Improving the early diagnosis of myocardial infarction in primary care: the added value of a rapid assay to detect heart-type FABP, a novel marker of cardiac injury.

In dit onderzoek is onderzocht of een snelle diagnose van het acute myocardinfarct door huisartsen bij patiënten met klachten zoals pijn op de borst verbeterd kan worden door het toevoegen van een bedside test van het hart-type vetzuurbindend eiwit (H-FABP), een biomarker voor acute hartschade. Uit het onderzoek bleek dat de H-FABP sneltest gebruikt kan worden in de patiëntengroep met een lage verdenking op een hartinfarct. De test kan helaas niet gebruikt worden om een hartinfarct uit te sluiten, omdat de test in 16% van de gevallen een hartinfarct niet detecteert. Wel kan de test gebruikt worden als extra veiligheid om geen hartinfarct te missen: als de huisarts de patiënt niet naar het ziekenhuis wilde verwijzen, maar de test is positief, zou de patiënt tóch verwezen moeten worden.

Wij hebben een groot diagnostisch onderzoek verricht in de regio Utrecht naar een nieuwe bloedtest om hartinfarcten (‘acuut coronair syndroom’) vast stellen. De test meet een eiwit dat vrijkomt bij hartschade (heart-type fatty acid-binding protein (H-FABP), Cardiodetect® test). Met een vingerprik worden 4 druppels bloed verkregen en aangebracht op de testkaart. Binnen 15 minuten is de uitslag af te lezen. Huisartsen namen de test bij 298 patiënten verdacht van hartinfarct af. 22% had een acuut coronair syndroom. Bij een positieve test had 65% een acuut coronair syndroom, bij een negatieve test had 84% géén acuut coronair syndroom, ofwel specificiteit 94% en sensitiviteit 43%. De test zal altijd als aanvulling gebruikt worden op anamnese en lichamelijk onderzoek. Uit ons onderzoek blijkt dat de H-FABP sneltest gebruikt kan worden in de patiëntengroep met een lage verdenking op een hartinfarct. De test kan dan helaas niet gebruikt worden om een hartinfarct uit te sluiten, omdat de test in 16% van de gevallen een hartinfarct niet detecteert. Op dit moment, 1 jaar en 8 maanden na de start van inclusie van patiënten hebben wij 260 deelnemers (87%) ingesloten in ons onderzoek waarbij wij nagaan of een snelle diagnose van het acute myocardinfarct door huisartsen bij patiënten met klachten zoals pijn op de borst verbeterd kan worden door het toevoegen van een bedside test van het hart-type vetzuurbindend eiwit (H-FABP), een biomarker voor acute hartschade. Er worden 3-5 patiënten per week geincludeerd en we liggen op schema om de benodigde 300 patiënten te halen voor het einde van de subsidieperiode. Drie huisartsenposten (avond-, nacht-, en weekenddiensten) en inmiddels negen gezondheidscentra participeren. Daarnaast participeren drie ziekenhuizen in de regio met hun cardiologen, laboratorium- en eerstehulppersoneel. Er deden zich geen serieuze problemen voor bij het onderzoek. Deelnemers, huisartsen en de betrokken partijen in de ziekenhuizen zijn allen enthousiast. De database wordt dagelijks bijgehouden en volledig opgeschoond. Er zijn zowel nationale als internationale presentaties over de studie geweest in 2006 en 2007. Twee artikelen zijn ingediend, een over de pilot-studie en een over de opzet van de studie. Verder wordt er gewerkt aan een tweetal literatuurstudies. Een over patiënt- en huisartsdelay bij klachten van pijn op de borst verdacht voor een acuut coronair syndroom en een over verschillende soorten biomarkers bij het acute myocardinfarct.

Samenvatting van de aanvraag

Study objective: To assess whether the use of a rapid bedside blood test for heart-type fatty acid-binding protein (H-FABP; a novel biochemical marker of cardiac injury) decreases the diagnostic uncertainty in patients suspected of an acute coronary syndrome (i.e. acute myocardial infarction (AMI) or unstable angina) in primary care and to assess its impact on the balance between costs and effects of the management of these patients. Study design: Prospective diagnostic efficiency study. Study population: A total of 300 patients with symptoms suggestive of acute coronary syndrome (notably chest pain) that present themselves to one of the participating out-of-hours general practitioner (GP) services (huisartsenposten) or GP group practices. Excluded are patients who are too ill to undergo history taking / physical examination (and require instant referral according to the GP) and patients in whom the onset of symptoms occurred more than 24 hours ago. Diagnostic measurements: At the initial visit the following measurements will be performed: (1) capillary whole-blood for blinded assessment of the H-FABP test by the GP at the beginning of the encounter; (2) standardised clinical assessment (signs and symptoms) by the GP, summarised in a clinical score; after the clinical assessment (and thus according to usual care, since the result of the cardiac marker levels is unknown) the GP will decide about referral of the patient. After 12 to24 hours, and in order to allow for a definitive diagnosis of AMI, a venous blood sample to determine levels of troponin T and CK-MB, and a 12-lead ECG will be obtained, either at the patient's home or in the hospital. For this purpose, certified laboratory personnel is available around the clock. Outcome measure / reference standard: All diagnoses will be checked and confirmed at consensus meetings of an outcome panel (involving 2 cardiologists and 1 GP). They will use all available information except the rapid H-FABP test results, but including the biomarker levels and ECG findings obtained after 12-24 hours and, if available, information from hospital discharge letters. The presence of AMI (i.e. the outcome parameter) will be determined according to the ESC/ACC guidelines. Data analysis: Logistic regression/ROC analyses will be applied to quantify whether a diagnostic rule including or excluding the H-FABP test improves diagnostic accuracy. The areas under the curves (AUC) and observed and expected absolute numbers of AMI patients across probability cut-offs will be compared. Power calculation: Since our study includes 2 diagnostic determinants (clinical score and H-FABP test) 20 patients with an AMI are required. Because in around 10% of suspected patients in primary care AMI is confirmed, 200 patients with suspected AMI are required. We will include 300 patients to allow for subgroup analyses according to gender and age. Based on incidence figures of AMI, the participation of 3 out-of-hours GP services (each involving more than 100,000 inhabitants) and 10 group practices will provide more than enough participants during 18 months of inclusion. Economic evaluation: The balance between costs and effects will be assessed using a discrete event model with costs per additional 28-day survivor as the primary outcome. A limited number of strategies with and without H-FABP will be compared using the results from this study, paying specific attention to the time between the onset of symptoms and the start of treatment. Costs are limited to direct medical costs and the indirect non medical costs. To obtain costs per life year gained and costs per QALY, long term costs and effectiveness will be calculated using extrapolations based on the literature. Time schedule: 0-6 months: study preparation; 6-24 months: inclusion period; 24-30 months: data analysis / reporting.
