Enhancing full recovery in patients with bipolar disorder

Een bipolaire stoornis kenmerkt zich door terugkerende stemmingsepisoden waarbij zich zowel (hypo)manieën, depressies als gemengde episoden kunnen voordoen. Na het kortetermijnherstel richt een behandeling zich onder andere op het goed monitoren van symptomen zodat nieuwe depressies of manische episoden worden voorkomen. Daarnaast is ook het persoonlijke herstel van belang. Hoe kom ik tot een plezierig, betekenisvol en betrokken leven, ook in aanwezigheid van deze psychische kwetsbaarheid?

In dit project wordt onderzoek gedaan naar een nieuwe positief psychologische interventie voor mensen met een bipolaire stoornis. Positief psychologische interventies richten zich op positieve gedachten, emoties en gedrag via bewezen effectieve strategieën zoals dankbaarheid, genieten, optimisme en iets doen voor anderen. Draagt het toevoegen van deze nieuwe interventie aan de gebruikelijke zorg bij aan de duurzame mentale gezondheid van mensen met een bipolaire stoornis? Dat is de vraag die hopelijk wordt beantwoord in deze studie.

Eindverslag

In dit driejarige project doen we onderzoek naar een nieuwe positief psychologische interventie voor mensen met een bipolaire stoornis. Het onderzoek wordt uitgevoerd in verschillende centra verspreid over Nederland: Dimence in Deventer, GGZ inGeest in Amsterdam en Hoofddorp, Mediant Enschede en GGNet Doetinchem. In het eerste jaar van het onderzoek (1-12-2017 t/m 30-11-2018) waren we bezig om ethische toestemming voor het onderzoek te krijgen en de interventie te ontwikkelen. In augustus zijn we begonnen met de inclusie. Daarnaast werden begin september 2018 de behandelaars getraind die de interventie gaan geven. In november dit jaar is de eerste behandelgroep van start gegaan bij Dimence in Deventer en we verwachten dat rond januari 2019 meerdere groepen op andere locaties kunnen starten.

Samenvatting van de aanvraag

­DOEL/VRAAGSTELLING Is een welbevinden-interventie (‘Dit is jouw leven’) aangeboden aan patiënten met een bipolaire stoornis (BS) in remissie in aanvulling op de gebruikelijke zorg (GZ), (kosten)effectiever dan alleen GZ? ­HYPOTHESE In vergelijking met GZ is de interventie in aanvulling op GZ (kosten)effectiever in termen welbevinden, depressieve symptomen, en terugval in acute episoden. ­STUDIE OPZET Deze studie is een pragmatische gerandomiseerde gecontroleerde studie. Metingen vinden plaats op baseline, post-interventie, en follow-up momenten 6 en 12 maanden na baseline. ­STUDIEPOPULATIE/DATABRONNEN Patiënten met een BS I of II die zich bevinden in remissie. Inclusiecriteria zijn 1)4 = ondersteunende sessies in laatste jaar, en 2) residuele depressiesymptomen. Exclusiecriteria zijn 3) een acute episode, 4) verslavingsproblematiek en/of 5) hoge niveaus van welbevinden. ­INTERVENTIE Dit is jouw leven is gericht op persoonlijk herstel en focust o.a. op sterke kanten, veerkracht, post-traumatische groei en positieve relaties. De interventie bestaat uit 8 bijeenkomsten van 2 uur. Evaluaties van BS-patiënten die nu een pilotstudie volgen worden gebruikt ter aanpassing. ­GEBRUIKELIJKE ZORG /VERGELIJKING GZ voor BS bestaat uit (2-12) ondersteunende sessies en farmacologische onderhouds-behandeling, en soms aanvullende psychotherapie. ­UITKOMSTMATEN Welbevinden als de primaire uitkomst wordt gemeten met de Mental Health Continuum – Short Form (MHC-SF). Secundaire uitkomsten zijn depressiesymptomen(Quick - Inventory of Depressive Symptomatology), manische symptomen (Altman Self-Rating Mania Scale), en psychopathologie (Brief Symtom Inventory). Kosteneffectiviteitsanalyses worden gedaan met de EQ-5D-5L en de Trimbos and iMTA questionnaire on Costs associated with Psychiatric illness. ­SAMPLE SIZE BEREKENING/DATA-ANALYSE Er zijn in totaal 112 deelnemers nodig. Analyses worden gedaan op basis van het intention to treat (ITT) principe met generalized estimating equations (GEE). ­KOSTENEFFECTIVITEITSANALYSE/ BUDGET IMPACT ANALYSE De interventie zal QALY-winst in de populatie opleveren tegen matige additionele kosten, namelijk €6,000 per gewonnen QALY. De Budget Impact Analyse (BIA) wordt uitgevoerd vanuit verschillende perspectieven. ­TIJDPAD Het project duurt 36 maanden. In maanden 1-10 bereiden we de studie voor. In maanden 10-28 voeren we de interventie uit. In maanden 28-36 verzamelen we lange-termijn data en analyseren data. ENGELS ­OBJECTIVE(S)/RESEARCH QUESTION(S) Is a well-being intervention (‘This is your life’) offered to bipolar disorder (BD) patients in remission in addition to usual care (CAU), more (cost-)effective than CAU only? ­HYPOTHESIS In comparison to CAU only, the intervention in addition to CAU will result in significantly greater benefits in terms of well-being, depressive symptoms, and relapse into acute episodes. ­STUDY DESIGN This study concerns a pragmatic randomized multicenter trial. Measurements take place at baseline, post-intervention and follow-up 6 and 12 months from baseline. ­STUDY POPULATION(S)/DATASETS Patients with Bipolar I or II in the euthymic phase. Inclusion criteria are 1) 4 = supportive sessions in the last year, and 2) residual depressive symptoms. Patients are excluded when 3) they suffer from an acute episode or 4) addiction problems, and/or 5) have optimal levels of well-being. ­INTERVENTION This is your life targets personal recovery and focuses on six components, including personal strengths, resilience, post-traumatic growth, and positive relationships. The intervention consists of 8 meetings of 2 hours. Evaluations of BD patients whom currently participate in a pilot study inform adaptation of the intervention. ­USUAL CARE /COMPARISON CAU for BD in euthymic phases comprises of 2-12 supportive sessions and maintenance pharmacological treatment, and may include psychotherapeutic treatments. ­OUTCOME MEASURES Well-being (primary outcome) is measured with the Mental Health Continuum – Short Form (MHC-SF). Secondary outcomes include depressive symptoms (Quick - Inventory of Depressive Symptomatology), manic symptoms (Altman Self-Rating Mania Scale), and psychopathology (Brief Symtom Inventory). Cost-effectiveness analysis are performed using the EQ-5D-5L and the TiC-P. ­SAMPLE SIZE CALCULATION/DATA ANALYSIS A total of 112 patients is needed. Analyses will be done on both intention to treat (ITT) and per protocol basis using generalized estimating equations (GEEs). ­COST-EFFECTIVENESS ANALYSIS/ BUDGET IMPACT ANALYSIS The intervention is anticipated to generating QALY-gains at a modest additional cost, i.e. €6,000 per QALY gained. The Budget Impact Analysis (BIA) assesses how budgets will change when offering the intervention over a range of implementation levels. ­TIME SCHEDULE The project takes 36 months. In months 1-10 we prepare the trial. In months 10-28 we execute the trial. In months 28-36, we gather follow-up measurements and analyze the data.
