In Amsterdam is een wiskundig model ontwikkeld om voorspellingen te kunnen als en bepaalde meting bij patiënten herhaaldelijk wordt uitgevoerd. De methodiek is vervolgens toegepast op het in de gaten houden van patiënten die waren opgenomen op de intensive care unit van het AMC (zie ook project Costs and effects of strategies to prevent oversedation in Intensive Care patients). Deze patiënten werden kunstmatig in een diepe slaap (sedatie) gehouden. Doel van de analyse was na te gaan of het continu uitvoeren van een zogeheten BIS-meting gebruikt kan worden om een te diepe sedatie te voorspellen. De BIS-waardes nabij tijdstippen waarop oversedaties werden gezien, waren significant lager dan BIS- waardes nabij tijdstippen waarop geen oversedaties werden gezien. Dit suggereert dat het BIS signaal gebruikt kan worden als voorspeller van momenten van oversedatie. Het signaal is echter gevoelig voor allerlei externe factoren en daarom soms matig van kwaliteit.

Background ? Increasingly, (live-)functions of patients are almost continuously monitored. Assessment of the prognostic value of such a time-dependent covariate for an unfavorable outcome is difficult, especially when outcome may recur, and when the number of repeated measurements of the covariate is large. In the period before actual occurrence of the events, the covariates may show departure from steady state. If this departure can be recognized early, treatment may be changed before occurrence of the oversedation event. Objectives ? To develop statistical models that can be used to predict recurrent events in patients, on the basis of continuous or almost continuous monitoring of a covariate, and developing tools for finding the optimal summary of the covariate-values, and optimal time-lag for predicting the recurrent events. Study design ? Mathematical-statistical model development and application thereof to data from BIS-monitoring of oversedation in ICU-patients (>18) expected to remain 24 hours at least in the ICU. Intervention ? Generalization of existing mathematical-statistical techniques and algorithms to handle very large number of repeated covariate measures, and recurrence of events. Outcome measures ? Prognostic accuracy of the statistical models for a specific summary-measure at a specific time-lag is evaluated by comparing the event-probabilities in patients with or without events. Power/Data-analysis ? Cox regression with time-dependent covariates will be extended with shared- and more complicated frailty parameters to account for correlation between recurrence. Alternatively, pattern-mixture and pattern-selection models will be used with a random-effects model for the repeated covariate measurements. These will also be extended with frailty parameters. Time schedule ? 24 months Achtergrond ? Steeds meer worden levensfuncties van patiënten (bijna) continue gemeten. Evaluatie van de prognostische waarde van zo?n tijdsafhankelijke covariabele voor ziekte-uitkomst is moeilijk, vooral als de ziekte terug kan komen en als het aantal herhaalde waarnemingen van de covariabele heel groot is. In de periode voorafgaand aan feitelijke uitkomst, kan de covariabele afwijking van normale variatie vertonen. Als die afwijking snel herkend wordt, kan behandeling snel ingezet worden en de uitkomst wellicht voorkomen. Doel ? Ontwikkeling van statistische modellen die heroptredende uitkomsten in patiënten kunnen voorspellen op basis van continu gemeten covariabelen, en de ontwikkeling van gereedschap om de optimale samenvatting van de covariabele-waarden en de optimale tijdsperiode voor het voorspellen van de events te bepalen. Studie opzet ? Ontwikkeling van wiskundig-statistisch modellen en toepassing ervan op data van BIS-monitoring van oversedatie in IC-patienten (>18j) die daar minimaal 24 uur zullen blijven. Interventie ? Generalisatie van bestaande statistische technieken zodat grote aantal herhaalde metingen en events geanalyseerd kunnen worden. Uitkomstmaat ? De prognostische accuratesse van de modellen voor specifieke samenvattingsmaten en voor tijdsperiodes wordt bekeken door event-kansen te vergelijken in patiënten die of geen events hebben doorgemaakt. Power/Data-analysis ? Cox regressie met tijdsafhankelijke covariabelen zal gegeneraliseerd worden met zogenaamde shared- en complexere frailty-parameters om rekening te houden met herhaalde uitkomsten.
