan. De extra kosten van de oefensessie bij de fysiotherapeut waren geringer dan de extra uitgaven die de andere patiënten hadden aan pijnstillers, bandages etc. en het verlies aan arbeidsproductiviteit (werkverzuim). In totaal was de behandeling via de fysiotherapeut € 155,-- goedkoper.

Samenvatting van de aanvraag
The clinical guideline of Dutch general practitioners suggests exercise for the management of patellofemoral pain syndrome (PFPS), though it also states that evicence is insufficient regarding its effectiveness. Our recent Cochrane review showed limited evidence that exercise was beneficial, though the quality of the trials was such that further research was recommended to confirm or to disprove this conclusion. In this study we will evaluate the (cost)effectiveness of exercise therapy and information/advice on PFPS in adolescents and young adults compared to nformation/advice on PFPS only in a randomised clinical trial. Study population: Patients in the age 14-35 years consulting for PFPS in general practices and sports clinics. Interventions: Patients will be randomised into two groups, stratified for clinical setting and age. The exercise group will receive advice and information on the background of PFPS of the physician and undergo a standardised exercise program, supervised by physiotherapists (9 session during 6 weeks), with contuniation by home exercises. The control group will only receive advice and information on the background of PFPS by the physician, similar to the advice given by general practitioners and sports physicians in a normal care situation. Primary outcome measures: Pain levels, knee function, and subjective perception of recovery after three months and one year. Outcome measurements will occur at baseline, after 6 weeks, and three-monthly (after 3 and 12 months also by telephone). Power and (cost)analysis: 61 patients per group are needed to statistically show (beta 0.8, alpha 0.05) an increase from 19% recovery in the usual care group to 41% recovery in the exercise group after one year. This estimate of difference between the groups is based on the Cochrane review. Counting with a dropout rate of 10% we include 136 patients in the study. Differences in outcome between the groups will be analysed on the basis of intention to treat as well as per protocol, if necessary adjusted for baseline differences and co-interventions. For the economic evaluation a cost-utility analysis will be performed that expresses health improvements in Quality Adjusted Life Years (QALYs). The relevant costs to be included are the direct medical costs and productivity costs. Time schedule: Month 1-4 to prepare the trial; month 5-17 to include and follow-up the patients and to administer the intervention, month 18-29 to follow-up the patients and to analyse the 3 month results, month 30-36 to analyse the one year results (effect and costs). In de richtlijn voor huisartsen wordt aangeven dat bij patellofemorale knieklachten (PFPS) spierversterkende oefeningen mogelijk effectief kunnen zijn maar dat hier nog geen bewijs voor is. Onze recente systematische review liet "limited evidence" zien voor de effectiviteit van oefentherapie bij PFPS; een studie van voldoende omvang en kwaliteit zou deze conclusie moeten bevestigen dan wel weerleggen. In deze studie zal de (kosten)effectiviteit van oefentherapie in combinatie met het geven van adviezen en uitleg door de arts in vergelijking tot alleen het geven van uitleg en adviezen door de arts bij patiënten met PFPS geëvalueerd worden in een gerandomiseerde klinische trial. Studiepopulatie: Patiënten die de huisarts of de sportarts consulteren voor PFPS in de leeftijd van 14-35 jaar. Interventies: Patiënten worden gerandomiseerd in twee groepen gestratificeerd voor setting en leeftijd. De oefentherapiegroep krijgt uitleg over de aandoening en advies van de arts in combinatie met een gestandaardiseerd oefenprogramma onder begeleiding van de fysiotherapeut (9 sessies gedurende 6 weken) met continuuering d.m.v. een thuisoefenprogramma. De controle groep krijgt alleen uitleg en adviezen van de arts zoals dat nu in de dagelijkse praktijk gebruikelijk is. Primaire uitkomstmaten: Pijnniveau, kniefunctie en mate van ervaren herstel na drie maanden en na één jaar. Deze uitkomstmaten worden na 6 weken en driemaandelijks gemeten (na drie en na 12 maanden ook telefonisch). Power en (kosten)effectiviteitsanalyses: Om een statistisch verschil in herstel aan te tonen na één jaar (bèta 0.8 en alpha 0.05) van 19% in de controle groep ten opzichte van 41% in de oefentherapiegroep zijn 61 patiënten per groep nodig. Rekening houdend met 10% uitval worden 136 patiënten geïncludeerd. De resultaten zullen zowel op basis van intention to treat als op basis van per protocol geanalyseerd worden, wanneer nodig geadjusteerd voor baselineverschillen en verschillen in co-interventies tussen de beide groepen. Voor de economische evaluatie zal een kosten-utiliteitsanalyse worden uitgevoerd waarbij gezondheidsverbetering wordt uitgedrukt in Quality Adjusted Life Years (QALYs). Relevante kosten in deze analyse zijn de directe medische kosten en de productiviteitskosten. Tijdsplan: maand 1-4 de voorbereidingen; maand 5-17 de inclusie, follow-up en interventies; maand 18-29 de follow-up en analyse van 3 maands resultaten, maand 30-36 de lange-termijn analyse (effecten en kosten).
