Cost effectiveness of trauma-focused treatments for childhood-related Posttraumatic Stress Disorder in adults

Patiënten met Posttraumatische Stress Stoornis (PTSS) ten gevolge van misbruik en/of geweld in hun jeugd hebben vaak ernstige psychische klachten. Exposure-behandeling is effectief voor PTSS, maar sommige hulpverleners denken dat mensen met trauma’s uit de jeugd te kwetsbaar zijn voor deze behandeling.

In de IMPACT-studie (Improving treatment for patients with childhood abuse related PTSD) hebben we bij 150 patiënten met PTSS ten gevolge van jeugdtrauma, exposure vergeleken met twee innovatieve behandelmethoden: een intensieve vorm van exposure en exposure voorafgegaan door emotieregulatie-training. Alle drie de behandelingen leidden tot grote verbeteringen van PTSS- en andere symptomen. In de intensieve conditie ging de verbetering sneller dan in de gefaseerde conditie. Na 1 jaar was er geen verschil in effecten. Er is dus geen reden om patiënten met jeugdtrauma’s exposure te onthouden. De behandelingen waren even effectief en veilig als bij patiënten met recentere trauma’s.

Patiënten met Posttraumatische Stress Stoornis (PTSS) ten gevolge van misbruik en/of geweld in hun jeugd hebben vaak ernstige psychische klachten. Exposure-behandeling is effectief voor PTSS, maar sommige hulpverleners denken dat mensen met trauma’s uit de jeugd te kwetsbaar zijn voor deze behandeling. Samenvatting van de aanvraag

DOEL/VRAAGSTELLING Zijn een gefaseerde behandeling en een geïntensiveerde traumagerichte behandeling doelmatiger dan de standaard behandeling bij patiënten met posttraumatische stress stoornis ten gevolge van vroegkinderlijk trauma (CA-PTSS)? HYPOTHESE Beide onderzochte behandelingen zijn doelmatiger dan de standaard behandeling. STUDIE-OPZET Multicenter randomized controlled trial STUDIEPOPULATIE 150 CA-PTSS patienten INTERVENTIE Gefaseerde behandeling: 16 wekelijkse sessies: emotieregulatievaardigheden gevolgd door traumagerichte behandeling. Geïntensiveerde behandeling: 14 sessies (3 sessies/week, 4 weken, 2 booster sessies). GEBRUIKELIJKE ZORG 16 wekelijkse sessies traumagerichte behandeling. De behandeling is effectief gebleken, maar leidt tot veel uitval in patienten met CA-PTSS. UITKOMSTMATEN: Primair: PTSS-symptomen. Secundair: responder rate, depressieve symptomen, drop-out, kwaliteit van leven en emotieregulatie. SAMPLE SIZE BEREKENING / DATA-ANALYSE 50 patiënten per conditie, gebaseerd op een effectsize verschil van 0.4. Multilevel logistische regressie en multilevel mixed models. KOSTENEFFECTIVITEITSANALYSE / BIA Kostenutiliteitsanalyse o.b.v. patiëntrapportage (maatschappelijke kosten per QALY) en kostcalculator spreadsheet model (BIA) TIJDSPAD: Maand 1-2: Laatste fase van voorbereiding / METC procedure; research infrastructure Maand 3-18: Inclusie en dataverzameling Maand 19-30: Dataverzameling follow up en data analyse; implementatieplan Maand 31-36: Economische evaluatie, rapportage, publicaties OBJECTIVES / RESEARCH QUESTIONS The aim of this project is to investigate the cost-effectiveness of two innovative forms of trauma-focused therapy for patients with childhood abuse-related posttraumatic stress disorder (CA-PTSD): phase-based therapy (PBT) and intensive trauma-focused therapy (i-TFT). The effects will be assessed post-treatment and after a 6 and 12 months follow-up in an intention-to-treat analysis. Results will be disseminated and included in treatment guidelines. The ultimate goal is to improve quality of care and contribute to treatment innovation for this severely ill target population, while at the same time lowering societal costs. HYPOTHESIS PBT and i-TFT will be more cost effective than standard TFT in adults with CA-PTSD. STUDY DESIGN Multicenter RCT. STUDY POPULATION 150 adult patients with CA-PTSD. INTERVENTION PBT: 8 weekly sessions skills training, 8 sessions TFT. i-TFT: TFT delivered in 12 sessions over the course of 4 weeks, and 2 booster sessions. USUAL CARE /COMPARISON TFT: 16 weekly sessions. OUTCOME MEASURES Primary outcome: PTSD severity at post-treatment. Secondary: responder rate, depression severity, dropout rates, quality of life and emotion regulation. SAMPLE SIZE CALCULATION / DATA ANALYSIS To detect effect size differences in PTSD severity between conditions at 16 weeks of d = 0.4, with alpha = .05 (2-tailed), n = 50 per condition yields a power of 0.8. Primary outcome and dichotomous secondary outcome parameters will be analyzed with multilevel logistic regression. All other continuous secondary outcome parameters will be analyzed with multilevel mixed models COST EFFECTIVENESS ANALYSIS / BUDGET IMPACT ANALYSIS Cost-utility analysis based on patient reports (societal costs per QALY), and cost-calculator spreadsheet model (BIA) TIME SCHEDULE Months 1-2: Finalizing preparation/METC approval; development of the research infrastructure Months 3-18: Inclusion and data collection Months 19-30: Data collection follow up and data analysis; implementation plan Months 31-36: Economic evaluation, report, submission of manuscripts for publication
