Syncope algorithms in the emergency department with structured follow-up. How effective is a standardised approach at improving diagnostic yield, quality of life and decreasing health care costs? The SYNERGY study

Syncope is wereldwijd goed voor 1% van alle bezoeken op de Spoedeisende Hulp. De risico’s lopen uiteen van onschuldig tot dodelijk. Vaak vindt men geen oorzaak, vooral door het kernprobleem dat specialisten zich richten op 'uitsluiten': de cardioloog probeert een ritmestoornis uit te sluiten en de neuroloog epilepsie. Buitenlandse onderzoeken toonden aan dat ‘Syncope Algoritmen’ die zich richten op 'insluiten' (wat heeft de patiënt wél?) betere én goedkopere zorg opleveren. In het Nederlandse zorgstelsel wordt deze aanpak nog niet toegepast. In dit project zullen wij de Syncope Algoritmen implementeren op de Spoedeisende Hulp van 4 ziekenhuizen. We meten voor en na de interventie. We letten hierbij op het aantal patiënten met een diagnose, de zorgkosten en de kwaliteit van leven. We verwachten dat het aantal patiënten met een diagnose toeneemt, de kwaliteit van leven verbeterd door een snellere behandeling en de kosten dalen via minder opnamen en minder nutteloze testen.

Syncope is wereldwijd goed voor 1% van de SEH-bezoeken. De risico’s lopen uiteen van onschuldig tot dodelijk. Vaak vindt men geen oorzaak, vooral veroorzaakt door het kernprobleem dat specialisten zich richten op 'uitsluiten': de cardioloog probeert een ritmestoornis uit te sluiten en de neuroloog epilepsie. Buitenlandse onderzoeken toonden aan dat Syncope Algoritmen die zich richten op 'insluiten' (wat heeft de patiënt wèl?) betere èn goedkopere zorg opleveren: het aantal diagnosen neemt toe en de kosten dalen via minder opnamen en minder nutteloze testen. In het Nederlandse zorgstelsel wordt deze aanpak op de Spoedeisende Hulp (SEH)nog niet toegepast. In dit onderzoek zullen we in 5 Nederlandse ziekenhuizen de nieuwe werkwijze implementeren: er zijn andere criteria voor de spoedopvang en bepaalde patiënten worden nu doorverwezen worden naar de nieuw opgerichte syncope poliklinieken.
We verwachten dat patiënten met deze aanpak vaker en eerder een juiste diagnose krijgen. Het is ook te verwachten dat er hierdoor minder zorgkosten gemaakt worden en dat de kwaliteit van leven stijgt.

Samenvatting van de aanvraag

Buitenlandse onderzoeken toonden aan dat Syncope Algoritmen (SA) die zich richten op 'insluiten' (wat heeft de patiënt wèl?) betere èn goedkopere zorg opleveren: het aantal diagnosen neemt toe en de kosten dalen via minder opnamen en minder nutteloze testen. In het Nederlandse zorgstelsel worden SA nog niet op de Spoedeisende Hulp (SEH) toegepast. VRAAGSTELLING In welke mate kunnen SA de diagnostiek op de SEH in Nederland verbeteren en de kosten verminderen? HYPOTHESE Met de toepassing van SA neemt het aantal patiënten met een diagnose toe en de zorgkosten af. De kwaliteit van leven verbetert door snellere behandeling. De beste diagnostische informatie wordt verkregen uit een gedetailleerd gesprek met patiënt en ooggetuige. Wij geloven dat de diagnostiek verbetert als de patiënt actiever bij dit proces betrokken wordt. STUDIE OPZET Multicenter trial met vergelijking voor en na de interventie STUDIEPOPULATIE Patiënten op de SEH met kennelijke syncope in 1 academisch ziekenhuis en 3 regionale ziekenhuizen INTERVENTIE De interventie bestaat uit meerdere componenten: - Triage op de SEH voor of opname, of verwijzing naar de huisarts, of een syncope polikliniek - Een innovatieve en meertalige e-health applicatie: de Health Communicator voor een actieve patiëntenparticipatie - Gestructureerde poliklinische analyse met speciale aandacht voor reflexsyncope GEBRUIKELIJKE ZORG/VERGELIJKING Syncope zorg zonder triage op SEH, e-health applicatie en gestructureerde poliklinische analyse. UITKOMSTMATEN Primair: aantal correcte diagnosen (goudstandaard: follow-up door expert-panel na 1 jaar) Secundair: opnamen i.v.m. syncope, maatschappelijke kosten door syncope in het jaar na SEH-presentatie, aantal onderzoeken en consultaties, tijd tot een diagnose na SEH bezoek, aantal patiënten met aanvalsrecidief en kwaliteit van leven SAMPLE SIZE/ DATA-ANALYSE Met een power van 0.8, en een alpha van 0.05, zijn er per arm 360 patiënten nodig: 52% van alle SEH-patiënten met syncope tijdens het onderzoek. De uitkomstmaten worden met een multilevel analyse met correctie voor clusters vergeleken. KOSTENEFFECTIVITEITSANALYSE (KEA) & BUDGET IMPACT ANALYSE (BIA): KEA op basis van de trial data (diagnostiek kosten per accurate diagnose), kostenutiliteitsanalyse op basis van de trial data (maatschappelijke kosten per QALY) en kostcalculator spreadsheet model (BIA). TIJDPAD Mnd 0-6: ontwerpen Health Communicator, Mnd 3–20 md inclusie, Mnd 15-32 follow up, Mnd 32-48 data analyse & implementatie OBJECTIVE/ RESEARCH QUESTIONS Syncope is very common and has a broad differential diagnosis. The frequent failure to identify benign or malignant causes results in high costs. Syncope Algorithms (SAs) abroad have shown to improve diagnostic yield and reduced costs. HYPOTHESIS Implementation of SAs in the Netherlands will increase diagnostic yield, avoid unnecessary admissions and improve quality of life (QoL) by offering timely diagnosis & treatment STUDY DESIGN Multicentre trial comparing the pre- and post-implementation period STUDY POPULATION Syncope patients in the emergency department (ED) of 1 university hospital & 3 regional hospitals INTERVENTION SA implementation has multiple components: - ED triage resulting in admission, outpatient or GP referral - Innovative multilingual communication system facilitating active patient participation to maximize the yield of history taking in syncope - Structured outpatient evaluation OUTCOME MEASURES Primary: proportion of patients with an accurate diagnosis (as determined by an expert panel at 1 year follow-up) Secondary: number of admissions because of syncope, time to diagnosis following ED presentation, syncope recurrence, Healthcare and societal costs within 1 year following ED presentation, number of syncope-related tests and consultations, QoL SAMPLE SIZE CALCULATION/ DATA ANALYSIS Based on a power of 0.8 and alpha of 0.05, we need to recruit 360 per group, i.e. 52% of all eligible syncope cases.
